research have guided my practice until now. I have used the work learned from clinical and nursing textbooks to foster the knowledge I
have acquired.

Kristina has demonstrated competent and safe care to her patient. She makes sure her bed patients are safe by lowering her patients bed,
side rails up on bed and patient surrounding clear if patient is ambulatory. She demonstrate she understand the role of a nurse, act in a
professional manner, asks question to clarify any situation she doesn't understand and always adhere to Humber college policies and
procedures.

K.M. : In todays health care system, delivery processes involve numerous interfaces and patient handoffs among multiple health care
practitioners with varying levels of educational and occupational training. During the course of a 4-day hospital stay, a patient may interact
with 50 different employees, including physicians, nurses, technicians, PSWs and others. Effective clinical practice thus involves many
instances where critical information must be accurately communicated. Team collaboration is essential. When health care professionals
are not communicating effectively, patient safety is at risk for several reasons: lack of critical information, misinterpretation of information,
unclear orders over the telephone, and overlooked changes in status. Effective team sharing and discussion foster collaboration. This was
done in pre- and post-conferences instances amongst the clinical team.

K.M. : I have documented clearly, accurately and in timely manner using Meditech at WOHS at Etobicoke General. All vital signs and
nursing assessments were inputted in a precise manner. Nurses need to chart effectively for several important reasons. First and foremost,
documentation serves to meet the needs of the patient. It is a communication tool nurses and other providers create and access to ensure
continuity and to deliver safe, quality healthcare to each patient. It is also used as a toll to protect the hospital and the health care
professionals that work within practice settings.

providing care. One way that nurses maintain boundaries and build nurse-client relationships based on trust is by respecting clients rights
around confidentiality and privacy. Ontarios privacy legislation (PHIPA) supports and extends the College of Nurses of Ontarios (CNOs)
standards on nurses accountabilities pertaining to clients personal health information. I have upheld these standards and have kept
patient privacy and confidentiality by respecting the client's rights.

Kristina is learning to communicate with the inter-professional health care team.He did not communicate with the doctors as the doctors
communicates the the primary nurses of the patients. She report relevant information to the appropriate personnel. She document clearly
using meditech and also uses meditech to research patient information.eg. blood work. This student also ensure patients privacy at all
times and keep patients information

K.M. : Interactions should be polite and show respect for fellow care providers. Active listening between the team is a helpful technique, as
it focuses attention on the speaker. Clarifying an understanding of each health care team member's role and responsibilities for care and
follow-up will benefit everyone. I have done to the best of my ability to use these communication techniques. I believe that my confidence
with communicating with doctors could improve however.

K.M. : I continue to set a therapeutic relationship with my client by actively listening to clients key concern during my initial assessment.
From my preceptor, I learned the key to best client care is therapeutic listening during first round of assessment to get clients key care
needs and then set priorities accordingly as per the care needs of all other patients culled from

K.M. : Under clinical teacher guidance, I communicate to the primary nurse, charge nurse and the physician when appropriate and when
client's well-being is of concern. For example: when clients blood work data are off: when creatinine level is too high and liver function test
shows any abnormalities. I also pay particular attention to hemoglobin in post patients especially below the mark of 100. I also look for any
symptoms like shakiness, headache, dizziness, and signs of fatigue and I document and relay the information to the doctor so better care
can be planned.

K.M. : Every individual is diverse and unique. Supporting these diversities and integrating them positively within the clinical practice
environment is imperative and benefits client well-being. I believe that I have been satisfactory in this aspects. The team members at WOHS
divide the work based on their scope of practice; they share information to support one anothers work and coordinate processes and
interventions to provide a number of services and programs. The patient and family are included as key members of the team.

guidelines that reference and encourage collaborative practice. Being respectful and to actively listen to your peers is integral to keeping a
healthy working environment.

values and MARs is integral from preventing any mistakes from happening, ultimately promoting the right care.

K.M. : The College of Nurses works as an important role in order to protect the public. Assessing concerns about the conduct and
competence of nurses is one of the ways in which the College of Nurses works to protect the public. Most matters are resolved through
remediation and resolution processes. The College also responds to concerns that a nurse's physical or mental health is affecting, or could
affect, their ability to provide appropriate and right care. I believe that thus far I have done my due diligence to protect the public through
collaboration and consultation.

K.M. : Evaluating my own limitation and strength as a nursing student and as a novice and based on my preceptors positive feedback
has been an integral part of my experience in the clinical. Through my evaluation and self reflection, I began to see how far I need to go to
become a good nurse. I set my learning plan in the mid term and execute my learning
plan and am now comfortable priming IV, setting IV rate as per doctors order using drip rate formula. Under my preceptors kind guidance,
I have successfully did saline flush, injecting enoxaparin, G tube flush, G tube feeding, change IV bag, monitor IV TBA, fluid intake,
discontinue IV saline lock using proper technique in front of my preceptor, I have learned the importance of putting on pressure with a dry
gauze when disconnecting a saline lock to prevent bleeding. And I always continue
to ask questions my skills and confidence to enhance my learning and understanding when I am in doubt about any
procedure. I continue to evaluate my beliefs when giving culturally sensitive care. I make new learning plan on a continuous basis based
on my learning needs and learning goals that promotes quality assurance nursing practices.

Kristina has improved in her nursing skills throughout this semester. She takes great pride in her nursing practice is always prepared for her clinical
rotation. Kristina assists peers and nurses on the floor. She has cared for patients with trache, foley and G-tubes. This student has practice doing
subcutaneous injections, administering PO and IV medication, administering medication through G-tube, doing simple and assisted in complex
dressing, suctioning and monitoring patients Foley catheter, insert foley catheter, caring for nephrostomy tube. She also collected specimen swabs,
(MRSA) Kristina uses meditech for patient review and documentation. She asks questions on anything she was unsure about and uses technology to
research relative information. Kristina interacts well with patients, patients family, staff and peers. Kristina is a team player and is always seeking out
extra learning opportunities. The student was encouraged to continue improving on her nursing skills , continue to improve her confidence and to do
proper documentation and assessment on her patients at all times.
