1. B I D I N F O R M A T I O N & L O C A T I O N Simms Building Group requests your quote for the new US Bank facility at Westport for divisions 2, 3, 4, 5, 6, 7, 8, 9, 10, 12, 15 and 16. This project consists of new construction of a single story 4,000 SF ground up bank branch and a four (4) lane drive up banking canopy with parking and circulation for vehicles on a 0.94 acre site. This is a LEED project This work will include, but is not limited to earthwork, planting irrigation, asphalt paving, concrete paving, pavement markings, foundations, flatwork, masonry, structural steel, rough carpentry, wood trusses, finish carpentry, water repellents, thermal insulation, EIFS, weather barriers, standing seam metal roof panels, sheet metal and trim, joint sealants, hollow metal doors and frames, flush wood doors, glass and glazing, aluminum storefronts, drywall, ceramic tile, acoustical panel ceilings, linoleum tile flooring, modular carpet tile/walk off tile, exterior painting, interior painting, panel signage, toilet, bath and laundry accessories, fire extinguishers and cabinets, bank specialties and equipment, plumbing, mechanical and electrical. Note: No Bid shall be withdrawn for a period of sixty (60) days subsequent to the opening of bids, without the consent of the Owner. Note: For subcontractors that are providing install only of Owner furnished items will need to include the quantity of material requirement for installation. Please refer to Owner Purchased Items List on sheet G100. Note: This is a 100% Union project. All bids are due in writing at the Simms Building Group Inc.s office on or before the above mentioned date and time. 2. BID DOCUMENTS: Bids shall be taken from plans and specifications entitled " US Bank MO, STL Westport U.S. Bank, Corporate Real Estate as prepared by KWA Architects dated April 26, 2013. Bidders are responsible for complete review of all contract documents, specifications, addendums, and site conditions in preparing their bids. Each bidder is to include all work shown for their respective bid categories and include all cost for this work in their bid amount. No additional cost will be considered for work shown on the bid documents, but left out of the provided bid. Should a Bidder find discrepancies in, or omissions from the Drawings or Documents, or should he be in doubt as to their meaning, he should at once notify the Architect and/or Simms Building Group, who will send a written instruction to all Bidders. Neither the Contractor, Owner, nor the Architect will be responsible for any oral instructions. Any instructions or addenda issued during the time of bidding are to be covered/included in the Proposal and they will become a part of the contract. 3. PROJECT SCHEDULE: 24 WEEKS 4. GENERAL CONDITIONS: a) Simms Building Group assumes that the bid received includes and incorporates all information contained in the Construction Documents as enumerated above and these bid instructions. Additional consideration after receipt of bids will not be given for omitting any of the above sources of bid information. b) No exclusions or changes from drawings, specifications, or bid instructions will be permitted without written acceptance from Simms Building Group. c) All work to be done in strict accordance with all Local, State, and National Codes whether shown correctly or not on working plans and shop drawings. It shall be the responsibility and expense of this subcontractor to follow these codes. d) Terms: All invoices must be in the Simms Building Group office on or before the 20th of the calendar month in order to be processed for payment on or after the 20th day of the succeeding month. Payment is contingent upon Owner payment to General Contractor. e) Insurance Requirements: Subcontractor shall furnish Simms Building Group with a Certificate of Insurance including coverage in minimum limits as follows: 1) $1,000,000 each occurrence and $2,000,000 annual aggregate for the following: Commercial Bodily Injury and Advertising Injury, Commercial Property Damage, Automobile Bodily Injury, and Employers Liability. 2) Workmens Compensation Statutory.

3) $5,000,000 each occurrence for Umbrella Liability and $5,000,000 annual aggregate for Umbrella Liability.
