Since my last visit, it seemed some memory changes were setting
in for the member.
When I asked to see her medications, she said she had been out
of them since the blizzard, which was about a month ago! She
asked if I would contact her pharmacy.
The prepopulated medication list on my tablet indicated she was
on medications for hypertension, hyperlipidemia and
depression/anxiety.
In speaking with the pharmacy, I verified the medications and
discovered all were eligible for refill. I ordered them for the
member to pick up that afternoon. Quite predictably, on exam I
found her blood pressure to be very high, but she was
asymptomatic.
I called her primary care provider and arranged for her to be seen
that afternoon. A case management referral was also submitted
as the member would benefit from information on mail order
prescription services from the health plan to help prevent this
from happening again.
I educated the member on the risks of uncontrolled blood
pressure and the importance of taking the medication as directed.
I also suggested she get in a routine of checking her medication
supplies when she hears weather alerts in order to not run out.
The member was very happy for all the help and said she looked
forward to seeing me again next year.
