Service Tax Registration Requirements for Single Premise

Submit 2 sets of below mentioned documents in 15 days of making online application. Keep one set for self.

Particulars ( TAKE EACH AND EVEY APPLICABLE DOCUMENT DULY SIGNED OR PAY BRIBE . TOTAL 3 SETS )
1) Print out of the filled ST-1 form duly signed by proprietor/ authorized person at the end of the application. 2) Copy of PAN Card of the assessee. 3) Documents Required for the premises for which registration is sought-

( ANY ONE OF THE FIVE BOXES AND ALL REQUIREMENTS WITHIN )

If your own property is being registered:1) Annual Tax Payment Receipt Showing name and address of applicant or Copy of Sale Deed or Society Maintenance bill showing name of applicant. OR If the property being registered is of relative/any other person ( no rent payable to such person ) and is self owned by that person :1) Annual Tax Payment Receipt Showing name and address of such relative/person or Copy of Sale Deed or Society Maintenance bill showing name of such relative/person. 2) NOC of the owner to use premises (Sample Enclosed as per attachment 2) 3) PAN card of owner OR If property being registered if rented by applicant:1) Lease License/rent agreement or rent receipt of Registered Co. Op. Housing Society at least for a tenure not less than one year from the date of application for registration if rent more than 10 L , ST registration number of owner / lessor. 2) If rent more than 10 Lakh , ST registration number of owner / lessor. OR If property being registered is rented to the relative/any other person associated to the applicant and given to applicant for use/registration purpose by lessee/tenant and no rent payable by applicant to lessee for such use:1) Rent agreement between original owner and lessee/tenant. 2) Note on relation between applicant and lessee. 3) NOC of the owner to allow applicant to use premises for business (Sample Enclosed as per attachment 2 ) 4) PAN card of owner. 5) NOC of the lessee to allow applicant to use premises for business (Sample Enclosed as per attachment 2 ) 6) PAN card of lessee. 7) If rent more than 10 Lakh p.a , ST registration number of original owner / lessor. OR If property being registered is rented to a person associated to the applicant and given to applicant for use/registration purpose and rent payable by applicant to 1st lessee for such use:1) Rent agreement between 1st lessee/tenant and applicant. 2) Rent agreement between original owner and 1st lessee/tenant. 3) NOC of the owner to allow applicant to use premises for business (Sample Enclosed as per attachment 2 ) 4) PAN card of owner. 5) NOC of the lessee to allow applicant to use premises for business (Sample Enclosed as per attachment 2 ) 6) PAN card of lessee. 7) If rent payable by applicant to 1st lessee is more than 10 Lakh p.a , ST registration number of 1st lessee.

4) Front Page of any one major bank account pass book of proprietor/proprietorship having photo of applicant on that page and any one internal page showing transactions of not later than three months from date of application. 5) Copy of blank cheque containing - a) Name of the Bank and Address & b) Account Number. 6) Note on Name and address of the Proprietor with Any one of 1. Passport 2. Voter Identity Card 3. Driving License of Proprietor. 7) Land Line Telephone Bill or Electricity Bill of Last 3 Months for premise/office for which registration is applied with service tax . 8) POINT 8 ONLY IF AUTHORIZED SIGNATORY ( ALL 4 ) 1) Note on Name and address of Authorized signatory if any. 2) Pan of authorized signatory 3) Any one of 1. Passport 2. Voter Identity Card 3. Driving License 4. Bank Passbook showing name and address, along with photograph of Authorized Signatory if any . 4) Letter of Authority in favor of Authorized Person (enclosed as per attachment 4 ).

9) Declaration stating that the business of the applicant has crossed a turnover of 9 lakhs in the current financial year and mention two aaof the major clients along with quantum of business made with them. ( enclosed as per attachment 3 ). 10) Undertaking in Annexure V ( enclosed as per attachment 1 ). 11) 3 Passport size photos of the applicant.
