The members daughter and wife were very concerned. The
member walked away from his family at the dinner table on
Easter Sunday because he said he was not hungry and wanted to
sit by himself in the other room.
I discovered he had a history of strokes. He recently had a fall that
was still causing him back pain. In reviewing his medication
profile, I found that among his medications, he was taking a heart
medication and blood thinner as well as self-medicating with
ibuprofen for his back pain.
I advised him to stop taking the ibuprofen because of the
potential interaction with a couple of his medications. I called his
primary care provider and made an appointment for him to be
evaluated. I also placed a care management referral.
In following up with him a few days later, the member said his
doctor added a medication for pain relief that did not have any
potential for interaction with his other medications. Additionally,
the doctor ordered X-rays of his back and hips and prescribed an
antidepressant. A follow-up visit to the doctor was scheduled. The
member also noted, a care manager had called him to become
involved in his care.
