Fraktur metatarsal
Fraktur terisolasi dari metatarsal tunggal tidak umum pada anak-anak. Lebih umum adalah
fraktur beberapa metatarsal, biasanya hasil cedera menghancurkan seperti benda berat jatuh di
kaki anak. Arteri dan vena lokal biasanya terluka juga. Menyetel kembali-ment dari metatarsal

Fraktur Os calcis
Pada anak-anak, tulang cancellous dari calcis os relatif tahan terhadap fraktur. Neverthclcss,
naksir atau kompresi jenis offiracture dapat terjadi ketika seorang anak jatuh dari ketinggian

Tibialis epiphysis
Cedera inversi parah ofthe pergelangan kaki mungkin procduce tipe IV intra-artikular patah

Mayoritas patah poros tibialis di children relatif undisplaced dan ini dapat dijelaskan sebagian

The Lutut dan Paha

Avulsion Fraktur Spine anterior tibialis

Diagnosa
Pemeriksaan fisik mengungkapkan lutut terlalu bengkak karena hemarthrosis kotor. Patela bisa

Komplikasi
Dislokasi berulang patela merupakan komplikasi troublesome cedera ini (Gambar. 16,74).

Diagnosa
Diagnosis jelas dari examination klinis saja karena kelainan khas (Gambar. 16,75). Karena

Pengobatan
Selama dekade terakhir, pengobatan fracturcs dari poros femoralis pada anak haschanged
dramatis, alasan utama adalah pengurangan jumlah tempat tidur rumah sakit dan kebutuhan
untuk menggunakan metode pengobatan yang memungkinkan anak untuk dibuang dalam
beberapa hari lebih dari beberapa minggu. Mantan metode traksi selama beberapa minggu diikuti
dengan pinggul Spica cast adalah aman dan suara, tetapi harus diganti dengan alasan

3. Lama Dari 10 Tahun. Setelah period singkat di traksi, poros fraktur femoralis diperlakukan

Untuk alasan dibahas di awal bab ini, pertumbuhan berlebih sementara selalu terjadi setelah
fraktur pengungsi dari poros femoralis. Jumlah rata-rata pertumbuhan berlebih adalah 1 cm dan}

Komplikasi
Komplikasi yang paling serius dari patah tulang femur poros pada anak-anak adalah Volkmann
ischcmia (sindrom kompartemen) dari saraf dan otot karena baik untuk femoralis kejang arteri
atau perdarahan dan edema dalam kompartemen jaringan lunak. Kejang, yang pada gilirannya
dapat menjadi sekunder untuk air mata dari intima, diperparah oleh traksi yang berlebihan pada
ekstremitas retak. Manifestasi klinis yang akan datang iskemia Volkmann di ekstremitas bawah

Volkmann's ischcmia. Associated compartment syndromes must be treated urgently by fasciotomies. The permanent effects of Volkmann's ischemia (compartment syndrome) and
subsequent Volkmann's ischemic contracture are tragic (Fig. 16.82).

Complications
If internal skeletal fixation has not been used or if it has been inadequate, fractures of the femoral
neck in children are likely to be com-plicated by nonunion and a progressive coxa vara deformity
(Fig. 16.88).

When the femoral head has lost its blood supply by disruption of its vessels at the time of a
fracture, the result is posttraumatic anascular necrosis, a complication that occurs in
approximately 30% of children with this in jury. There is little radiographic evidence of this
complication until several months have clapsed. The ossific nucleus stops growing for at least 6
months after the injury and at first appears relatively radiopaque (relative to the posttraumatic
osteoporosis of the living bone in the acetabulum and femoral shaft). Later, when the ossific
nucleus is being revasculari2ed and reossified, it appears absolutely radiopaque as new bone
is laid down on dead trabeculae. Subsequently, the femoral head may become deformed, as
described in the section on Lcgg-Perthes' disease in Chapter 13. The treatment of the
complication of posttratunatic avascular necrosis of the femoral head in children is the same as
that previously described for Legg-Perthes' disease in Chapter 13.

Diagnosa
The clinical deformity of a posterior dislocation of the hip-flexion, adduction, and internal
rotation-is characteristic (Fig. 16.90). Traumatic anterior dislocation of the hip is rare in
childhood but when it does occur, the hip is held in the opposite position-extension, abduction,
and external rotation. Postcrior dislocation is obvious radiographically (Fig. 16.91).

Pengobatan
As long as the hip is dislocated, the torn capsule and surrounding structures constrict the
femoral neck vessels and jeopardize the blood supply to the femoral head. For this reason,
traumatic dislocation of the hip represents an emergency. The dislocation should be re-duced as
soon as possible to present the serious complication of avascular necrosis of the femoral head.
Indeed, in children whose hips are re duced as soon as possible to present the serious
complication of avascular necrosis of the femoral head. Indeed, in children whose hips are
reduced within 8 hours from the time of injury, tile incidence of avascular necrosis is low,
whereas in those whose hips have remained unreduced for longer than 8 hours, the incidence of
this complication is high (approximately 40%).
Closed reduction is accomplished by applying upward traction on the flexed thigh and forward
pressure on the dislocated femoral head from behind. After reduction, which must be perfect both
clinically and radiographically, a hip spica cast is applied with the hip in its most stable
position-extension, abduction, and external rotation. Immobilization of the reduced hip is
maintained for 6 weeks to allow strong healing of the torn capsule.

Complications
The acetabular margin, being largely cartilaginous in children, is seldom fractured, and the
sciatic nerve is seldom injured. The complication of posttraumatic avascular necrosis of the
femoral head has been described above in relation to fractures of the femoral neck.
The longer the hip remains dislocated, especially after 8 hours, the higher the incidence of this
complication.
Soft tissue interposition of capsule or labrum in the joint prevents perfect reduction. The
residual subluxation leads to degenerative arthritis; such soft tissue interposition, often best
detected by CT, should be removed surgically.
Panggul
The pelvis of a child is more flexible and more yielding than an adult's because of the
cartilaginous components at the sacroiliac joints, triradiate cartilages, and symphysis pubis.
Consequently, serious fractures of the pelvis arc not common in childhood, but they do occur as
the result of a severe injury such as occurs in an automobile accident.
The most important aspects of fractures of the pelvis in children arc not the fractures themselves
but rather the associated complications-extcnsive internal hemorrhage from torn vessels and
cxtravasation of urine from rupture of(hc bladder or urethra.
Diagnosa
Physical examination reveals local swelling and tenderness, and in unstable fractures there may
also be deformity of the hips as well as instability of the pelvic ring. Special radiographic
projections are required to assess the precise nature of a pelvic fracture because the
anteroposterior projection provides only a two-dimensional concept of the injury. The lateral
projection, which would normally provide the third dimension, is unsatisfactory because of
overlap of the two innominate bones. Thus, to obtain a three-dimensional concept of the
disturbed anatomy of the injury, it is necessary to obtain: 1) an anteropostcrior projection; 2) a
tangential projection in the plane of the pelvic ring (with the rube directed upward 50 degrees);
3) an inlet projection look-ing down into the pelvic ring with the tube directed downwards 60
degrees. Computed tomography is useful in obtaining a three-dimensional appreciation of the
precise sites of the fractures and the position of the fragments in the pelvis, including the
acetabulum.
Pengobatan
The emergency care ofa child with a fractured pelvis centers on the two major complications.
The pelvis is a parricularly vascular area, and displaced liactures of the pelvis may tear vessels
(such as the large superior gluteal artery
in the sciatic notch) with resultant major hemorrhage. A child may lose as much as 60% of
circulating blood volume into the peripelvic and retroperitoneal tissues, resulting in severe
hemorrhagic shock. The recognition and treatment of shock have been discussed in Chapter 15.
While the child's shock is being treated, a catheter should be inserted into the bladder to
investigate the possibility of associated injury to the bladder or urethra. If there is blood in the
urethra and a catheter cannot be passed, the urethra is almost certainly torn. Hence a suprapubic
cystotomy must be performed pending surgical repair of the urethra. If the catheter can be passed
into the bladder and the urine contains blood, a cystogram should be carried out immediately to
determine if the bladder has been ruptured, in which case it should be repaired as soon as
possible.
Because the boric of the pelvis is principally of the cancellous type, and because its blood supply
is abundant, fractures of the pelvis unite rapidly. Treatment of the various types of fractures is
aimed at correcting significant tiacnu-c dc(brmirics to prevent malunion and rcsultznt
disturbance of function.
Stable Fractures of the Pelvis
Fractures that do not transgress the pelvic ring do not interfere with stability of the pelvis in
relation to weightbearing and do not require reduction.
In children, particularly in athletic boys, a sudden violent pull on the hamstring muscles may
avulse their origin, the ischial apophysis. This injury usually heals well but may result in a
fibrous union.
Isolated fractures of the ilium are of little significance and require only protection from
weightbearing until pain subsides within a few weeks.
A "straddle" injury of the pelvis (which may occur as a child loses his or her footing while
walking along the top of a fence) may cause one or more fractures of the inferior pubic rami but,
more importantly, is likely to produce a tear of the urethra.
Unstable Fractures of the Pelvis
Complete separation of the symphysis pubis and opening out of the pelvic ring is best re duced
by internally rotating both hips. The reduction is maintained in a well-molded hip spica cast (Fig.
16.92).

Lateral compression of the pelvis may produce a "bucket-handle" fracture in which the
fractured half of the pelvis rolls forward and inward (Fig. 16.93). In children, this type of fracture
can usually be managed by externally rotating the lower limb, and the reduction can be
maintained by the application of a wellmolded hip spica cast.
Unstable fractures in which one half of the pelvis is driven proximally by an upward thrust
require continuous skeletal traction through the femur to obtain and maintain reduction.
Alternative methods of treatment for complex and markedly unstable fractures of the pelvis as
well as those that involve the acetabulum include external skeletal fixation and open reduction
and internal fixation with plates.

CHILD ABUSE
Distasteful and difficult to understand as it may be, the tragic truth remains that some infants and
small children are, in fact, physically abused within their own homes by a disturbed parent, an
older brother or sister, or even a baby sitter. Such child abuse tends to be repeated and often
results in multiple mus-culoskeletal injuries, frequently referred to by the sickening synonym
"battered baby syndrome," a repulsive yet realistic term. A more euphemistic synonym is
"nonaccidental injury"
Fractures in children under younger than 3 years are not very common, but it has been estimated
that at least 25% of fractures in this age group are caused by child abuse.
Diagnosa
The victim ofsuch pathological behavior may not be brought for medical attention immediately.
When the child is brought, the history of injury given by the parents is often vague and may even
be deliberately misleading. There is usually something mysterious about the mishap in that the
severity of the injury or injuries is incompatible with a history of a simple fall and this should
arouse your suspicion.
Physical examination may reveal multiple bruises, often in varying stages of resolution, which
suggests multiple assaults over time. The child usually has a sad countenance-and for good
reason (Fig. 16.94).

Further examination revealed multiple bruises in various stages ofrcsolution over the girl's trunk
and limbs. These physical findings suggest repeated assaults.
Radiographic and scintigraphic (bone scan) examinations under such circumstances need to be
extensive and should include the skull, chest, and all four limbs. Skull fractures, multi ple rib
fractures, "corner" metaphyseal fractures (close to the epiphyscal plate), epiphyscal
separations (rarely), and pcriostcal new bone formation in the limbs are the most characteristic
skeletal injuries. These multiple fractures may also be in varying stages of healing, an
observation that usually indicates repeated assaults (Fig. 16.95). These musculoskeletal injuries
from child abuse are often forerunners of more serious injuries. Magnetic resonance imaging is
useful in the assessment ofany associatcd soft tissue injuries.

Pengobatan
Infants and children suspected of having been physically abused should be admitted to hospital
for complete investigation (as well as photographic documentation). The physician or surgeon
who suspects child abuse has a moral, and legal, obligation to report the suspicion of such
abuse to the local authorities, including a Suspected Child Abuse and Neglect (SCAN) Program
and the police, who then proceed with the necessary investigation and action. Records of
previous attendance at the hospital should be studied. Indeed, if a central registry of physically
abused children is kept in the community, this should also be consulted because the parents may
not consistcntly bring their child to the same hospital, particularly in a large community.
Sadly, if these protective and preventive steps are not taken, a significant percentage of these
helpless and hapless little children will eventually succumb to multiple injuries of increasing
scvcrity-particularly potentially fatal cerebral injuries-that arc wittingly and willfully being
inflicted upon them.
