Inside this packet you will find important documents that will help the Police, Fire Department or Ambulance personnel should you ever
have a medical emergency in your home or in your vehicle. There are two sections to this packet and both medical information forms
should be filled out in PENCIL. The one for your home should be filled out and placed back into the clear envelope your packet came
in and placed on the inside right of your freezer.
The Arizona State Highway portion of the packet should be filled out and placed in the glove compartment
of your vehicle.
Both pieces should be placed so that the CBS 5 Red Badge E-Pack logo is facing out so that it is
easily identified by emergency first responders.

Cómo usar su paquete
Dentro de este paquete encontrará documentos importantes que ayudarán a la policía, el departamento de bomberos o el

Instrucciones: Después de llenar los formularios, vuelva a

con la tarjeta para las primeras personas en responder, en la
guantera. Las dos calcomanías van en la ventana delantera
derecha y laventana trasera izquierda del vehículo. Éstos tienen

vehículo.
