$50- Includes a Camp T-shirt. Rates for more than
one in a family are available(2 for $90). A non-
refundable deposit of $20 should be given with the
application. The remainder can be paid at
registration. Check-in at the Middle School Gym
entrance.

dates set forth in this application. In consideration of your acceptance I hereby release Coach Michalik, Tri-
Center High School, and all of their employees from all claims on account of any injuries which may be
sustained by our daughter while attending camp. I also certify that our daughter is medically fit to
participate in camp.
