Grinder Pre-Use Inspections


Incident Alert - Worker Cuts Hand while using a Grinder
A 1st year apprentice was grinding a weld when the grinding disc made contact with the knuckle of his left hand.
The contact resulted in the worker receiving 4 stitches to close the wound. (26 Feb 13)

Investigation Findings:
The grinder had been visually inspected by the worker prior to use and on quick examination appeared to be
in satisfactory working condition. The Guard was in place and in the correct position, the handle was
attached and the cord and trigger were in good condition. While performing grinding duties, the guard
slipped exposing the workers hand to disc.
The investigation identified that the slotted groove of the bearing housing which holds the guard at an even
plane on the housing had been worn down preventing proper engagement of the keeper latch. The guide tab
on the grinder guard had also been worn down allowing the guard to move out of position

Corrective Actions:
All grinders are to be immediately inspected by a competent worker and all pre-use inspections going
forward must include:
Look for loose or missing screws on the housing or keeper and guard assembly.
Ensure the keeper latch for the guard is operative and locks. Test that the guard is locked from
movement.
Inspect the guide tab on the guard assembly for excessive wear.
Perform a test run of the grinder prior to use to feel for vibration and listen for signs wear.
If a worker feels any vibration at the startup of a grinder, or notices any lose or missing parts (eg. screws) the
equipment is to be immediately removed from service, quarantined, inspected and properly repaired prior to
being returned to service. If the unit cannot be repaired it is to be permanently removed from service.
Additional recommendations:
Have a competent worker facilitate a Pre-Use Grinder Inspection session at your toolbox or safety
meeting
Refer to and review the THA for Grinder Operation
Create a schedule for all grinders being utilized to be inspected monthly by a qualified person
