User Reported by statu s Location : Magnetic separator 1A (JT1). mats RIDHWAN0627 Period : During unloading activity. Event : 1-Alarm MS1A run feedback failure appear from SCADA. 2-Maintenance on shift reset alarm from local panel and try to restart back.Indication lamp for 'Thermic magnet'on at local panel. 3-During restart (maintenance mode),suddenly small spark happen. 4.Immediately MS1A from SCADA and give instruction to maintenance on shift to bypass the MS1A system. 5-Visual inspection, no any abnormal found. Root causes : Unknown Action : -Temporary bypass the system to allow unloading activity. -Instruct to Minarex h/k to clear coal around MS1A area as a precaution action. 29/9- attend today, revw 1week 18/10-Need further discussion for this issue 1/11-looking for technical spec 11/11-PR No.1000057178, RFQ stage 29Nov- Still RFQ stage, waiting procurement to issue PO, revw 1m 10/4-Re-tender iprg, rev 1w revw NAZARUDDIN Location : SR A slewing platform Defect : SR A energy chain cable carrier dropped. Period : During reclaiming Twisdraai Pile A. Event : 1) 12-Apr-2012, 2115hrs, SR A trip on Slew drive VFD run f/back failure. 2) Reset the alarm but another alarm triggered, SRA slew 3) Alarm at VFD panel shows "F11" that means overcurrent. 4) Suspect there is motor power cable leaked/snapped. 5) Further inspection found energy chain cable carrier dropped at slew platform to boom conveyor. Root causes : Open. Action : 1) Found 2 cables nearly snapped. E&I Maint secured the defected cables. 2) Handed over to E&I on shift for rectification job assisted with Maintenance on shift. 13Apr: Required to erect scaffolding (1days), to inform Ops once scaffold completed. Revw next week. iprg MOHDNOOR Location Defect Event : CUL10 : Bucket rear eye broken : 1) During detail inspection on chain & buckets, found 1 bucket rear eye was broken. 2) Carry out temporary welding to allow operation resume. Period : During unloading TB365 at 1945hrs, 11-Apr-12 Root causes : Open Action : 1) Re-welding the broken rear eye bucket only for temporary and need to change with new bucket when there is a window. 12.04.2012 11:59:29 FAIZ D (FAIZ0791) 12/4-Chk & plan to atnd Location : CUL20 FAIZALHASNAN Defect : Bucket rear eye broken Event : 1) Operator inform bucket hanger detached during operation 2) Further inspection by Maintenance in Shift, bucket rear eye broken at bucket no 41. Period : During unloading TB365 Root causes : Open Action : 1) Control unloading flowrate. 12/4-To chk ampere trending, revw 1w : CUL20 BUKHARI0746Location Period : During unloading TB360 Defect : Rubber skirts worn out causing spillages at that area Root causes : open 26/3- Waiting window ZARUL0753 Latest vibration reading is 8.62mm/s RMS in Alarm level ( Zone DPossible to equipment damage ).High amplitude ( 8.43 mm/s rms) at 11X motor RPM. Suspected due to weak structure at gearbox foundation. Please investigate and rectify this problem. 07.07.2011: Plan to investigate & site inspection with REL 9/9- Waiting REL submit report 13/9- To carry out visual inspection 29/9- Plan to re alignment, revw 1week 4/10-Alignment done, submit request form to REL 7/10-Correction: Plan to alignment 21/10-Vibration spectrum shows GM 11X with sidebands. Suspect gear started to wear 24.11.2011-Plan do during this outage 20/3- Waiting next window NAZARUDDIN SR B beam for Tripper need to repaint.
