General Focus of the Plan:
The general focus of the plan is to increase students individual responsibility when it
pertains to assignments: knowing what the assignments are, when the assignments are
due, prioritizing the assignments, estimating how much time it will take to complete each
assignment, and the completion of assignments in a timely matter.
Specific Goals in relationship to G-Hs Comprehensive School Improvement Plan
and District goals:
My plan does not directly relate to the reading, math, or science district goals, but
promoting the growth of individual responsibility (executive skills) should increase learning
in all areas. Students who complete assignments and study for tests in a timely manner
tend to perform at a higher level.
Rationale for the Plan:
What student learning will this Plan address and what baseline data will be used
to analyze the progress?

My plan focuses on the students gaining various strategies they can use to complete
assignments in a timely manner within the academic and community settings. Specific strategies
and tools will be utilized based on individual student preferences and needs (i.e. MyHomework
app, Executive Skills, assignment book, Google Calendar, etc.)
The baseline data will be provided using an average of the students performance during
the second semester of the 2014-2015 school year. This data will be obtained through the weekly
missing/late assignment documents I have kept. This data will be used for both the long-term
goal and to analyze the progress of my goal.
Spring 2014-2015 baseline data:
The average number of missing assignments from 3 consecutive weeks in April for nine
students is 46 missing/late assignments. For this plan, each individual students assignment
completion percentage will be monitored.

How does your Plan relate to G-Hs annual/district goals (APR, CSIP, Essential
Learnings)?

My plan relates to the G-H annual/district goals by helping to produce:

Effective communicators who can adapt techniques to meet their needs as well as
the needs of their audiences
Collaborative workers who respect and appreciate individual and group
differences and demonstrate high standards, effective work habits, and strong
work ethic
Productive citizens who value and respect diversity and the unique qualities of
others
Critical Thinkers who assess the value of information to solve problems and make
educated choices and decisions
Self-Directed Learners who develop their own unique talents and abilities and
prepare themselves for careers

How will the strategies of this Plan lead to accomplishing these goals?

The strategies of my plan (which Jody Albertson from AEA 267 has offered to help me
establish) will help accomplish the goal by not only providing the students with the means to
successfully identify, prioritize, and complete assignments; it will also promote self-advocacy
and responsibility.

Individual Career Development Plan


The Individual Career Development Plan allows the teacher to develop, implement, monitor, and evaluate
progress toward individual outcomes, which are specific to your unique professional needs.

Initial Conference
During this time, we will be discussing your desired outcomes for the year. We
will discuss steps to achieve outcomes, resources needed, timelines, indicators
of progress, and evidence of achievement.

Mid-Year Conference
Informal observations will take place during the year to assess progress toward the
desired outcomes. A mid-year conference and observation discussions will be used as a means of
communication between you and the principal. The conferences will include discussion of progress
toward outcomes. Mid-Year self-reflection sheet to be completed prior to the conference.

Final Conference
