1. The host and host’s parent are responsible for the behavior of the guest. There are
consequences for not following the rules. (Security Strictly Enforced!!)
2. Picture identification and tickets for BOTH Host and Guest will be checked at the door or
no admittance. NO EXCEPTIONS!!!
3. Identification/ticket stubs for both Guest and Host must be produced during the dance if
requested by supervising personnel. If identification cannot be produced, the individual will
be directed to leave the dance.
4. White outs or alterations to this form is not acceptable.
5. Guest must enter with sponsor.
6. Sponsors must buy guest ticket when sponsor buys his/her own ticket.
7. Once you leave the dance, you are not allowed to re-enter. NO IN AND OUTS!!
8. NO ONE OVER 21 YEARS OLD ALLOWED!
9. Any student falsifying this document is subject to consequences including suspension.
10. Attire must be appropriate for the dance.
