Collection of Personal Information


The College of Nurses of Ontario (the College) collects the information in the QA Program Learning Plan Form if you have been selected for Practice Assessment
under the general authority of the Regulated Health Professions Act, 1991, S.O. 1991, c. 18, the Nursing Act, 1991, S.O. 1991, c. 32, and its regulations; and the
Colleges by-laws. The College collects the information for the purpose of assessing your continuing competence through its Quality Assurance Program. Appropriate
measures are taken to safeguard the confidentiality of the personal information you provide and all documents become the property of the College.

How am I going to achieve my goal? Identify a minimum of three (3) learning activites with timeframes/timelines.
I will achieve my goal by April16, 2015: 1. by practicing case scenarios in access lab
2. By practicing online Simulations on SLS
3. Attending seminars at the clinical setting

I will learn more about insulins , get proficient in IV administration.

Goal #2: Activities and Timeframes


How am I going to achieve my goal? Identify a minimum of three (3) learning activites with timeframes/timelines.

1) Review textbooks and nursing skills videos.


2) Practicing with IV and Injection Kits provided to us.
3) Shadowing with the nurse and grabbing more opportunities in the setting.
I will achieve all of these goals by mid of my Pre-Grad which is March 15, 2015. I will be more
confident when administering IVs and insulins on my own.

I will be more confident in catheterization.

How am I going to achieve my goal? Identify a minimum of three (3) learning activites with timeframes/timelines.

1) Review textbooks and nursing skills videos


2) By observing other employees and my peers.
3) Getting clear instructions on how to practice sterile procedures when doing catheterization.
I will achieve the goals, mentioned above by March 25, 2015.

I will continue to practice all of my 3 learning goals and will get more
confident about my practice and will be able to demonstrate it to my
