The Jaguars of HSL-60 had an extremely successful 2014! The


squadron excelled during an international Joint Warr
Exercise
to the North Atlantic in the spring, and also completed an
extremely successful four-aircra
six-month Firescout VTUAV
deployment in support of AFRICOM
al Operations Forces. HSL
60 was awarded the Blue "M" Award for Medical Readiness an
unprecedented fifth year in a row while also earning the FY14
Noel Davis Battle Efficiency Award, the FY14 Golden Wrench Award,

the FY14 Retention Excellence Award, and the CY14 Safety "S"

Award!

The HSL-60 Jaguars provided the template for safe, efficient,

effective operations and training throughout Calendar Year 2014.

We answered the call each and every time and made the most of our

opportunities, maintaining our tactical acumen, advancing our

capabilities, and supporting the mission of the United States

Navy and the United States Navy Reserves.

Narrative

Our operational commitments began with Detachment TWO aboard


the USS ELROD (FFG 55) with four MQ-8B Firescouts. Detachment
TWO successfully completed more than 300 overland Intelligence,
Surveillance and Reconnaissance (ISR) sorties in support of
Special Operations Forces, developing pattern of life for U.S.
Africa Command's number one target. Executing 650.4 total
mishap-free flight hours, which included 506 overland ISR flight
hours and 133.6 maritime support flight hours, Detachment TWO
participated in operations ACTIVE ENDEAVOR, JUKEBOX LOTUS, OAKEN
LOTUS, JUNCTION SERPENT, and JUNCTION LIAM in the C6F and AFRICOM
areas of responsibility (AOR).
HSL-60 also conducted an IDCERTEX/ISATT onboard USS JAMES E.
WILLIAMS (DDG-95) with two SH-60B's. During this exercise,
Detachment ONE executed 120 mishap-free flight hours.
This
prepared the Detachment for JOINT WARRIOR 14-1, a multi-national
exercise focusing on coordinated operations and relationship
building with our NATO Allies. While underway for 66 days,
Detachment ONE completed 180 mishap-free flight hours and
provided AT opportunities for many of our Selected Reservists
(SELRES).
During that same period, HSL-60 also provided an SH
60B display for Mexico's Secretariat of the Navy (SEMAR) in
Washington, D.C in support of the foreign military sales program.

HSL-60 flawlessly executed its annual flight hour allocation


despite very limited detachment availability, flying over 1,700
mishap-free flight hours and requesting zero annual flight time
waivers for the SELRES pilots and aircrew. The squadron
qualified eight Airborne Use of Force (AUF) Pilots, four AUF
Aircrewmen, one Night AUF Instructor Pilot, and maintained
currency for 15 U.S. Coast Guard Precision Marksmen-Airborne
(PMA) in order to support the nation's war on illicit narcotic
trafficking in the Fourth Fleet AOR. HSL-60 sustained maximum
combat readiness, supporting all tasking requested from the
Active Component, CHSMWL, and Reserve Component, MSW.
HSL-60
concluded the year with an unprecedented 100% sortie completion
rate.
HSL-60
ns the Commander's, Naval Air Force U.S.
Atlantic Fleet {CNAL}, expert in the Airborne Use of Force
practice while maintaining all primary mission sets of the
HSL/HSM community.
During our Atlantic Undersea Test and
Evaluation (AUTEC) detachment and a Peruvian submarine exercise,
our aircraft were able to gain over 17 hours of submarine contact
time, conduct 16 satisfactory simulated attacks, and successfully
deploy eight exercise torpedoes.

Narrative:

a.
The squadron's most recent Aircraft Maintenance Inspection (AMI) was
conducted in November 2012.
The results were 37 programs graded as "On
Track", zero graded as "Off-Track", and four graded as "Needs More
Attention".
The inspection team also presented BRAVO ZULUs to 10 Sailors.
b.
HSL-60 held a Direct t-laintenance Man Hour per Flight Hour (DMMFH) of
15. CHSt--lWL's OP20 goal is 20.1 DMMFH for the SH-60B.
HSL-60's DMMFH was
below this goal 9 out of the 12 months in CY2014.
c.
Tasking in 2014 consisted of Det TWO, a four plane MQ-8B Firescout
detachment embarked on USS ELROD (FFG 55) supporting Special Operations
Forces; Det THREE supporting the multinational Joint Warrior exercise onboard
the USS Jfu~ES E. WILLIAMS (DDG 95); and Det ONE's completion of ISATT and
COMPTUEX in preparation for a Counter Narcotics deployment in January 2015.
HSL-60's maintenance plan allowed for the remarkable completion of 1,764.3
mishap-free SH~60B flight hours and 696.5 mishap-free MQ-8B flight hours. A
complete team, HSL-60's focus on planning, resource management, and effective
synchronization of manpower, readiness, and mission ensured the Com."'l1and was
able to respond to all operational requirements.

Per references (a) and (b), HSL-60 is submitted for selection as
the Commander, Naval Air Forces Reserve nominee for the CNO CY14
Safety Award. Enclosure (1) is forwarded to support this nomination.
2. HSL-60 enjoyed an extremely successful year in 2014 by infusing
everyday operations with sound Operational Risk Management (ORM)
principles. Tasking consisted of a multinational Joint Warrior
exercise as well as an MQ-8B Fire Scout detachment. The dynamic and
often unpredictable nature of these operations required an additional
level of focus on
and adherence to tested tenants of ORM. The
first of these was HSL-60 DETACHMENT THREE which deployed aboard USS
JAMES E. WILLIAMS (DDG 95) in support of ISATT/COMPTUEX in the frigid
January wa.ters of the VACAPES. The
se consisted of
approximately 120 flight hours designed to integrate the ship and air
team into an effective fighting unit and culminated with a seven day
"Battle at Sea" to further increase the fidelity of multiple fighting
forces above, below and on the open sea.
Shortly thereafter DETACHMENT THREE deployed aboard USS JAMES E.
WILLIAMS (DDG 95) to complete a JOINT WARRIOR exercise in the wintry
North Atlantic. They successfully executed approximately 180 mishap
free flight hours in support of the 33 nation Joint Warrior 14 1
exercise.
DET THREE was recognized and received numerous accolades
from the Multinational Task Group Commanders as the go-to asset for
ASW, SUW, MIO/VBSS, and Logistics missions.
Next came DETACHMENT TWO deployed aboard USS ELROD (FFG 55) with
four MQ-8B Firescouts.
DETACHMENT TWO successfully completed more
than 300 overland Intelligence, Surveillance and Reconnaissance
sorties in support of Special Operations Forces, developing pattern of
life for U.S. Africa Command's number one target. Executing 650.4
mishap free flight hours, DETACHMENT TWO participated in operations

ACTIVE ENDEAVOR, JUKEBOX LOTUS, OAKEN LOTUS, JUNCTION SERPENT, and


JUNCTION LIAM in the C6F and AFRICOM areas of responsibility.
The 2014 focus for the Jaguars was on Training. We deployed a
detachment to Andros Island AUTEC to achieve required annual flight
minimums and to hone our Anti-Submarine Warfare tactics and procedures
against real world threats. Operating extensively in the live fire
training environment exposes weakness in safety procedures and
programs, but HSL-60 thrived in this environment executing 92.2
deployed flight hours and 6 Light Weight Torpedo firings in support of
unit and fleet training.
The Jaguars of HSL-60 consistently meet all mission requirements
while still ensuring that safety remained the top priority through the
successful use of the ORM process which resulted in ZERO FTS/AC/SELRES
pilots and aircrew requiring annual flight time waivers!
3. HSL-60 has embraced the Safety Management System and is actively
implementing the four pillars of safety policy, safety risk management
(SRM) , safety assurance, and safety promotion. The following
initiatives and practices have been implemented and improved upon by
HSL-60 to enhance safe operations:
a. Completed the Command Safety Assessment/Maintenance Climate
Assessment/ Administrative Support Personnel Assessment Surveys with
impressive results that indicated Command morale and safety culture
are high and a top priority in the squadron.
b. HSL-60 identified our most hazardous activity Night Airborne
Use of Force (N-AUF) in our top 5 survey, and increased our efforts to
provide more training opportunities for all pilots with respect to
this challenging mission. We collaborated with the USCG to increase
boat training opportunities and aerial gunner practice increasing
standardization between USCG and Navy teams.
c. The HSL-60 maintenance department submitted nine CAT I EI/HMRs,
one BASELINE TROUBLE REPORTS (BTR), and two EXPENDED EXPLOSIVE REPORT
(EER). Additionally, they recognized three Jaguars as "Safety Pro" for
outstanding use of ORM practices.
d. The maintenance department received "On Track" grades during
one CHSMWL Aviation Maintenance Inspection, two CHSMWL pre-cruise
Detachment Readiness Inspections, one CHSMWL post-cruise Detachment
Readiness Inspection, and one CHSMWL Conventional Weapons Technical
Proficiency Inspection.
e. Planned and executed two safety stand-downs prior to major
holiday leave periods and one "Safety Pause" after the release of
significant SIR class A mishap in Naval Aviation.

f. Conducted interactive ORM briefs prior to each flight that


identifies hazards and generates controls to reduce hazards for the
specific mission.

-SH-60B: Faulty impact switch for blown CAD.

-SH-60B: Failure of the foam MAD assembly.

-MQ-8B: Engine chip detector light.

-MQ-8B: Fuel filter missing.,

-MQ-8B: #2 hydraulic pump spline sheared.

-MQ-8B: Engine oil seal failure.

-MQ-8B: Transmission leak.

-MQ-8B: Tail rotor retaining nut installed backwards.

Highlights of the Jaguar Safety Program include:


a.

Zero class "A" mishaps.

b.

Zero class "B" mishaps.

c.

Zero class "C" mishaps.

5. The Safety Management System at HSL-60 is working well. We have


enjoyed a Zero flight and ground mishap year.
We actively promote
Safety at every meeting and Quarters, and emphasize the use of ORM and
CRM not just in the cockpit, but in plane captain boards, and
maintenance actions. Squadron personnel are prepared to detect risk
and are empowered to make the decisions to eliminate unnecessary risk,
mitigate the remaining risk and continue to protect people and
equipment from injury and damage.
Performing under the safety mantra of "Mission First, Safety
Always," HSL-60 has proven itself worthy of selection as the CNAFR
nominee for the CNO Safety Award for CY14.

1. You, the sponsor, are responsible for contacting the member. The quickest tool for initial contact is through global
address book via MS Outlook. Also another valuable tool is the utilization of social media such as Facebook and
Twitter. If all else fails, the attached orders listed the member's transferring command. You may call the duty office
and ask to speak to the member. Remember that you must establish contact with incoming personnel within 10
working days of notification.
2. Once you have established communication with the member, have the member open and update the Prospective Gain
Questionnaire at any time by logging into their NSIPS Self Service account (the same location as E-Ieave) and going to
Employee Self Service>Electronic Service Record>Tasks>Gain Questionnaire. Information entered here will become
available for you to view. Ensure you follow through and ensure the member updates this information.
3. Please offer your assistance to the member and his or her family in any way possible. This includes selecting a mutually
agreeable location to meet the Sailor and/or family upon their arrival. Be sure to emphasize that you both need to
remain in communications and that you need to know when he/she is in the area.
4. You as the sponsor must contact your member **monthly**. Please obtain the incoming member's Navy and personal
email address and send to me so I can send the CMC's/CO's Welcome Aboard Letter.
**We have moved to Jacksonville and the member's orders reflect Mayport. Please make sure that they are aware

mission is Fleet Contributory Support; conducting counter-drug
operations in the Caribbean Sea and Pacific Ocean. We are proud
to fly the MH-60R Seahawk Helicopter on additional missions as
well, including surface and undersurface warfare, search and
rescue, and vertical replenishment.
