Negative Political Feedback or the Auditor's Tar Baby?

Comments on "An Examination of the Problem of Modelling Political Responses in Public Sector Effectiveness Auditing" Colin A. Sharp

One of the most influential advocates of the pursuit of utilization focused evaluation, Michael Quinn Patton (1980), has shown the value of parables in understanding the depth of a qualititative approach to investigation of the effectiveness of human services. A devotee of the qualitative school of programme evaluation might be reminded of the Uncle Remus fable of the "tar baby" (Harris, 1904) when reading Smith's (1992) attempts (in an article in this issue) to prop up a failing "production function modelling". The analogy is tempting for those human services providers who, like "Br'er Rabbit", seem to cast auditors in the role of "Br'er Fox", with a political figure (perhaps not unlike George Orwell's Big Brother), the bear of the patch. Human services professionals are fortunate that Smith has provided such a succinct summation of the intricacies of performance auditing based on regression analysis and production function modelling. However, it seems that the modification suggested to the standard approach to modelling, namely incorporation of "negative political feedback", may have become something of a "tar baby" for him, and anyone who comes to his aid. Despite the effort to enhance the regression approach, it did not make the modelling more accurate in forecasting the patterns of data from performance indicators selected. Although the author's approach seemed at least a legitimate attempt to forward research in this area, it is doubtful that this venture has advanced the cause of accountability of public sector human service delivery. Indeed it may be part of the continuing diversion, of entanglement in a tiring struggle over methodological refinements which bear little resemblence to the concerns of the decision makers and their accountability to their clients. Smith's article had conceptual problems with regard to its assumptions of a nexus between performance indicators, regression modelling and accountability for service delivery. Even if there is an appropriate association between these concepts, and if this

modelling was worthwhile, the issue of political influences could be handled by reference to established non-linear models. The modelling process Smith chose was limited by the database available and the indicators selected. Each of these issues will be dealt with in turn. Is there a Nexus between the Regression Modelling, the Performance Indicator Approach, and Public Accountability?
The approach advocated in the article reviewed is based on the premise that, in order to monitor the effectiveness of public sector organizations satisfactorily, "it is necessary for auditors to build numerical models linking inputs and outputs" (P. Smith, article in this issue). Unfortunately, there are tenuous assumptions of linearity implied in this method which will be addressed later. Nevertheless, assuming there is a linear relationship, the averaging process may not be sensitive to the potential and the limits of the services to meet the objectives. An alternative approach might be to attempt to implement Gilbert's (1978) assessment based on the performance exemplars. Gilbert (1978, p. 21) defines "exemplary performance as the worth of the historically best instance of the performance". By identifying the exemplar, and, by comparison, the "potential to improve performance (PIP)" for each other case, one could assess the performance of comparable organizations. Certainly this approach would engender the competition in the public sector which Smith is pursuing. Typical competence is inversely proportional to the potential for improving performance (the PIP), which is the ratio of exemplary performance to typical performance. However, in offering to help Smith with Gilbert's theories, one begins to be entangled in the tar baby that Smith has created. Such interesting terminology as "accomplishments" still leaves the managers of human services focusing mainly on measures of outputs and processes as performance indicators, rather than concentrating on outcomes for users and the impact and benefits overall. There are those among the allied health and welfare professionals, and other stakeholders (including some accountants), who would reject the meaningfulness and/or practicality of this positivist/reductionist approach (Lincoln and Guba, 1985; Morgan and Smircich, 1980; Strauss, 1987). Also the more moderate evaluation professional (e.g. Attkisson et al. , 1978) would question Smith's approach, at least in the less capital intensive interventions and more sociopsychological human service programmes (e.g. Tomkins and Groves, 1983a, 1983b). Their arguments would focus on the "impact" on the wellbeing of the individual clients or recipients of the services being audited and on the perceptions of the other stakeholders in the audit and evaluation of the services concerned (Culyer and Horisberger, 1983; Hargreaves and Attkisson, 1978; Patton, 1980). Admittedly, part of the problem Smith's modelling would encounter with these critics is due to a clash of paradigms between the "scientific" and/or "quantitative" approach of economists and statisticians (e.g. Abdel-Khalik and Ajinkya, 1979; 1983) of which Smith seems to subscribe, versus the "naturalistic" and/or "qualitative" approach of many human services professionals (Lincoln and Guba, 1985; Morgan and Smircich, 1980; Patton, 1980; Strauss, 1987). It seems both schools or paradigms vie for the attention and acceptance of the decision makers (Zaleznik, 1970) and so become subject to the influences of power political discretion affecting the evaluation process (Martin, 1982; 1991) while straying from their fundamental professional obligations to the community. Identification of Need Indicators Although the author has attempted to analyse and model available data, one cannot help being concerned for the soundness of the field if all that is available is the kind of database described in the paper (provided by the National Health Service on maternity services in England). Particulary confusing is the use of "percentage of population aged over 64", "percentage of elderly living alone" and "percentage of population under five" shown as "needs indicators" in the author's Table I. How they can be construed as indicators of need for maternity services is a good exercise for one's imagination. Consequently, one has to raise technical issues relating to inclusion of inappropriate data as a source of error, which could contribute to the ineffectual modelling

demonstrated by the author. More seriously, many human service professionals would be concerned about how realistic is the inclusion of these types of data when the models derived are used for programme decision making. The Term "Negative Political Feedback" Smith claims, quite correctly, that "a major weakness in the models described so far is . . . that they fail to take account of possible reactions to the published indicators". However, in illustrating his argument he then becomes entangled deeper with his tar baby, by postulating the influences of presumed political interventions, which are thought to act as negative feedback on the system. The first consideration here is: would this situation, if it did in fact occur, warrant a new terminology? The phenomenon described has been known in evaluation practice as "goal displacement", i.e. managers become focused on achieving some supposedly desirable criteria on the performance indictors of those programmatic factors which they can measure, while losing sight of the intended goals or outcomes of the service they are supposed to provide, but which may not have such convenient performance indicators. However, this is where Smith's tar baby becomes hazardous for other evaluation travellers around the human services patch. The author's appeal to correlations and linear trends and to "the terminology of cybernetics" for his interpretation of regression analyses seems confused or at best imprecise. In trying to clarify the situation, one can become involved with the problem. For example, first he says that "the superficial implication of the models estimated so far is that resource provision is immaterial to health outcome in the maternity services!" In effect this suggests a "null result" where no statistically significant correlation or regression coefficient was obtained. Then he goes on to postulate something like a straw person in the form of a "'common sense' negative correlation between resources and mortality rates". He proceeds to knock down this straw person in favour of a "positive correlation between mortality rates and resources" by references to this phenomenon as "negative political feedback" (hereafter called NPF). It is not clear whether the reader is to believe both of these correlations occur at once or in sequence. What is their relationship with the positive and negative feedback loops he postulates? If the interpretation is that both processes occur as underlying forces causing the corresponding correlations, then it is imprecise to call this a negative feedback effect. Indeed, if the author believes so strongly in the cybernetic approach, why coin a new term, when the MIT Professor of Systems Engineering, Jay Forrester, had already coined the term "non-linear coupling" to denote a very similar phenomenon, i.e. a "shift in dominance from one system loop to another" (Forrester, 1968, p. 2-1)?

Such a growth towards a limit often occurs in the introduction of a new product to a market. At first, the product is successful, sales produce revenue that supports more sales effort to produce still more revenue. But at some point, sales become increasingly difficult the market approaches saturation, or the easy initial sales cause the supplier to become careless and neglect quality, or the manufacturing capacity for producing becomes overloaded (Forrester, 1968, pp. 2-21 to 2-22). The important thing to note about the similarity with Smith's interpretation is that this type of phenomenon has both a positive feedback loop and the negative feedback loop. So, by analogy, it would be misleading to describe the model as simply a "negative political feedback" effect. The other aspect relevant here is that the overall process is non-linear in its transfer function. This raises questions about the appropriateness of a regression analysis. Nevertheless, the analogy with such systems phenomena has a limited usefulness in explaining the maternity services data, because, in Forrester's example, the data is readily quantifiable as the number of products sold. In Smith's example, the data appear to be more complex. Choice of Maternity Services as the Database for Model Development There are various approaches to assessment of organizational effectiveness,

most of which recognize the need for multiple data sources and levels of analyses to account for the ongoing changes in organizations (Cameron and Whetton, 1981; Crocket, 1978; Mahoney and Weitzel, 1969). Smith's focus on client demographic data at the expense of other organization

effectiveness information limited his ability to address the political phenomena. Assuming the non-linear model to be correct, there is still the problem of identifying the causal factors, and in particular the role of political sensitivity on the part of maternity service managers. As an illustration of the narrowness of Smith's paradigm-bound approach to accountability, consider one of the reasons for proffering the NPF hypothesis. He presumes that it is the managers of the health services and their behaviour in manipulating the resources which is the only factor having a major effect on the performance indictors of perinatal mortality. No consideration was given, in this mechanistic, or at best nave, econometric model, to the consumers' choices. Surely, an economist would give some consideration to the market for the services. In this case the consumers' choices could be regarded as the responses of the pregnant women to the perinatal services offered. Let us assume they are as politically aware as the managers of the medical/hospital based maternity services. The consumers might be sensitive to the alarming propsect of child birth in their supposedly non-performing, and/or imposingly technologically resourced maternity service. It is possible that these women may have exercised their right to choose, and opted for home or midwife "low-tech" delivery. Given the other performance indicators of lower socioeconomic status, there may have been increased birth complications as a result of this choice. A more naturalistic and qualitative approach to investigating these phenomena might have been more sensitive to the underlying factors of the system, such as the maternity services preferences of the women concerned. A study gathering such qualititative data might provide comparisons of interview transcripts of a range of consumers. For example, the survey might target women of different socioeconomic status who had different types of experiences of maternity services (e.g. conventional services users with successful deliveries of healthy children, or deliveries which were not successful), and compare their

responses and outcomes with those of users of other services. By demanding the expression of the goals in a five point behaviourally anchored rating scale, the technique leads participants to document, in a public way, the likely acceptable and unacceptable results. When the programme is completed and the decision makers and service providers are accountable for what they have done, they will at least then have a common benchmark for estimating the effectiveness of the programme. This technique is even more powerful if the consumers of the service are involved in the development of the goal attainment scales for the programme they are using (Kiresuk and Lund, 1978). They also advocate the development and use of health indexes. But they point out that it would "almost certainly be premature in the current state of our knowledge . . . to place a money value on changes in a measure of health" (Culyer and Horisberger, 1983, p. 354). Smith takes a simplistic approach (namely, a quantitative cost-benefit analysis based on poor performance indicator data) and then tries to impose a politically sensitive, qualitative governor on the system, but in the process he proceeds to create a tangled mess. If his point about political influences is correct, why not develop a more appropriate approach from scratch, such as a "gains/losses" analysis, instead of a cost/benefit approach? Conclusion

This review has attempted to expose some of the inherent problems of testing the econometric approach put forward by Smith. It was suggested that much of his concern to overcome the inadequacies of this approach constituted a distraction from the more fundamental issues which decision makers need addressed. In attempting to avoid this narrow, postivist, paradigm-bound approach, consideration should be given to combining qualitative and quantitative data collection and analysis methods. Ultimately, sensitivity to the consumers' perceptions, and market phenomena, should be part of the auditor's approach.
