Fistulas can arise in any part of the body, but they are most
common in the digestive tract. They can also develop
between blood vessels and in the urinary, reproductive, and
lymphatic systems. Fistulas can occur at any age or can be
present at birth (congenital). Some are life-threatening,
others cause discomfort, while still others are benign and go
undetected or cause few symptoms. Diabetics, individuals
with compromised immune systems (AIDS, cancer) and
individuals with certain gastrointestinal diseases (Crohn's
disease, inflammatory bowel disease) are at increased risk
of developing fistulas.

Skrofuloderma:
Tuberkulosis kutis murni sekunder yang terjadi secara
pekontinuitatum dari jaringan di bawahnya, misalnya kelenjar
getah bening, otot dan tulang. Skrofuloderma terjadi terutama
pada anak-anak dan dewasa muda pada bagian kulit yang
berada diatas nodus limfatikus dan daerah yang kelihatan
tulangnya. Perjalanan penyakit termasuk keluhan utama dan
keluhan tambahan. Dimulai dengan infeksi sebuah kelenjar
yang selanjutnya menjadi berkembang menjadi periadenitis.
Beberapa kelenjar kemudian dapat meradang, sehingga
membentuk suatu kantong kelenjar klier packet. Pada
stadium selanjutnya terjadi perkejuan dan perlunakan, mencari
jalan keluar dengan menembus kulit diatasnya, dengan
demikian terbentuk fistel. Fistel tersebut kian melebar,
membentuk ulkus yang mempunyai sifat-sifat khas.

4. Mengapa dokter menyarankan untuk tes Tuberkulin?
Tes Tuberkulin:
Pada anak, uji tuberkulin merupakan pemeriksaan paling
bermanfaat untuk menunjukkan sedang/pernah terinfeksi
Mikobakterium tuberkulosa dan sering digunakan dalam
"Screening TBC". Efektifitas dalam menemukan infeksi TBC
dengan uji tuberkulin adalah lebih dari 90%.
Penderita anak umur kurang dari 1 tahun yang menderita
TBC aktif uji tuberkulin positif 100%, umur 12 tahun 92%, 24
tahun 7
8%, 46 tahun 75%, dan umur 612 tahun 51%. Dari
persentase tersebut dapat dilihat bahwa semakin besar usia
anak maka hasil uji tuberkulin semakin kurang spesifik.
Ada beberapa cara melakukan uji tuberkulin, namun
sampai sekarang cara mantoux lebih sering digunakan. Lokasi
penyuntikan uji mantoux umumnya pada bagian atas lengan
bawah kiri bagian depan, disuntikkan intrakutan (ke dalam
kulit). Penilaian uji tuberkulin dilakukan 4872 jam setelah
penyuntikan dan diukur diameter dari pembengkakan
(indurasi) yang terjadi.

DEFINISI
Uji tuberkulin (tuberculin skin test/TST) merupakan alat
diagnostik yang sampai saat ini mempunyai sensitivitas dan
spesifisitas cukup tinggi untuk mendiagnosis adanya infeksi

Hasil tes Mantoux dibaca dalam 48-72 jam, lebih
diutamakan pada 72 jam
Catatan:
Reaksi hipersensitivitas terhadap tuberkulin yang munculnya
cepat (immediate hypersensitivity reactions) dapat timbul
segera setelah suntikan dan biasanya menghilang dalam 24
jam. Hal ini tidak mempunyai arti dan bukan menunjukkan
hasil yang positif.
INTERPRETASI TEST MANTOUX
Tes Mantoux dinyatakan positif apabila diameter indurasi >
10 mm. Kemungkinan yang perlu dipikirkan pada anak
dengan hasil tersebut:
a.
Terinfeksi tuberkulosis secara alamiah
b.
Infeksi TB mencakup infeksi TB laten, sakit TB aktif, atau
pasca terapi TB.
c.
Pernah mendapat imunisasi BCG (pada anak dengan
usia kurang dari 5 tahun)
d.
Pada pasien usia kurang dari 5 tahun dengan riwayat
vaksinasi BCG kecurigaan ke arah infeksi alamiah TB bila hasil
uji Mantoux > 15 mm.
e.
Infeksi mikobakterium atipik
Meskipun demikian, hasil uji Mantoux > 5 mm dapat
dipertimbangkan positif pada pasien tertentu seperti :
a.
Pasien dengan infeksi HIV
b.
Pasien dengan transplantasi organ atau mendapat
imunosupresan jangka panjang seperti pasien keganasan atau
sindrom nefrotik
False Negative
Pasien-pasien tertentu yang terinfeksi tuberkulosis mungkin
dapat menunjukkan hasil tes Mantoux yang negatif. Kondisi
demikian disebut dengan anergi. Anergi kemungkinan terjadi
pada pasien:

Polymerase chain reaction is a new tool in diagnosing atypical
mycobacterial infections and can even be performed on tissue
specimens of the standard formalin-fixed paraffin-embedded type. [36]
The purified protein derivative test result is usually negative in
infections with atypical mycobacteria.
Medical Care
Infections with atypical mycobacteria can be treated with a variety
of antibiotics. Clarithromycin has shown good efficacy against a
broad range of atypical mycobacteria, but some organisms are
resistant, and proper sensitivities must be obtained.
Effective treatment of M kansasii infection can usually be
accomplished with a rifampin-based regimen, or a rifabutin-based
regimen can be used for patients who are HIV seropositive and
receiving antiretroviral therapy.
Jousse-Joulin et al[37] described skin and joint infection by M
chelonae treated with rescue treatment with interferon gamma.
Han et al[38] analyzed clinical and microbiologic features of 115 cases
involving rapidly growing mycobacteria isolated at the University of
Texas M.D. Anderson Cancer Center from 2000-2005. Antimicrobial
susceptibility test results demonstrated that M abscessus was the
most resistant species and thatMycobacterium mucogenicum was
most susceptible.
Surgical Care
A combined therapeutic approach, including surgical drainage,
debridement, and prolonged (>3 mo) treatment with combined
antimicrobial agents, has been used in some cases of atypical
mycobacteria.
In some cases based on clinical assessment, successful treatment
requires aggressive debridement of all infected subcutaneous
tissues and skin.
Treats infections caused by susceptible gram-negative and grampositive organisms, in addition to infections caused by
susceptible Chlamydia,
Rickettsia,and Mycoplasma species

Tuberkulosis kutis adalah tuberkulosis pada kulit yang di
Indonesia disebabkan oleh Mycobacterium tuberculosis dan
mikobakteria atipikal.

skrofuloderma merupakan bentuk yang tersering yang didapat
(84%), disusul tuberkulosis kutis verukosa (13%), bentukbentuk yang lain jarang ditemukan. Lupus vulgaris yang
dahulu dikatakan tidak terdapat ternyata ditemukan, meskipun
jarang. Bentuk tersebut dahulu merupakan bentuk yang
tersering terdapat di negeri beriklim dingin (Eropa). Di Amerika
Serikat sejak dahulu jarang terdapat tuberkulosis kutis.
Lupus vulgaris
Lupus vulgaris merupakan bentuk yang sering dan mengenai
terutama pada bagian yang sering terpapar misalnya pada
wajah dan ekstremitas. Cara infeksi dapat secara endogen
atau eksogen. Gambaran klinis yang umum adalah kelompok
Liken skrofulosorum
Lesi biasanya terjadi di daerah leher pada anak yang
menderita tuberkulosis tulang atau nodus limfatikus. Kelainan
kulit terdiri atas beberapa papul miliar, warna dapat serupa
dengan kulit atau eritematosa. Mula-mula tersusun tersendiri,
kemudian berkelompok tersusun sirsinar, kadang-kadang di
sekitarnya terdapat skuama halus. Tempat predileksi pada
dada, perut, punggung dan daerah sacrum. Perjalanan
penyakitnya dapat berbulan-bulan dan residif, jika sembuh
tidak meninggalkan sikatriks
Eritema nodusum
Kelainan kulit berupa nodus-nodus indolen terutama pada
ekstremitas bagian ekstensor. Diatasnya terdapat eritema.
Banyak penyakit yang juga dapat memberi gambaran klinis
sebagai Eritema Nodusum., yang sering: lepra sebagai eritema
nodusum leprosum, reaksi yang terjadi karena Streptococcus B
Hemolyticus, alergi obat secara sistemik, dan demam
reumatik.
Eritema induratum
Eritema induratum adalah suatu peradangan kronis dari
pembuluh darah arteri dan vena bersifat jinak, dan disertai
nekrosis lemak. Kelainan kulit berupa nodus-nodus indolen.
Tempat predileksinya pada daerah fleksor. Terjadi supurasi
sehingga
terbentuk
ulkus-ulkus.
Kadang-kadang
tidak
mengalami supurasi, tetapi regresi sehingga terjadi hipotrofi
berupa lekukan-lekukan. Perjalanan penyakit kronik residif.

Papulonecrotic Tuberculids
Background
The tuberculids, first described by Darier in 1896, represent a form
of cutaneous hypersensitivity reaction to tuberculosis (TB) antigens.
[1]
Although many types of tuberculids have been described, most
are now understood to not be uniquely caused by tuberculosis.
However, papulonecrotic tuberculids (PNTs) and lichen scrofulosorum
are still widely accepted as true tuberculids.
The entity papulonecrotic tuberculid was first established by Pautrier
in 1936 as a distinct tuberculosis-associated disorder, when he
described the characteristic clinical and histopathologic features.
Papulonecrotic tuberculid is a chronic, recurrent, and symmetric
eruption of necrotizing skin papules arising in crops, involving
primarily the arms and the legs. A hallmark of this condition is that
lesions heal with varioliform scarring. The eruption is believed to
represent a hypersensitivity reaction to tuberculosis antigens
released from a distant focus of infection. Most patients react
markedly to the Mantoux skin test (purified protein derivative, PPD)
and may exhibit other evidence of current or past tuberculosis
infection.
Pathophysiology
The pathophysiology of papulonecrotic tuberculid is controversial.
Most authors believe this disease entity is triggered by an initial
Arthus reaction to mycobacterial antigens. This is then followed by a
hypersensitivity reaction in which antigens undergo opsonization by
antibodies, followed by immune complex deposition in small
cutaneous blood vessels. The ensuing complement cascade triggers
a leukocytoclastic vasculitis, leading to destruction of vessel walls
with ensuing tissue necrosis. However, other authors dispute this
mechanism, citing the lack of leukocytoclastic vasculitis in some
cases. Instead, they propose that the primary lesion is the result of
subacute lymphohistiocytic vasculitis that leads to thrombosis and
subsequent tissue necrosis.
Whatever underlies the pathophysiology, a consensus has been
reached that papulonecrotic tuberculid represents a true
hypersensitivity reaction rather than the result of a local cutaneous
tuberculosis infection. This is based on the observation that
papulonecrotic tuberculid lesions have consistently failed to either
stain positive for, or culture out, mycobacterial organisms. Although
the organisms are absent, mycobacterial DNA have been detected in
approximately half of the biopsy specimens subjected to polymerase
chain reaction. These observations support the idea that lesions of
papulonecrotic tuberculid are the result of released mycobacterial
antigens in the setting of a concurrent but distant infection.
Epidemiology

A decreasing incidence of papulonecrotic tuberculid began in the
second half of the 1900s and the decline continues to this day. This
phenomenon is attributed to aggressive tuberculosis control now
found in wide practice. Currently, almost all cases come from areas
outside of North America with high endemic rates of tuberculosis.
However, a rare US case, involving a previously healthy 9-year-old
girl from Chicago, Ill, was reported in 1990. [2] Historically, reports
indicate that young women and children are especially susceptible
to this disorder.
History
The
characteristic
lesions
are
small,
erythematous,
inflammatory papules that undergo central ulceration and heal
spontaneously within weeks, leaving a varioliform scar.
Lesions arise in symmetric crops, typically with an acral
predilection. Characteristically, lesions develop over the extensor
surfaces, particularly the knees, the elbows, and the dorsum of the
hands and the feet, although widespread involvement may be
present.
New lesions form as older lesions resolve, giving the eruption a
polymorphous appearance.
Oral lesions have not been reported to date.
Papulonecrotic tuberculids have been reported to coexist with
lesions of erythema induratum,[8] as well as lichen scrofulosorum. [9] In
several reports, cutaneous lesions resolved with appropriate
antituberculosis therapy.
Physical
Primary lesion: The characteristic initial lesions are 2- to 8-mm,
erythematous papules that become pustules and undergo central
ulceration forming hemorrhagic-crusted papules. See the image

Bilaterally symmetric papulonecrotic lesions on the
lower extremities.
Causes
The eruption is a form of an exaggerated host immunologic response
to a mycobacterial infection involving the cutaneous vessels. Active
tuberculosis is reported in as many as 40% of patients.

The goal of pharmacotherapy is to eradicate the underlying
mycobacterial infection that causes papulonecrotic tuberculid.

Class Summary
These agents are effective in the treatment of mycobacterial
infections. Therapy with a 4-drug combination of rifampin, isoniazid,
pyrazinamide, and ethambutol (RIPE) is recommended to avoid the
development of resistance.
