Instructions and Guidelines For

EDU 1020: Goals Assignment


1. Using the guidelines in Chapter One, create a long-term, mid-term and short-term goal in at least
three categories. Categories might be Health & Fitness, Education, Career, Romance,
Spirituality, Wealth, Travel, Hobbies etc.
2. Each goal should be only one complete sentence.
3. Your short-term and mid-term goal in each category should be benchmarks towards your long
term goal. Your goals in each category should correlate with each other.
4. Your long-term goals should be compatible. The achievement of one of your long-term goals
should not prohibit the achievement of one of your other long-term goals.
5. Your goals must follow the SMART guidelines which are:
Specific (How much? How Many? To what standard? )
Measurable (Can your success towards the goal be effectively determined?)
Achievable (Is the goal something you have control or influence over? Do you have the needed talents,
skills and resources to accomplish the goal or can you get them?)
Relevant (Will it make a difference in your life?)
Time Referenced (by when)

6. Your goals should be positive.
7. Your goals should be personal.

8. Motivational authorities recommend that you write goals as though you have already
accomplished them. Use present tense.
9. It is recommended by some that goals be written using action verbs rather than passive verbs (am,
be).
10. Name each category.
11. Complete the following template and submit it.

1st Category: Obtain My LCSW License by 2020
Long-Term Goal Statement: After receiving my MSW degree from The U of U. I will take the
States licensing exam and receive my LCSW License and work within the field by 2020 year end
Mid-Term Goal Statement: Attend, engage, and pass all required class at SLCC by the summer
of 2016 Then transfer to the University of Utah, (U of U).
Short Term Goal Statement: Attend, and pass all required class at SLCC during the spring 2015
semester. Register for SLCC graduation ceremony. Complete the transfer process to the U of U.

Action Steps: (Identify the concrete things you plan to do to reach your short-term goal in this
category.)
1. Walk daily to school to maintain health, attend all my classes and do the work given.
2. Mindfully taking one step at a time successfully completing each stage, independently.
3. Ask questions, use the recourses provided by SLCC, colleges, and personal instincts.
4. Study, research, read, and focus on remember as much of everything I learn, as I can.
5. Return thanks by giving my time, knowledge, and abilities to others that may have need.

Narrative Statement: How will your life look when you have achieved this long-term goal?
This will help you determine relevance and establish motivation.Through the epic experience of living the
life as of a dedicated full time college student, and effectively passing each class, that within itself will
have successfully taught me its overall targeted goal of teaching all a general knowledge with a maximum
learning experience possible.
What obstacles will you need to overcome or what are you willing to sacrifice to achieve your long-term
goal? (This step will help you determine achievability.) As a prior professional over the road truck driver,
I had to sacrifice the daily activities of my wife and children. So I could aid in my half of the
responsibilities as a parent, and too meet the needs of my children and my wife. Now that Ive returned to
school, looking yet again for another occupation, I sacrifice living a different lifestyle. I sacrifice having
the money I once had while working, however, now that Im a fulltime student thats able to work
because of furthering education, and to obtain a degree, the rights to have and posses the title of another
professional, this time in the mental/medical field.
Anther obstacle I must overcome is, to constantly stay on top of my assignment currently given to
me by each instructor, and receive a reasonable grade according to the work handed in.
Preferably, receiving an (A) and holding a 4.0 GPA would be Preferred, however, much I give up
regardless if its being homeless or simply hungry I still need to stay focused on making it happen and
endure until the end has been achieved.

I deserve this goal because: (This step will help you bolster your motivation.)
1. Successfully reaching my goal, means I can legally help others live a more balanced life and
happier.
2. Because it is my true calling. My true destiny. The true thing, that I feel fits me the way I
naturally. Helpful and always willing to share personal adversities that were successfully
closed in my perspective in a positive way, that educated me somehow in some way.

2nd Category: Taber

T. Fellows Clinical Psychologist Ph.D.

Long-Term Goal Statement: To Provide Clinical psychology services for Children, Adolescents,
Adults and Families. That suffers from whatever that may be. By the end of 2021
Mid-Term Goal Statement: Successfully obtain an A.S. Degree in psychology/Social Work on or
around the summer 2016 from SLCC.
Short Term Goal Statement: Continue to successfully pass each class, and semester until mid
and long term goals have been reached, starting with this current semester, spring 2015.

Action Steps: (Identify the concrete things you plan to do to reach your short-term goal in this
category.)
1. Doing daily homework and turn-in/submit, every given assignment on time or before.
2. Meditate daily to teach myself patience, and how to focus on one thing at a time.
3. Volunteer time as a mediation instructor at the Salt Lake Juvenile Delinquent Center.
4. Eat healthy and stay physically active and maintain longevity.
5. Smile, find & identify a thoughtful jester someone did or you did for someone.

will help you determine relevance and establish motivation.) Meeting and greeting new clients. Engaging
in their treatment program and provide a stable source of recourses that aid success in of my each
clients needs. File and complete assessments
of new clients and continue to further my education, by
studying new findings as they come out.
What obstacles will you need to overcome or what are you willing to sacrifice to achieve your long-term
goal? (This step will help you determine achievability.) Giving up some personal activities like
internment in order to study and or assist a client in need. Spend time and money to help provide
emergence help to clients or others in need that I can help.

3rd Category: Work as a social worker at the Utah department of work force services by 2020.
Long-Term Goal Statement: After receiving my MSW degree from The U of U. I will take the
States licensing exam and receive my LCSW License and work within the field by 2020 year end
Mid-Term Goal Statement: Satisfactory pass all class, graduate and receive an AA/AS degree
diploma as a Social Worker by the summer of 2016.
Short Term Goal Statement: Continue to successfully pass each class, and semester until mid
