Appendix A - STATEMENT OF ACKNOWLEDGEMENT

DOCUMENT REQUIRES SIGNATURE BY BIDDER: All Bidders are required to sign and submit
to ADT the Statement of Acknowledgement that follows with Bidders intention of
participation/non- participation of this RFP. This document must be signed by a duly authorized
representative of the Bidder organization and returned to ADT by September 27, 2013 at 4:00
PM.

I/We hereby acknowledge that I/We have read and understood the Request for Proposal (RFP).

Our response adheres to all administrative and other guidelines outlined in the RFP. I/We hereby
acknowledge that we may be disqualified from the RFP process if all guidelines are not followed.

I/We hereby acknowledge that I/We understand that the information and proposal we may
provide will form part of any future contract with ADT.

I/We hereby acknowledge that I/We understand ADT reserves the right to do business with
anyone at any time for any reason without further notice to us.
