OBJETIVOS
POBLACION OBJETO
Dirigido a pacientes mayores de 18 años, de ambos sexos, afiliados a Salud Total EPS-s en
regímenes contributivo y Subsidiado.
DEFINICIONES Y DIAGNOSTICOS:
Examen Físico:
Evaluar Frecuencia Cardiaca y Respiratoria.
Descartar la presencia de: Piel Seca de aparición reciente o que haya requerido
tratamiento, engrosamiento de la piel, uñas quebradizas, caída del pelo, baja temperatura
de la piel.
Examen Neurológico: Presencia de movimientos lentos, retraso de los reflejos
osteotendinosos (Aquiliano) y Bradipsiquia.
Edema periférico (descartar antes alteraciones Cardiovasculares primarias), Edema
periorbitario.
Palpación de glándula tiroides, Bocio, adenopatías cervicales satélites al igual que el
hallazgo de masas en región anterior de cuello.
Ayudas Diagnosticas:
TSH :
Pacientes en tratamiento:
- Tomar TSH una vez al año una vez se haya alcanzado una dosis ideal y el
paciente se encuentre asintomático. Si es así la dosis no requiere ajustes.
T4 Libre:
- Persistencia de sintomatología a pesar de estar tomando levotiroxina, antes de 6
semanas (periodo en el que aun no hay variación de TSH).
- Sospecha de Hipotiroidismo subclínico
Auto anticuerpos de tiroides (Anticuerpos Antitiroglobulínicos y Antimicrosomales)
y Anticuerpos Antiperoxidasa ( No POS):
Utilizados para detectar:
patología tiroidea de etiología auto inmune (ver anexo).
Alteración tiroidea en embarazo.
Pacientes en tratamiento con:
Amiodarona o Interferón.
Historia de abortos.
Ecografía de Tiroides:
Si se palpa crecimiento de glándula Tiroides, nódulos o masas en región anterior
de cuello (Anomalías morfológicas de la tiroides).
Gammagrafía de Tiroides:
SI hay presencia de 2 o mas nódulos tiroideos que se asocian con Zulewski mayor
a 5 puntos o Diagnostico de Hipertiroidismo.
Biopsia por Aspiración con Aguja Fina (BACAF):
Estudio de masas dependientes de Tiroides y descartar malignidad de nódulos.
MANEJO E INTERVENCIONES:
Tratar con Levotiroxina cuando puntuación en escala de Zulewski > 5, o entre 2 y 5 con
sintomatología por 3 meses y TSH por encima del rango normal. Se debe evaluar
tratamiento con TSH en 8 semanas y hacer los ajustes en la dosis según resultado.
Los ajustes en la dosis están justificados si la TSH está suprimida (0.0 mUI/L) en donde se
debe suspender temporalmente, en sujetos con TSH levemente disminuida (entre0.1 y 0.2
mUI/L) se continua la terapia solo si el paciente se encuentra asintomático.
METAS:
Mantener a los pacientes Hipotiroideos con TSH 0.21 a 4.5mUI/L.
Aumentar la precisión clínica para realizar solicitud correcta de TSH
Optimizar el manejo de pacientes con hipotiroidismo acorde con la mejor evidencia
científica.

Instrucciones de Uso:
Según el juicio clínico se debe agregar un punto por cada uno de los signos y síntomas
presentes. Además por la asociación positiva con la edad y algunos síntomas se debe sumar un
punto a todas las mujeres menores de 55 años en la puntuación final de la sumatoria.
Interpretación:
Los puntos de corte definidos para hacer Diagnóstico Clínico de Hipotiroidismo son:
Entre 4.6 y 10.0 Hipotiroidismo (clínico y/o subclínico), Repetir prueba en 8 semanas.
