FATALITY DUE TO ELECTRICAL SHOCK

Date of Incident: 12th September 2012


Time of Incident: App. 05.15 am
Location: Oil and Gas Construction Project.

A fatal incident occurred involving a Non-Destructive Test (NDT) personnel. The


deceased was electrocuted while adjusting the position of portable lighting pole
connected with trailing cable to distribution board which is 50 meters away. Medic
at site performed CPR and brought the victim to site clinic and later taken to
hospital where the Medical Doctor pronounced him dead.
Preliminary finding shows that there was electricity leaking along the cable
connecting to the lighting pole and that a few connections of the trailing cable
which partly placed on wet ground.
The project team immediately Stop Work and all temporary

were inspected.

Recommendations
Ensure all safety precautions in handling electrical equipment are in place,
among others:
 Trailing cables, temporary lighting and its connections are inspected
on regular basis
 Electric Leakages Circuit Breaker (ELCB) are rated accordingly and
inspected on regular basis
 Assign person as a Coordinator for night work
 Conduct awareness training on electrical safety including rescuing
electrocuted victims
