OUR OBSERVATION & VERIFICATION : Due to overturning of the truck the HDPE bags got torn & damaged : Loss & damage to the consignment of pulses due to accident of carrying truck at Samboli, Hapur while enrooting from Bareilly to Delhi. : Insured has provided us letter of monetary claim dated 05 July 2012 : Insured has not provided us the damaged certificate.

CIRCUMCENTANCES OF LOSS

A consignment containing Chanti Masoor Dal was dispatched from Hanuman Trading Company, Bareilly to Shivam Sales, Assam vide Invoice # 211 dated 02.07.2012 under consignment note # 1291 dated 02.07.2012.

While enrooting to Assam the truck carrying consignment met with an accident near Samboli, Hapur, U.P resulting the truck got toppled and the consignment spread on the road. The truck driver informed about the accident to the truck owner who in-turn informed the Insured. Insured accordingly lodged claim on the Insurers and we were appointed for the survey of the loss. 7.00 VERIFICATION OF LOSS

Soon after the information of the loss on 03 July 2012 we immediately visited the accident site at Samboli, Hapur, Ghaziabad on same day i.e. 03 July 2012 and found the toppled truck was lying on the road and dal spread on the road. On inspecting the truck we found some of the bags were intact/sound and escaped from damages in the incident. We accordingly advise the insured to call another truck and load the sound bags in truck. Accordingly the sound bags were loaded into another truck and sent for the final destination. During discussion the insured informed that rest dal would be sent back to their premises and after inspection they would decide whether the same could be used after reprocessing or not? We accordingly advise the insured to call for the truck and refill the dal into new bags so that the truck along with the dal could be weighed. We accordingly waited at the site till night but the insured had failed to arrange for the truck. We thereafter advise to shift the dal to their premises and inform us before unloaded of the dal, so that we could visit their premises and the joint weighment of the dal could be carried out. But instead of our advise insured did not proceed as our instruction and had not given us any opportunity to verify the damages. We had obtained and verified following documents from the Insured.

The quantity of damage is as per physical inspection. The rate of damaged item had been taken as per the invoice.

COMPUTATION OF LOSS

The assessment of loss is as follows. Note:1. Insurers are requested to obtain the damage certificate from the Insured. 2.
