What Is The Hazelwood
Student Assistance Program?
Why A Student Assistance
Program?
The Hazelwood Schoo! District, in concert
with Florissant Psychological Services, Inc.,
is offering a student assistance program
for students who attend Hazelwood School
District schools and their families.
Students and their families are eligible
for four counseling sessions at Florissant
Psychological Services at no cost to the
family. Client confidentiality wil be
maintained. No specific information
relating to individual cases will be released
without the written authorization of the
student and his/her parents/quardians, <
PARTICIPATION IS VOLUNTARY.
The purpose of this student assistance
program is to promote access to services
which may be beneficial to students and
their families. These services are
available to students who attend Hazelwood
District schools and their families. The staff
Of Florissant Psychological Services, Inc.
specializes in working with children,
adolescents, and families.
Office Hours:
Monday ~ Thursday
9:00 AM ~ 6:00 PM. >.
Friday
10:00 AM— 4:00 PM.
24 hours notice is needed if your
appointment needs to be canceled or
changed, or the session will be forfeited.
Where Will Services Be
Provided?
Families eligible for and desiring student
assistance program services should
contact Florissant Psychological Services,
Inc. and identity themselves as desiring
services offered through the HAZELWOOD
STUDENT ASSISTANCE PROGRAM.
The office is located at 701 St. Francois
St. in Florissant. An initial appointment
