A self-attested copy of proof of identity (Any one of the following) Passport Election/ Voter's ID Permanent Driving Licence Government employee ID Card Defence ID Card Permanent Account Number (PAN) Card II. A self-attested copy of proof of residential address (Any one of the following) Passport Election/ Voter's ID Permanent Driving Licence Society Outgoing Bill (only from registered societies) Latest Electricity / Water/ Telephone Bill Property Tax bill Domicile certificate with address issued by Municipal Corporation Please provide separate documents for proof of identity and proof of residential address. Only a valid passport will be accepted as both. III. A recent passport size photograph IV. Declaration form: Please submit the above mentioned documents along with the declaration. Note: In case of Add-on credit cards, please complete the declaration and provide us with the above documents for each Add-on credit cardholder(s) in addition to the documents for the primary credit cardholder.
