Oladapo demonstrated this outcome by adhering to the policies and procedures of the agency and by ensuring that he follows the uniform
policies by not wearing his uniform to the practice setting. He kept his name tag visible at all times to indicate his status as a student. As
well, he demonstrated Ethical Standards by not imposing his values on his residents and by keeping his relationship professional. He
maintained safe environment by keeping the rooms free from clutters and by keeping the side rails up while the residents were in bed

patient that i know my colleague had the previous week, i like to ask my colleague every information about the patient and the care he or

O.O.J. : I research to know more about the medications of my patients. I always watch videos and read textbooks to be well informed about
the medications of my patient. For example, if i am giving an insulin injection to my patient, i research to know why they are getting the
particular type of insulin and why they get the insulin injection before eating.

Oladapo demonstrated this outcome by communicating effectively to his peers and to the staff to collect information about his residents.
For the most part he maintained privacy by closing the doors and windows during care. He ensured that he documented his care into the
POC and eMar under the observation of his instructor or assigned staff. In addition, he endeavored to communicate relevant information
such as blood sugar results to the appropriate staff. However, he needs to remember to communicate to the charge nurse ahead of time
the skills that he planned to perform. That way the nurse will have an idea of what skills to leave for him.

new observation about my patient. For example, i noticed one of my patients was developing pressure ulcer on their buttocks, i informed
the charge nurse about it and the right intervention was implemented.

care that he planned and introduced himself as a student nurse. In the mornings he consulted with the assigned staff to understand his

the patients outcome. For example, i work well with the personal support worker at the nursing home. Before going to meet my patient, i
ask the personal support worker to know every information about my patient. This helps me to provide the best care for my patient.

Throughout the fourteen weeks he demonstrated the principles of a good team player who is not easily offended. He exhibited the abilities

Oladapo demonstrated this outcome by safely and competently providing compassionate care for his residents. His assessment skills
need improvement in some areas such as respiratory assessments but he was able to provide the basic assessment necessary to provide
competent care. For example, abdominal assessment, wound assessment, bowel routine, skin assessments, vital signs, pain and comfort
measures.

Oladapo demonstrated this outcome by collaborating and consulting with the staff, peers and instructor to provide safe competent and
compassionate care. As mentioned earlier, in the mornings and throughout the day he consulted with the team to get to understand his
residents routine and current status prior to initiating care. He asked for assistance in certain areas of care such as blood sugar monitoring
and insulin administration.

things i need to improve on is identifying the priority needs of my patients and how to apply my theoretical knowledge in clinical practice. I
know i will need to review the health assessment video before going to clinical practice, i will also need to do more reflective thinking on
the nursing skills i perform in clinical practice.

Oladapo demonstrated this outcome by being able to think and carry out a procedure with minimal assistance. For example, when inserting
a Foley catheter he was able to explain the skill before the actual initiation and with some guidance from his instructor. However, there is
lots of room for development in this area. He is encouraged to continue to review his course materials to develop his ability to critically
think and problem solve.

I will continue to do more reflective thinking on the information i have read in the textbook, and i will practice physical assessment more on my friends
and family. I will continue to demonstrate professionalism and accountability. My strengths at the end of the semester are checking vital signs of the
patient, checking blood glucose, administration of medications through G. feed; oral medication, giving insulin injections, complete bed bath, in and
out catheter and bed making.
