UNIVERSITY OF THE PHILIPPINES COLLEGE OF LAW GUIDELINES FOR ENROLLMENT, AY 2013-2014 REGISTRATION PROCEDURE/CHECKLIST FOR FIRST YEAR LAW

STUDENTS

Notify the Office of the College Secretary (OCS) in writing of your intention to enroll on or before March 20, 2013 together with the payment of reservation fee of P5,000.00. Pay P100.00 for the evaluation/admission fee at the Cashiers Office, (Code No. 9232-884-951-005) Institute of Small Scale Industries (ISSI) on or before March 20, 2013. Submit the Official Receipt to the OCS. Submit to OCS the following on or before May 3, 2013:

Official Transcript of Records (OTR) Acceptable OTR : with remarks of Copy for University of the Philippines only or no remarks. Not acceptable OTR : with remarks For Reference Only, Not Valid for Transfer, For Employment Purposes etc. Certificate of Graduation Photocopy of NSO Birth Certificate (original to be presented) Photocopy of NSO Marriage Contract (original to be presented) for married women 3 pcs. passport size colored photos Accomplished student directory form (downloadable from http://law.upd.edu.ph ) Accomplished student information sheet (downloadable from http://law.upd.edu.ph )

Secure your University Admission Slip from OCS from May 13, 2013 onwards. In the admission slip, check your temporary account and password to enable you to update student profile and to enlist online. Check the website: http://crs.upd.edu.ph for more information. Go to the U.P. Health Service for your physical and medical examination. Present your University Admission Slip (Note: Do not leave this slip with the U.P. Health Service). The Health Service will then issue an X-ray and Medical Slip as evidence of your having undergone the physical and medical examination.

During registration: Go to the OCS Registration Counter, Room 117, Malcolm Hall. Bring along the University Admission Slip, Medical Slip and Form 5A with you. You are not permitted to enroll in the evening class unless you present proof of your full-time employment status such as an employment contract. If employed with the government, present a copy of your official appointment paper. If you applied for STFAP (Socialized Tuition Fee and Financial Assistance Program) benefits, get your certificate of STFAP benefits from the Scholarship and Financial Assistance Office, Room 303, Vinzons Hall. Financial aid is available through Scholarships. Tuition Fee: P1,000/unit (Family Gross Income of 1M and below), P1,500/unit (Family Gross Income of more than 1M). Secure your U.P. Identification Card at the Registrars Office. Have your I.D. card countersigned at the College of Law Library (Espiritu Hall). Attend the College Orientation Program which will be scheduled sometime in June 2013. Proper attire is required.
