How To Apply:
1. Fill in the required details in the form. Make sure nothing is left out.
2. Purchase Postal Orders in value & quantities of:
RM30.00 – 1pc, RM20.00 – 2pcs, RM10.00 – 14pcs,
(Post Office’s Commission Charges - RM0.50 each x 17pcs = RM8.50)
Small Pos Ekspres envelope (RM2.50) - 1pc & Medium Pos Ekspres Envelope (RM3.00) - 1pc.
3. At the PAY TO column on the Postal Orders fill the names of:
No 1 for the RM30 Postal Order,
No 2 for the RM20 Postal Order,
No. 3, 4, 5, 6, 7, 8, 9, 10, 11, 12, 13 , 14, 15 & 16 individually on each piece RM10 Postal Order and the
remaining RM 20 Postal Order made payable to :
JV MANAGEMENT.
4. The Small Pos Ekspres envelope is to be made self addressed. Blank the sender’s name & address.
5. On the Medium Pos Ekspres envelope, write the company’s name & add:
JV MANAGEMENT. P.O.Box 928, 10810 G.P.O. Penang.
6. Place the form, photocopy of I/C, all Postal Orders & the Small Pos Ekspres envelope into the Medium Pos
Ekspres Envelope. Seal & drop it into the YELLOW Pos Ekspres Posting Box.
7. Wait for your confirmation reply from the company to be delivered to you. You can then start to share this
great benefit with people around you.
(THE COMPANY WILL ONLY ACCEPT COMPLETED POSTAL ORDER)

Cara Penyertaan:
1. Isikan borang ini dan pastikan semua maklumat dilengkapkan.
2. Beli Wang Pos dalam nilai & kuantiti:
RM30.00 – 1 keping, RM20.00 – 2 keping, RM10.00 – 14 keping,
(Komisen Pejabat Pos RM0.50 sekeping x 17 keping = RM8.50)
Sekeping Sampul kecil Pos Ekspres berharga RM2.50 &
Sekeping Sampul Sederhana Pos Ekspres berharga RM3.00.
3. Isikan nama di ruangan Bayar Kepada di Wang Pos seperti berikut :-
Nilai RM 30.00 – nama No. 1,
Nilai RM 20.00 – nama No. 2,
Nilai RM 10.00 – nama No. 3, 4, 5, 6, 7, 8, 9, 10, 11, 12, 13, 14, 15 & 16.
Dan sekeping lagi bernilai RM20.00 Bayar Kepada : JV MANAGEMENT.
4. Di Sampul Kecil Pos Ekspres, tuliskan nama dan alamat sendiri. Nama dan alamat pengirim dibiarkan kosong.
5. Di Sampul Sederhana, tuliskan nama dan alamat syarikat:
JV MANAGEMENT. P.O.Box 928, 10810 G.P.O. Penang.
6. Masukkan borang, salinan kad pengenalan, kesemua wang pos serta sampul kecil kedalam sampul
sederhana. Masukkan kedalam Peti Pos Ekspres warna KUNING.
7. Tunggu sehingga jawab balas dari syarikat. Seterusnya bermulalah impian anda dengan berkongsi program
ini dengan sesiapa sahaja.

另外一张 RM20.00 填上公司的名字: JV MANAGEMENT。
4. 小号邮政快递信封(Small Pos Ekspres Envelope)填上您(申请人)的名字与地址。
中号邮政快递信封(Medium Pos Ekspres Envelope)填上公司的名字 JV MANAGEMENT 与地址:
P.O.Box 928, 10810 G.P.O. Penang.
5. 将申请表格、身份证复印本、所有邮政汇票(Postal Orders)和小号邮政快递信封(Small Pos Ekspres Envelope)放入中号
邮政快递信封(Medium Pos Ekspres Envelope)内。密封后投入邮政快递邮筒(Pos Ekspres Mail Box)(黄色)。
6. 等待公司递送您(申请人)的推荐表格和分享工具于您,之后您可开始分享这简单、有效的互助计划于您周边的人。

TERMS & CONDITIONS

2. The company will send out the weekly Postal Orders to the members by :-
i) Postal Mail – members who receive 4 pieces and below.
ii) Pos Ekspres – members who receive 5 pieces or more.
3. The company will not be/held responsible for any loss or damage of the Postal Order during the
mailing/delivery process.
4. Weekly commissions from Monday-Saturday will be closed and finalized on the following week’s
Monday and sent out for mailing/self collect on Tuesday.

I hereby acknowledge that I have read and understand and agree to the Terms & Conditions set by the
company JV MANAGEMENT and will abide with it at all times. I understand that the company reserves
the right to amend, modify and/or eliminate any of the terms & conditions, rules, procedures and
benefits at the company’s sole discretion with or without any prior notice.
