I Manpreet Kaur aged 23 years D/o S. Surinder Singh R/o V.P.O. Khema Khera,
Tehsil Malout, Distt. Sri Muktsar Sahib, do hereby solemnly affirm and declare as
under :1.

That I passed my 10+2 in Medical Stream from Saint Fateh Singh Convent
School, Maur Mandi in the year March 2011.

That thereafter passing my 1o+2 I appeared in B.H.M.S. Course at

Chandigarh College.

3.

That besides above B.H.M.S. Course I did not appeared for any other
education after passing my 10+2.
