Application Instructions: This document is set-up with form fields that must be typed on a computer. After filling-in
the blanks and answering the questions, print the document, get signatures where appropriate, and give
recommendation forms to teachers.

I hereby certify that I have been informed of the requirements I must meet and maintain to be a member of the
Student Leadership Team of the ____________ Band.

If appointed to a student leadership position for the __________ school year, I will:
a) Abide by and uphold all of the rules and regulations set forth in the __________ Band Handbook.
b) Uphold the high standards of conduct expected of me as a student leader.
c) Show an exemplary level of character, integrity, and high moral standards.
d) Demonstrate high musical and artistic standards to the best of my ability.
e) Support and attend all band activities and attend all leadership-sponsored activities to the best of my
ability, except when excused in advance by the Director.
f) Willingly attend all band council committee meetings as scheduled by the Drum Majors and/or Directors.
g) Attend meetings or work as needed on leadership activities before school, during school, during lunch,
after school, or when necessary to fulfill the obligations of my student leadership position.
h) Participate in the planning and execution of all student leadership activities.
i) Complete all my obligations as a student leader for the entire school year or forfeit my right to participate
in subsequent years.

I understand that a violation or unwillingness to abide by this contract agreement is sufficient cause for grounding,
suspension, or impeachment from my student leadership position, and/or ejection from the student leadership team.
