1.0

INTRODUCTION

1.1. SCOPE CONTRACTOR shall carry out the Design and Detailed Engineering, Procurement, Installation, Construction, Pre-commissioning, Commissioning, and Testing of the FACILITY as described hereafter in this Appendix A and referred to as WORK. 1.2. BACKGROUND Qatar Petroleum (QP) operates a number of offshore oil and gas production facilities in the territorial waters of Qatar, in the Arabian Gulf. Two of these Production Stations are PS2 (Maydan Mahzam) and PS3 (Bul Hanine). Both of these productions stations contain Living Quarter(LQ) modules PS-2Q and PS-3Q respectively. Both the original existing LQs (PS-2Q and PS-3Q) were designed for a manned capacity of 45 personnel. In 2001, a study concluded that the manned occupancy was to be increased to accommodate 90 personnel. As a result both LQ platforms were renovated, to meet the revised accommodation levels, and work completed in 2004. The new LQ modules (similar in layout) are a combination of two new additional accommodation decks, with the existing 3-tier accommodation decks mounted above, supported on a new topside jacket in a new position. In addition a new helideck was installed. The original height difference between the lowest deck and the Helideck of the old accommodation LQ Block was approximately 22.0m: EL + 100.00 Level 1(Lower Deck) and EL + 122.00 Helideck Level (Upper Deck) The new height difference between the lowest deck and the Helideck of the combined accommodation LQ Block is approximately 26.5m: EL + 100.00 A Deck Level (Lower Deck) and EL + 126.30 Helideck Level (Upper Deck) The current internal staircase configuration from Deck A to Level 3 is very steep and poses problems for Operations personnel to manage MEDIVAC cases.

1.3. PROJECT OBJECTIVE The objective of this CONTRACT is to provide a personnel lift system. This will be a Full height single elevator scheme located externally Open plan lift shaft without pressurized HVAC system at both PS-2Q & PS-3Q living quarter platforms. The lift system will serve five floors to meet operations daily requirementsand for the handling of casualty cases including MEDIVAC. 1.4. EXISTING FACILITIES PS-2Q Living Quarters Platform This LQ platform consists of 5 main levels accommodating living facilities & Utilities with two additional levels above as Old & New Heli-Deck floors. The lower modules @ Levels A, B & C are new modules, above which the old LQ modules @ Levels 1 , 2 & 3 are installed. Level A @ EL.99.970 & Mezzanine Deck @ 102.870 - This level incorporates the Control room, Instrument room, Electrical room, Station office, Utility equipment, survival Capsules and other services areas. The bridge link to PS-2J connects at this level. Level B @ EL.105.970 - This level accommodates the OIS office, SMS / communication office, Prayer room and other living and recreation rooms. Level C @ EL.110.270 & Level 1 @ EL.111.770 - This level accommodates Dining rooms, Stores, Laundry, Potable water tanks and other utility equipment. Level 2 @ EL.115.570 - This level accommodates gymnasium, store and other living & recreation rooms. Level 3 @ EL.119.370 - This level accommodates Reception, Sick bays and other living rooms. Heli-Deck @ EL.123.322 is the Old LQ Heli-Deck structure, above which the new Deck @ EL.126.322 is installed which is being used for Heli landing.

APPENDIX A The existing platform DCS system in PS-2 is a Yokogawa FF based Distributed Control System. The input/output from the field are connected to the existing marshalling/FF interface cabinets located in the CER. The platform ESD system in PS-2 is a TMR (Triconex Version 9.2) safety system which covers the entire ESD function for the PS-2 complex. There exists a remote extension Triconex Chassis cabinet ESD-PNL-011 located in the instrument room or CER (Control Equipment Room), installed as part of Living Quarters upgrade project. All the TPSD signals are hardwired directly to the main ESD system in PS-2G. The remote extension Triconex Chassis in PS-2Q is connected with the main ESD system in PS-2G via triplex optical fiber cables. The existing Fire & Gas system in PS-2 (2G/2Q) is a TMR (Triconex Version 9.2) based system. Existing PA/GA system installed at PS-2Q is Federal Signal System and Loudspeaker with Model No. AMR6 (Federal Signal make). PS-3Q Living Quarters Platform This LQ platform consists of 5 main levels accommodating living facilities & Utilities with two additional levels above as Old & New Heli-Deck floors. Level A @ EL.99.963 & Mezzanine Deck @ 102.832 - This level is packed with Control room, Instrument room, Electrical room, Station office, Utility equipment, survival Capsules and other services areas. Bridge to PS-2J is landing on this level. Level B @ EL.105.961 - This level accommodates OIS office, SMS / communication office and other living / Prayer & recreation rooms. Level C @ EL.110.206 & Level 1 @ EL.111.770 -This level accommodates Dining rooms, Stores, Laundry, Potable water tanks and other utility equipment. Level 2 @ EL.115.570 - This level accommodates gymnasium, store and other living & recreation rooms. Level 3 @ EL.119.370 - This level accommodates Reception, Sick bays and other living rooms.

The lower modules @ Levels A, B & C are new modules, above which the old LQ modules @ Levels 1 , 2 & 3 are installed. The new LQ is connected to PS-3C via bridge at Level A @ EL.99.963. PROPOSED FACILITY The proposed FACILITY is to consist of the following: A full height Single Lift located externally to both PS-2Q and PS-3Q living quarters modules located on the East face of both the platforms. The lifts will be Rack and Pinion type without enclosed lift shaft. The lift system will be retrofitted to the existing platforms. The modifications that will be necessary for the inclusion of the new lift system will be as follows: New platforms, deck extensions and walkways will be provided on each lift landing decks. The lifts structural mast will be supported laterally on each deck level with vertical support being provided from below Deck A

APPENDIX A level via structural stiffening system connection to Deck A. The lift SUBCONTRACTOR will supply the structural mast that will support the lift. New platform extensions and walkway access shall be provided with new lighting system. The personnel lift shall be feed from the normal supply board PS-2Q-SB3202 (PS-3Q-SB-3302). During loss of normal power supply, the personnel lift will not be operable. There will be no lift operation during the loss of normal supply and/or Fire &Gas and ESD scenarios. The lift status signal shall be handed over to existing DCS system. The signals shall be hardwired from lift BCP to DCS system. Modifications to existing DCS shall be carried out to include lift status signals. Existing gas detectors placed near the hand rails are to be relocated in line with the new platform extensions and widening the walkways. The cables that are connected to the Gas detectors shall also be replaced with new cables. Modifications to existing PA/GA and telecom systems shall be carried out to include new devices installed in the lift as part of this CONTRACT. Relocation of life raft, life buoy, fire hose box and fire hydrant to hose outlets will be required as a result of the installation of the personnel lift system. Similarly the rerouting to the safe escape routes will be required. Piping works shall be mostly re-routing of existing piping to facilitate the proposed lift operations. Drawings in Appendix F show the proposed FACILITY, which shall be confirmed / further detailed by the CONTRACTOR during Detailed Design stage, according to design requirements. It shall be the responsibility of the CONTRACTOR to identify and provide any additional sub facilities and services required to, support the main FACILITY, to meet the CONTRACT objectives and for the COMPLETION of the CONTRACT. Any modification to the proposed FACILITY shall be carried out by the CONTRACTOR in consultation with QP. CONTRACTOR to take note of the above requirements and shall include provision for multiple visits to Doha and Offshore facilities for meeting and co-ordination with different QP user groups. 1.6 BACKGROUND TO FEED
a) QP undertook the FEED for final selected Option Full Height Single Elevator Scheme located externally - Open Plan Lift Shaft based on the Techno-Economical Evaluation Report findings which consisted number of

APPENDIX A options covering both external and internal to the living quarters accommodation.
b) The FEED progressed on the design basis agreed for the FEED and was successfully completed with all CONTRACT deliverables produced. c) All references to FEED shall be considered as Front End Engineering Design. d) ALIMAK or Equivalent Lift supplier who meets the requirements as outlined in this FEED package can supply the offshore lift suitable for Hazardous environment are to be considered for the Lift system. The FEED documents are prepared based on ALIMAK documents.

1.7 ENDORSEMENT AND GUARANTEE OF DESIGN


The FEED package defines the basic principles and requirements for design, procurement, supply, fabrication, construction, installation, precommissioning hook-up, commissioning and testing for the FACILITY and the WORK. The CONTRACTOR shall develop the detailed design based on the CONTRACTOR endorsed FEED Package.

FEED Package
The CONTRACTOR shall execute the WORK in accordance with this Appendix A Scope of WORK - and the specifications, drawings and other documents and instructions included in Appendix F TECHNICAL INFORMATION and all other relevant documents in other sections of this CONTRACT. It should be noted that all Disciplines Material Take-offs (MTOs) provided in Appendix-F are provided for information only. QP does not take any responsibility for the provided MTOs. The CONTRACTOR shall take full responsibility for all MTOs produced by him. Accordingly, any cost impact due to additional cost required for WORK shall be deemed to be included in CONTRACTOR scope. The FEED Package is contained in Appendix F Technical Information and contains the following and shall be considered as QPs minimum requirements for the Scope of the WORK. QP Corporate Standards, Specifications and Codes of Practices;

Endorsement of FEED Package


The CONTRACTOR shall review and validate the Front End Engineering Design (FEED) within the FEED endorsement period of the CONTRACT specified in the EPIC schedule, taking full ownership and responsibility for the design and associated technical documents / drawings as though the CONTRACTOR has undertaken the preparation of the FEED itself. CONTRACTOR shall provide written acceptance of the FEED Packages to QP within 45 days of CONTRACT EFFECTIVE DATE and no claims associated with the design modifications will be considered by QP after the 45 days period, unless changes have been specifically requested by QP. By providing the CONTRACTORS signature on the Form of Endorsement of the FEED, the CONTRACTOR confirms, with respect to the FEED, that: a) The CONTRACTOR has carefully and thoroughly reviewed the FEED and is satisfied with the quality, quantity, adequacy, consistency and degree of details of these documents;

b) The CONTRACTOR confirms that the FEED has been checked and found to be correct, complete, conforming to the CONTRACT Standards/Specifications and is fit for its intended purpose. Therefore the CONTRACTOR assumes responsibility for any errors, omissions, incoherence, contradictions, discrepancies and/or ambiguities of whatever nature; c) The CONTRACTOR endorses and takes full technical and commercial responsibility associated with the FEED;

APPENDIX A d) The CONTRACTOR confirms that the FEED are suitable as a basis of the Scope of WORK under the CONTRACT and commits not to question the FEED after endorsement of FEED package by the CONTRACTOR;
e) The CONTRACTOR waives any right of recourse whatsoever in respect thereof against QP.

PERFORMANCE GUARANTEE
The purpose of this section is to define the performance guarantees for Personnel Lift Package, outline the requirement for their verification during a test run, as well as remedial actions in case such performance guarantees should not be met. The Guarantee period of the Services shall commence from the date of COMPLETION of the SERVICES as entered in the CERTIFICATE OF COMPLETION and shall be for a period of one (1) year, till the issue of DISCHARGE CERTIFICATE.

Provision of authorization of access to offshore sites. Providing transportation to CONTRACTORs personnel to / from Doha and offshore location in accordance with conditions detailed in Appendix G. Providing all necessary documentation as requested by CONTRACTOR. However, CONTRACTOR shall be solely responsible for retrieval of necessary documentation. QP shall be providing the required assistance for the same. Review of detail designs, specifications, drawings, procurement documents, control documents, procedures, and any other documents required to be submitted to QP by the CONTRACTOR in accordance with the CONTRACT. Such review/approval/consent/acknowledgement by QP shall not relieve CONTRACTOR of any obligations, responsibilities of liabilities under this CONTRACT. Issue of all necessary completion certificates, as required under the CONTRACT, on satisfactory completion of the corresponding milestones. Taking over the FACILITY upon COMPLETION. Communication/Correspondence with the press and other news media, including finalization and approval of all public announcements. Provision of all materials, equipment, facilities and services as detailed in Appendix G of this CONTRACT.

CONTRACTOR Responsibilities

2. Provision of Quality Assurance and Quality Control systems covering all aspects of the WORK under this CONTRACT and the CONTRACTOR shall impose back-to-back systems on allSUBCONTRACTORS. The provision of a quality management system established by the CONTRACTOR in accordance with guidelines provided under QPs Quality Requirements for Projects (appended in Appendix F of this CONTRACT), ISO 9001 requirements and all other instructions elsewhere in this CONTRACT. Provision of a quality Document Control System that meets the requirements of QP throughout the CONTRACT period and allows easy electronic transfer of data to QP systems on COMPLETION as per Appendix D. To utilize only those documents and data issued by QP under this CONTRACT and not to utilize QP data otherwise obtained. Mobilization of all CONTRACTOR PERSONNEL and resources involved in the performance of the WORK in a timely manner after the CONTRACT EFFECTIVE DATE and de-mobilization as required ensuring the requirements of the CONTRACT are achieved and in accordance with procedures agreed with QP. Provision of all keyCONTRACTOR PERSONNEL as nominated in the CONTRACTORS project organisation chart included in this CONTRACT, subject to approval of QP. Performing Detailed Engineering as per the requirements of the CONTRACT, including identification and retrieval of all necessary documents from QP records not include in Appendix F. Performing Procurement, Subcontracting, Material Management, Storage and Preservation, Expediting, Inspection and Testing (including Third Party Inspection/Certification), Customs Clearance and other General Services in accordance with the requirements of the CONTRACT and all approved procedures ensuring the WORK is completed by the SCHEDULED COMPLETION DATE.

APPENDIX A 11. Provision of Health, Safety and Environmental considerations, personnel and equipment to meet the requirements identified under the CONTRACT including:
a) Compliance with all rules, guidelines, regulations, procedures, programmes and policies set forth in the Law for the Protection of Environment in the State of Qatar and QPs Environmental Requirements during all phases of the WORK. Both Documents are included in Appendix F. b) Ensuring all Documents relating to Health, Safety and Environment are maintained as live documents throughout the performance of the WORK. CONTRACTOR shall issue them in a closed-out form only when the WORK ceases to affect them. c) Complying with all health and safety regulations as required for working at the WORKSITE (S) where the WORK will be undertaken and provide QP with comprehensive safety reporting. d) Ensuring that minimum WORK is performed offshore. 12. Provision of all necessary onshore construction and fabrication facilities and equipment to enable the CONTRACTOR to undertake the WORK, including all necessary prefabrication, fabrication, construction, installation, erection, pre-commissioning and commissioning. 13. Provision of all resources and equipment to undertake on-shore loadout, offshore transportation and offshore installation of the FACILITY, all necessary protection, temporary safety equipment and internal and external sea fastenings. 14. Provision of all marine resources such as supply vessel for transportation of CONTRACTORs equipment & materials, accommodation workboat / barge complete with lifting crane facilities for the offshore WORK execution, and other support marine vessels (Anchor Handling Tugs, etc.). 15. Provision of all resources and equipment to undertake the offshore hookup of the FACILITY, installation of the personnel lift system along with all pre-commissioning and commissioning activities including provision of all necessary spares. 16. Provision of all necessary personnel to undertake maintenance of the FACILITY till COMPLETION and handover to QP.

1.9.3

THIRD PARTIES Responsibilities

CONTRACTOR shall be responsible for arranging the TPC services for equipment and materials in the country of origin for statutory, insurance, or any other reasons. CONTRACTOR shall submit a list of approved TPC Agencies to QP for review and approval. Refer Section 4.5.4 for details. The TPC shall function independently and report simultaneously to QP and CONTRACTOR.

PHILOSOPHIES and CONSTRAINTS


GENERAL
QP believes from its vast experience that one of the key foundation blocks for successful, efficient and cost-effective end product FACILITY is the quality of detail engineering and design. Therefore, QP places a very high importance on the CONTRACTORS selection of an internationally recognized Detail Engineering and Design SUBCONTRACTOR or in-house organization with a proven track record of undertaking and successfully delivering similar offshore projects. The design team should ideally be situated in a single location under one roof. Accordingly the Engineering and Design SUBCONTRACTOR shall mobilize their A Team for this CONTRACT as per the Project Organogram in Appendix E. QP will verify at any time during the CONTRACT DURATION by interviewingkey CONTRACTOR PERSONNEL.

2.2

CRITICALITY, AVAILABILITY & RELIABILITY


The performance of the WORK, shall not in any way affect the day to day operation of the existing LQ facility. The CONTRACTOR shall consider the criticality of personnel lift function and operation and ensure access to all levels of the existing LQ facility is maintained and not compromised. The design considers that the personnel lift will be capable of delivering maintenance-free operation except for essential preventive maintenance. The lift manufacturer shall be able to offer equipment suitable for an offshore application and shall comply, where deemed appropriate, with the relevant QP Offshore Guidelines and Standards. The equipment shall be designed for an operational life of 25 years with regular periodic maintenance.

2.3

OPERATING, CONTROL & SAFEGUARDING PHILOSOPHIES


Process safeguarding philosophies shall be based upon API 14C guidelines. Further safeguards may be included based upon lessons learnt through operational and design experiences. Primary criteria for the new FACILITY to be considered are: Minimum operator intervention. User operated (emergency condition). Monitoring parameters in control room. Display local and control room.

The personnel lift shall be fitted with the recommended SUBCONTRACTOR operational touch button controls and safety devices. The lift will be fitted with an internal Emergency Call facility linked to the Platform Central Control Room. On each level of disembarkation, Up and Down travel buttons will be installed along with a LCD display of the lift level position. Power supply to the personnel lift shall be directly feed from the Normal Supply Board. In the event of Normal power failure (tripped or due to fire / gas being detected or ESD condition), the lift if it is in operational travel, can return to the nearest floor position below and OPEN doors, manually. The personnel lift is not intended to be operational during the emergency power scenario (loss of normal power supply).

2.4

FIRE & SAFETY PHILOSOPHY The Personnel Lift Design, Installation shall be in accordance with requirements of Section 16.8 of the QP Fire & Safety Philosophy, QPPHL-S-001. Any deviations from the above shall be submitted to QP for approval. Lifts shall be designed to permit emergency egress by means of hatches in the ceilings and ladders within the lift car. Refer to QP Corporate Philosophy for Fire and Safety, clause 16.8.2, offshore plants. 1. Passive Fire Protection (PFP)

PFP will be considered, where necessary, to prevent the passage of heat, flame, smoke and gas for a period sufficient to enable safe escape and evacuation of personnel. The designation of fire-exposed areas and the selection of structures, equipment and instrumentation requiring fireproofing shall be in compliance with the QP specifications. 2. Active Fire Protection

The new escape routes shall be protected by fire water system in accordance with the existing active fire protection philosophy and specifications. Active fire protection systems and equipment shall be provided based on credible fire hazard scenarios. The firewater philosophy considers provision for control of burning, exposure protection and escape ways protection.

APPENDIX A Other existing firewater system component such as fire hydrants, monitors and hose reels shall be addressed in detail to ensure adequate coverage for the new FACILITY. The existing fire fighting equipment shall be relocated, if required. Appropriate portable fire extinguishers shall be provided for the escape routes in accordance with NFPA 10. The dry chemical fire extinguishers with a travelling distance of 15 m shall be provided for the proposed new escape routes. Foam and dry powder trolley units shall be allocated in areas with potential pool fire hazards. Handheld Carbon Dioxide extinguishers shall be provided to fight cable or electrical fires. Any deviations from the above shall be submitted to QP for approval 2.5 CONSTRAINTS Due to location of lifts the existing platform crane cannot be utilized to install the new personnel lifts. The rack and pinion type of lift requires a high starting current that is higher than the capability of the existing 190kVA emergency diesel generator. Hence in case of normal power failure the lift can not be fed from emergency diesel generator.

Daily Operations are not to be effected 2.6 INTERFACE with OTHER PROJECTS Projects being executed by QP and others may have an impact on the scope of this CONTRACT. CONTRACTOR shall take due consideration of these in his design, construction, and program. CONTRACTOR shall identify all requirements for information from other QP projects, which may impact on the WORK, and shall notify QP of the requirements for information in adequate time to ensure that the WORK is not delayed. It is the responsibility of CONTRACTOR to coordinate with all relevant parties to obtain the necessary information.

DESIGN BASIS
The design shall be carried out in accordance with good engineering practice, latest revisions of all QP Standards, recognised International Codes of Practice, Standards and Specifications and their addenda. List of applicable QP and International codes and standards are provided and

APPENDIX A 3.6 DESIGN LIFE The design life of the FACILITY shall be 25 years, with due note being taken of the operating regime and of the environmental conditions. The minimum continuous period of operation shall be 4 years for overall maintenance shutdown. 3.7 3.7.1 DESIGN CRITERIA General CONTRACTOR shall design, develop and engineer the entire FACILITY taking into account the following: 1) Design requirement 2) HSE/ safety for personnel & environment 3) Operability & maintainability 4) Compatibility & case of interface 5) Techno-Economical viability. 3.7.2 Mechanical The detail design work shall be undertaken in accordance with all the appended CONTRACT documentation and in compliance with all the relevant Design Bases Memorandum, Data sheets, Standards and Specifications referenced in the CONTRACT documents. 3.7.3 Piping The detail design shall be in accordance with QP Piping General Design Guidelines. 3.7.4 Instrumentation / Fire & Gas Detection System All instrumentation WORK shall be designed based on approved design basis, design drawings/documents and criteria to satisfy the requirements as specified in the Scope of WORK. APPENDIX A at PS-3Q, shall be provided by QP and shall be updated by CONTRACTOR during detailed engineering stage. The CONTRACTOR shall use these Specifications for the engineering design of the WORK. The General Specifications define the minimum design practice and requirements for the WORK. Additional or more stringent requirements may be imposed by other documents such as design drawings and equipment data sheets. The CONTRACTOR shall not modify the requirements or deviate from the requirements of the General Specifications or the more stringent requirements of other Project documents without prior written approval of QP. 3.7.5 Electrical The WORK shall be designed in accordance with all the appended CONTRACT documentation and in compliance with all the relevant Standards/Specifications referenced in the CONTRACT documents. The CONTRACTOR shall provide the electrical equipment in line with these specifications. It is the responsibility of the CONTRACTOR to familiarise himself fully, by site surveys, retrieval of document, with all aspects of the existing platform electrical system in order to ensure full compatibility between all new and existing systems. Any equipment or study not specifically covered in this document but is obviously necessary for the safe installation, operation and maintenance of the Personnel Lift, as intended, shall be deemed to be included in the CONTRACTOR Scope of responsibilities. 3.7.6 Structural / Architectural The detail design WORK shall be undertaken in accordance with all the CONTRACT documentation and in compliance with all the relevant Design Bases Memorandum, Drawing, Standards and Specifications referenced in the CONTRACT documents. 3.7.7 Loss Prevention All new safety and fire protection systems shall comply with the latest QP Corporate Fire and Safety Philosophy and shall be compatible with the existing systems. 4.0
4.1

SCOPE OF WORK
GENERAL
It is not the intent of Appendix A to specify completely the details of design, documentation, materials, procurement, supply, fabrication, inspection, demolition, installation, painting, pre-commissioning, commissioning, start-up and performance testing of the FACILITY. It is CONTRACTORs responsibility to ensure that the FACILITY is designed, procured, installed, pre-commissioned, commissioned, and tested in accordance with QP and international codes and standards specified herein, statutes, regulations, and safety codes, and is also consistent with good engineering practice to provide a safe FACILITY meeting the complete functional requirements as specified herein. CONTRACTOR shall be responsible for the complete Design and Detailed Engineering, Procurement, Installation, Construction / Demolition, Precommissioning, Commissioning and Testing of the FACILITY. The general scope split between EPIC CONTRACTOR and Lift Package SUBCONTRACTOR is tabulated in the Study report on Execution Plan and Method Statement, Doc. No. 2480-2012. QP shall approve all CONTRACTOR-prepared documents before commencement of the procurement and construction activities. Such approval by QP shall not relieve CONTRACTOR of his obligation to perform design/engineering so as to meet the intended FACILITY performance and quality requirements. The FACILITY shall be handed over to QP in a Ready-for-Startup state, totally integrated, as applicable, into the existing facility, utility and safety systems. Prior to final acceptance by QP, CONTRACTOR shall demonstrate that the FACILITY complies with the requirements stated in the CONTRACT. Any conflicting requirements shall be brought to QPs attention and CONTRACTOR shall not proceed without written approval from QP. In the absence of any guidance or agreement to the contrary, the most stringent requirements shall apply. The CONTRACTOR shall undertake the WORK as detailed within this Scope of work for the development of the Installation of Personnel Lifts at PS-2Q & PS-3Q Project, which includes, but is not limited to, the following: 1) Demonstrating the performance guarantees; including developing all necessary procedures.

APPENDIX A the WORK, including detailed design, engineering, procurement, onshore fabrication and construction, load-out, offshore transportation, offshore installation and hook-up, onshore and offshore pre-commissioning and commissioning, start-up and initial operation and maintenance of the FACILITY, management and supervision of all SUBCONTRACTORS and ancillary or temporary services which are necessary for the expeditious COMPLETION of the work in accordance with the CONTRACT.
b) Preparation and submission of Organisation Charts for detailed design engineering, procurement, onshore fabrication and construction, load-out, offshore transportation, offshore installation and hook-up, onshore and offshore precommissioning and commissioning, nominating key personnel for approval by QP. c) Co-ordination and control the WORK of each WORK group, e.g., design engineering, procurement services, project control, contracting, construction, pre-commissioning and commissioning to ensure the WORK is completed within the SCHEDULED COMPLETION DATE and according to the specifications and drawings as provided in Appendix F which shall be further detailed / developed by CONTRACTOR during the detail engineering phase and approved by QP. d) Undertake expediting and inspection of SUBCONTRACTORS equipment, materials and documentation as required by this CONTRACT. e) Provision of SUBCONTRACTORS assistance as necessary during construction, pre-commissioning, commissioning, start-up, initial operation and maintenance and performance test. CONTRACTOR shall be responsible for providing to QP, a full list prior to construction phase. of SUBCONTRACTORS required to assist QP/CONTRACTOR in all phases of the WORK. The CONTRACTOR shall be responsible for all travel, accommodation and subsistence, entry visa and transport arrangements for the SUBCONTRACTORS representatives. The SUBCONTRACTORS representatives shall be under the direction of, and shall report to, the CONTRACTOR. f) Implement a safety program throughout the execution of the WORK and submit Weekly and Monthly Safety Reports.

APPENDIX A g) Implement a quality assurance programmed throughout the execution of the WORK and submit periodic reports to QP.
Every week during the installation and testing of the WORK, CONTRACTOR shall prepare an installation and Commissioning report detailing the activities, test results, problems and resolution and submit to QP. h) Carry out quality, safety, technical and schedule audits with QP attendance and advise QP of the results of such audits, together with appropriate action taken and implement any recommendations into the design. i) Prepare a complete Project Administration Manual for the execution the WORK for approval by QP in accordance with the requirements of Appendix D and this CONTRACT. Undertake all management and co-ordination of all external interfaces associated with the WORK. These interfaces cover engineering, procurement services (including supply), construction, pre-commissioning, and commissioning activities.

k) Attend all management meetings with QP at CONTRACTORS home office, WORKSITE and/or QP Offices in Qatar in accordance with the requirements of the CONTRACT. l) Prepare a close-out report in accordance with the requirements and guidelines provided in EXHIBIT 11 and Appendix D. The CONTRACTORS close-out report development and update shall be done on monthly basis from the EFFECTIVE DATE in order to ensure the integrity and comprehensibility of the report.

m) Weekly and Monthly Reports covering all ongoing and planned activities encompassing all aspects of WORK. The contents and the format of information provided in the reports shall be in accordance to Appendix D to this CONTRACT. n) Undertake any and all other activities not covered above but required under the CONTRACT.

4.1.3 Documents Required from CONTRACTOR

CONTRACTOR shall be responsible for the demolition of existing facilities, as required by the CONTRACT. CONTRACTOR shall refer to the drawing for the demolition WORK scope. Refer the Study report on Execution Plan and Method Statement, Doc. No. 2480-2012 for Construction and Destruction drawing reference. CONTRACTOR shall prepare detailed drawings together with method statements to describe the sequence of the demolition WORK for the approval of QP. Demolition schedule shall highlight the sequences in relation to installation, operations and completion activities. CONTRACTOR shall provide all temporary supports required during the actual demolition/modification.

4.1.5 Performance Tests


The CONTRACTOR shall as soon as practical, commence the Performance Tests, in accordance with the requirements of this APPENDIX A to ensure all the performance guarantees are met.

4.1.6 Verification of Guarantees


The performance guarantees shall be verified in accordance to recognised international and QP Standards during the performance test. The detailed procedure to be used for conducting the performance test shall be developed by CONTRACTOR during the detailed engineering phase and mutually agreed and approved by QP.

Adequately qualified instructors shall provide such training. Proposed duration for training is one day for each group (shift). CONTRACTOR shall evaluate and submit his recommendation in this regard. Training shall be provided for two separate group (shifts) comprising a minimum of six people each shift, formed by Maintenance and Production Maintenance Staff. Training courses shall be scheduled well in advance of the scheduled commissioning of the lifts to allow trainees to participate in that commissioning. CONTRACTOR shall submit the training course content and schedule for QP review and approval. CONTRACTOR shall provide the trainees with all necessary training materials and tools to be used in the classroom and in the field.

APPENDIX A CONTRACTOR shall retrieve and verify accuracy of all relevant existing drawings and documents related to the WORK and to other related projects from various locations.
QP will assist CONTRACTOR in retrieving the documents, however, the overall responsibility for co-ordination lies with CONTRACTOR, whether native files of existing drawings/ documents are not available. CONTRACTOR shall reproduce/ re build the AutoCAD files from hard copies & update them for as built condition. CONTRACTOR shall compile a Survey Report containing all information gathered during this activity. Survey findings shall be arranged in order of location, plant, and sub-divided into discipline and distributed using the Project Document Distribution Matrix. CONTRACTOR shall survey all superfluous equipment, panels etc. and all auxiliaries and prepare the list of equipment that shall be demolished.

4.3

DESIGN BASIS MEMORANDUM


Upon verification of the design information and completion of the Site Survey, CONTRACTOR shall prepare the Design Basis with this information and shall prepare a Design Basis Memorandum (DBM) and submit to QP for review and comment. 2. CONTRACTOR shall check, review and verify the design and engineering information contained in this document including all aspects of the FEED supplied by QP to satisfy himself that they are adequate for the purpose in accordance with the CONTRACT. CONTRACTOR shall also check for errors, omissions, conflicts and technical accuracy. Any additional scope required to achieve the CONTRACT objectives shall be carried out by CONTRACTOR. 3. CONTRACTOR shall advise QP of any design alternatives that may offer better techno-economic solutions. 4. CONTRACTOR shall base his design on proven technology and on established design and construction techniques in the oil and gas industry. 5. The CONTRACTOR shall not assume that any documents or drawings in their possession at or prior to the award of CONTRACT are up-todate and CONTRACTOR shall not use such documentation in the performance of the WORK under this CONTRACT. The latest revisions of all drawings or documents deemed relevant to the CONTRACT shall be requested from QP in writing. CONTRACTOR shall also verify these drawings during site survey, for the as-built information and incorporate any missing information with QP approval, before proceeding with the detail design. 6. CONTRACTOR shall be responsible for correcting, any drawings or documents that are found to be incorrect due to insufficient site checks, errors, or omissions on the part of CONTRACTOR. 7. CONTRACTOR shall liaise with QP and other parties to finalize all tiein points with existing facilities and incorporate them in drawings and documents related to this CONTRACT. 8. CONTRACTOR shall ensure the maintainability criteria, maintenance philosophy and spare parts philosophy are fully considered in the development of the detailed design. 9. CONTRACTOR shall ensure that the detailed design and engineering is in full compliance with QPs latest corporate Standards, latest edition of International Codes, Specifications and Standards and guidelines, including those recently issued during/after FEED and hence were not incorporated into the FEED. 10. CONTRACTOR shall carry out all necessary calculations and conduct all appropriate design procedures and analysis as required to optimise the sizing and selection of equipment and the specification of materials.

11. CONTRACTOR shall produce all necessary engineering drawings, data sheets, specifications, purchase requisitions, procedures, and method statements for COMPLETION of the WORK. 12. CONTRACTOR shall produce and update equipment lists. QP shall allocate block tag numbers upon CONTRACTORS submission of tag request for incorporation into detailed design. 13. CONTRACTOR shall submit a list of proposed design software for QP approval prior to start of design work. Where requested by QP, CONTRACTOR shall submit software validation. 14. All design calculations shall be documented in reports, which CONTRACTOR shall submit to QP on completion of the design for approval. 15. CONTRACTOR shall produce all drawings in accordance with ES.D.10, (Latest Issue), provided in Appendix F. The drawings shall be created using AutoCAD software, Release 2007. 16. The personnel lift shall be designed taking into account the following minimum requirements: 1. The personnel lift shall be Rack and Pinion type. 2. The personnel lift shall be reliable and safe form of transport, suitable for use within a robust offshore environment. 3. The personnel lift should be of industry standard configuration suitable sized to accommodate the man-handling of a MEDIVAC casualty case from the plant area (Deck A) to the Sick bay (Level 3) of the LQ complex. The specifications of the personnel lift should be to an approved industrial norm, suitable for use on an offshore platform. The lift will have of total of five floors for disembarkation, namely Deck A, Deck B, Level C, Level 2 and Level 3 only. The travel distance shall be 19.40m from Deck A (TOS EL +99.970) to Level 3 (TOS EL +119.370).

APPENDIX A 4. The lifting capacity or the SWL (Safe Working Load) of the personnel lift, car size, travelling speed, waiting call time, etc. are all to be evaluated and considered when transporting the following:
Operations and maintenance personnel Casualty personnel on a stretcher Movement of kitchen provisions, furnishings and other goods between levels.

5. The personnel lift shall be of marine design and resistant to shock by boats berthing alongside the platform. The material for the lift shall be suitable for the offshore saliferous environment. 6. The landing doors shall be of centrally manual opening type and the car door shall be sliding manual opening type. 7. The car shall be constructed of anodized extruded Aluminium panels bolted to floor and roof to form a complete unit. The roof and floor shall be constructed of durable, aluminium plate with a non-skid surface. 8. Lift shall be provided with the GFD type safety device which stops the car if normal driving speed is exceeded. 9. The car and landing doors shall be interlocked to prevent the lift from operating unless the doors are closed and locked. The door cannot be opened unless the car has stopped at a landing. 10. The lift car shall be provided with fan & ventilation grids to create user friendly environment inside the car. 11. The lift car shall be provided with the trap door/emergency door on top of the lift cabin with ladder inside the car to allow access to the roof. 12. The design life shall be 25 years, with due note being taken of the operating regime and of the environmental conditions. The minimum continuous period of operation shall be 4 years for overall plant maintenance shutdown. 13. Deleted 14. APPENDIX A Identification / Quantification of demolition & decommissioning work scope

CONTRACTOR shall comply with the Minimum Instrumentation Design Requirements specified in Attachment 9, Project Specifications and applicable QP & International standards stated in Project Specifications. Requirements for instrumentation not covered shall be developed by the CONTRACTOR and be approved by QP. All specifications and datasheets etc. shall be submitted for QPs review and approval prior to implementation at site. CONTRACTOR shall develop a detailed design document package for engineering installation, testing and commissioning in accordance with the Standards and Codes listed, referred to in the contract documents. All the design documents shall be reviewed and approved by QP prior to implementation of the same at site. CONTRACTOR shall meet the testing and commissioning requirements as specified in this specification and other FEED documentation. CONTRACTOR shall perform the tie-in to the existing control systems as specified in the FEED documents. CONTRACTOR shall verify the tie-ins and plan the shutdown requirements, if required for co-ordinating with QP to execute the same. The CONTRACTOR shall arrange for the purchase of third party equipment within his scope and shall supply other equipment as indicated in the CONTRACT. The CONTRACTOR shall provide a full drawing and documentation package and shall perform his activities in accordance with the agreed supplier quality programme requirements. The CONTRACTOR shall carry out site surveys and data gathering including both hardware and software information as necessary and shall liaison with the QP to gain access to QP facilities. After site survey, the CONTRACTOR shall prepare and submit a detailed site survey report to the QP for review /approval. The CONTRACTOR shall perform all site installation and testing activities with the minimum disruption to existing operations. The CONTRACTOR shall liaise with the QP to co-ordinate site activities. The CONTRACTOR shall be responsible for carrying out all activities required to provide a complete and operable personnel lifts at PS-2Q & PS-3Q consistent with the CONTRACT objectives. All the instrument, controls and related equipment shall be specified to operate for a minimum continuous period of four years, withstanding the environmental conditions mentioned in the specification. The CONTRACTs Fire & Safety philosophy shall be based on QP Fire & Safety Philosophy QP-PHL-S-001.

Design shall be in accordance with QP safety philosophy QP-PHL-S001 Rev2 section 16.9. Two way communications from inside the lift to CCR shall be provided. Cabling for the same shall be routed to CCR via Radio Room. The multicore Telephone cables for the above shall be routed from the lift shaft pin to proper location in CCR to enable establishment of proper communication system. There shall be proper segregation of telephone and intercom cables. Voice Telephone Voice (telephone) communication system shall be provided to lift car. Subscription shall be via existing platform PABX switch. Multipair copper cable shall be extended from nearest telephone junction box to proposed location of lift car half-height junction box to allow the hook-up of lift car telephone to platform PABX switch. Voice (telephone) communication system shall have emergency power back-up in case of loss of normal power to ensure that lift passengers are able to contact control room under any emergency circumstances. PA/GA System There shall be a PA/GA system installed in the lift car in order to alert the lift passengers in case of emergency or to annunciate the platform confirmed gas or confirmed fire alarm. Redundant PA/GA system is envisaged necessary to provide high system reliability. Existing PA/GA loudspeaker loops shall be extended from nearest PA/GA junction box to lift car half-height junction box to allow the hookup of lift car PA/GA loudspeakers to platform PA/GA main equipment. The PA/GA speaker and other components to be installed in the lift car shall be Federal Signal make in line with the existing system.

4.4.9 Electrical 1. General

Requirements for electrical not covered shall be developed by the CONTRACTOR and be approved by QP. All specifications and datasheets etc. shall be submitted for QPs review and approval prior to implementation at site. The CONTRACTOR shall develop a detailed engineering and design package for engineering installation, supply of materials, labour, supervision, tools, equipment, testing, commissioning, and other services necessary to provide power supply and control interface to the personnel lifts in PS-2Q and PS-3Q. All the design documents shall be reviewed and approved by QP prior to implementation of the same at site. CONTRACTOR shall meet the testing and commissioning requirements as specified in this scope of WORK and FEED documentation. CONTRACTOR shall perform the tie-in to the existing power and control systems as specified in the FEED documents. CONTRACTOR shall verify the tie-ins and plan the circuit isolation or shutdown requirements, if required for co-ordinating with QP to execute the same. The main requirements but not limited to the following shall be included in the scope of CONTRACTOR:

APPENDIX A 3. Design Philosophy a) Power Supply


The personnel lift Mains power supply requirement shall be feed from 400V Normal Supply Boards in the PS-2Q (PS-3Q). Lift separate control supply and emergency/escape lighting requirements shall be from 230V Emergency Distribution Boards in the PS-2Q (PS-3Q).

b) Available System Voltage and Frequency


Lift Mains Distribution Supply : Lift Separate Control Supply : Walkway Lighting : 400/ 230V, 3 phase, 4 wire, 50Hz, solidly earthed 230V, 1 phase, 2 wire, 50Hz, 230V, 1 phase, 2 wire, 50Hz

In general, the electrical distribution system and equipment will be designed for a voltage variation of 10% and the frequency variation of 5%. However the combined variation of voltage & frequency will not exceed 10% at any time. All equipments shall be suitable for operation for these tolerances.

c) Load Requirements
Personnel lift power requirement shall be as per Lift SUBCONTRACTORS offer and confirmed requirement during purchase. CONTRACTOR shall review and validate SUBCONTRACTORS electrical data available in the FEED. The verified and validated data shall be use as basis during detailed design. Additional lighting loads shall be connected to the existing lighting circuit loops. Refer to FEED package for details.

d) Main Electrical Equipment


NORMAL SUPPLY BOARD (400V)

There are available spare cubicles in the Normal Supply Boards of PS2Q and PS-3Q. This will be retrofitted to suit the mains power requirements of the personnel lift. Refer to FEED documents for details. Wherever possible, the equipment type and models including trays and front cover retrofit shall be sourced / purchased from the same Switchboard Manufacturer that supplied the existing Normal Supply Board. CONTRACTOR shall validate at site the existing electrical equipment/component manufacturer, type and model including existing purchase order number as its reference in specifying retrofitted switchboard cubicles.

DISTRIBUTION BOARDS

Existing spare circuits in the Emergency Lighting and Small Power Distribution Board shall be utilised for the 230VAC, 1-phase control supply requirements of the personnel lift. Additional lighting requirement due to extended walkways and new lift landing area shall be connected from the existing lighting loops / circuits that are also feed from the existing Emergency Lighting and Small Power Distribution Board. These walkway and lift landing area lighting are considered as escape lighting.
PERSONNEL LIFT PACKAGE SYSTEM

The personnel lift is a complete system package to be completely engineered, supplied and delivered by Lift SUBCONTRACTOR. Installation, testing and commissioning by CONTRACTOR under the supervision of Lift SUBCONTRACTOR. Electrical equipment i.e. lift motors, controls and protection system are integrated with the lift car and its micro-processor based lift control system. Electrical power and control cables from the Lift Base Panel to the Lift Car are provided, installed and commissioned by SUBCONTRACTOR. The CONTRACTOR shall be responsible to validate, confirm and provide all the necessary electrical engineering, installation, testing and commissioning. CONTRACTOR shall be responsible in considering the Lift SUBCONTRACTORS voltage drop requirements to the lift system. CONTRACTOR shall coordinate with Lift SUBCONTRACTOR during detailed engineering to validate the information available during FEED and ensure that any changes by the Lift SUBCONTRACTOR are incorporated in the detailed design.

e) Cabling and Accessories Refer Design Basis Memorandum, Doc. No. 2480-9500 for details of cables, cable glands, cable sizing considerations etc. f) Lighting System
Generally, the Manufacturer and type of lighting fixtures shall be the same as existing which is 2 x 36W emergency fluorescent lamps with type protection Ex-e, Manufacturer Chalmit (or approved equivalent) suitable for hazardous areas. The lighting fixture shall be IP66 ingress protection (the same as existing installation). The new additional

The additional lighting requirements, where possible, shall be tied-in to the existing emergency lighting circuit loops. Refer to FEED documents for reference and guidance on existing lighting loop and circuit arrangements.
All lighting poles shall be designed to be the same as existing. Lighting installations shall be designed to provide the average inservice illuminances, in lux (lx) specified for the locations (as per Specifications ES.2.03.0001 revision B). g) Earthing System
The personnel lift including its drive motors and all its electrical components and the additional lighting fixtures/poles shall be connected to the existing earthing grid via the lowest practical impedance path. Lift Manufacturers earthing requirements shall be followed. Generally for rack and pinion hoist and lift the following should be noted: In addition, the base enclosure, mast, etc. shall be connected to earth by means of the green/yellow earth PE-conductor in the electrical supply connected to the earthing bar in the base electrical panel. The landing assemblies shall be connected to earth by means of bolting/fitting to the platform structure and the green/yellow earthing PE-conductor in the electrical wirings to the landings. The car lift is a metal structure. Drive motors and all electrical components having metal enclosure shall be connected to the earth by means of G/Y PE conductor. In addition, they shall also be connected to the electrical earth bar by means of a green/yellow PE-conductor. The entire lift including the electrical installation therefore forms a Faradays cage providing excellent protection for the passengers against touch voltage. The lift car, the base enclosure, the mast, the landing assemblies, the platform structure and the electrical earthing system should be positively bonded to each other and connected to the earth potential. Lightning Protection System, The personnel lift and all its electrical components shall be bonded to the existing platform. For offshore platforms no lightning protection is required.

The minimum size of earth conductors shall be 70 mm2 for main conductors and 35 mm2 for small electrical equipment.

h) Lift Drive Motors


The lift drive motors shall form an integral part of the personnel lift package and shall be suitable for use in the hazardous location classified as Zone 2, Group IIA, temp class T3. All motors shall be rated for continuous duty and shall be designed for direct on line starting unless otherwise specified for other method of reduce starting. Motor standard rating and voltages shall be suitable for power supply 400V, 3 phase, 50Hz. The motors shall be provided with anti condensation heaters.

4. Selection of Electrical Equipment for Hazardous Area


Electrical equipment for the lift including motor drive and safety devices shall be suitably specified for operation considering a hazardous area of Zone 2, Gas Group IIA, T3, as minimum. The FACILITY shall be designed in accordance with the requirements of the CONTRACT, and specifically with QP-PHL-S-001 QP Corporate Fire & Safety Philosophy and the project specific safety documents. CONTACTOR shall undertake a full survey of the existing plant and identify requirements for the additional safety and fire protection design in compliance with the QP HSE standards. CONTRACTOR shall review the design to ensure that health, safety and environmental aspects, including loss prevention, are addressed and appropriate measures taken in the design. The key activities are as follows: Develop, detail and finalise the Fire and Safety philosophy. Interface with various disciplines to develop, detail and finalise any HSE requirements. 4.4.23 Demolition of Existing Facilities


CONTRACTOR shall prepare a demolition package for all redundant and superfluous equipment, cables and auxiliaries etc. The package shall comprise all the demolition drawings, material take-off for tagged items, procedures and method statement. The demolition shall preferably be carried after commissioning of the new FACILITY.

4.4.24 Long Lead Items


CONTRACTOR shall identify items that have a delivery time greater than 9 months to the CONSTRUCTION SITE.

4.4.25 List of Deliverables

The CONTRACTOR shall ensure that the supply of all equipment, bulk materials, spares, consumables etc, in sufficient quantities so as not to have a detrimental effect on the progress of the WORK and to ensure that the CONTRACTOR meets the CONTRACT SCHEDULED COMPLETION DATE. The provision of equipment and bulk materials shall include, but not be limited to the following, to ensure the requirements of the CONTRACT are met: a. b. Supply of equipment; Supply of bulk materials, which shall include sufficient overbuy and spares per type and size of material so as to prevent holdups or disruptions in the performance of the WORK and to ensure that the CONTRACTOR meets the SCHEDULED COMPLETION DATE. The CONTRACTOR shall identify and provide all necessary initial inventory and six month operation consumables. Initial inventory shall cover the quantities required during the initial operation and maintenance in addition to pre-commissioning and commissioning of the FACILITY and shall be in accordance with the SUBCONTRACTORS recommendations. List of six month operation consumables alongwith quantities shall be submitted by CONTRACTOR before commissioning phase and approved by QP. CONTRACTOR shall establish SUBCONTRACTS with SUBCONTRACTORS approved by QP, to meet the requirements of this CONTRACT and in accordance with procedures agreed with QP. CONTRACTOR shall identify which elements of the Scope of WORK will be subcontracted and shall provide a list of all potential SUBCONTRACTORS in accordance with Appendix E. The CONTRACTOR shall indicate in Appendix E those SUBCONTRACTORS with whom CONTRACTOR has long term supply or call-off agreement and therefore provision of goods or

10. In establishing any SUBCONTRACTS the CONTRACTOR shall undertake a formal tendering exercise with a minimum of three (3) bidders capable of undertaking the WORK. The CONTRACTOR shall provide standard documentation and have a fully editable subcontracting process covering SUB-CONTRACTOR Selection, Bid Exercise, Tender Evaluation (Technical and Commercial), award including review process, which shall be approved by QP. Minimum SUBCONTRACTING Document required shall be in accordance with Exhibit 1. 11. The CONTRACTOR is required to comply with the list of SUBCONTRACTORS approved by QP as given in Exhibit 12 of this Appendix A-Scope of WORK. Approval of proposed SUBCONTRACTORS, for materials and services, which are not listed in Exhibit 12, shall be entirely at QPs discretion. 12. Deleted. 13. 4.5.2

Procurement

The CONTRACTOR shall undertake all necessary procurement activities to purchase all equipment, materials, chemicals, lubricants and other facilities to complete the WORK in accordance with the requirements of the CONTRACT. The procurement activities will include, but not be limited to, the following: 1. Prepare procurement documentation for equipment and materials that incorporates all necessary CONTRACT documentation, including purchasing specifications, data sheets Procurement shall be on a world wide competitive basis, except when technical specifications or regulations require otherwise, abiding where necessary by Qatar State regulations regarding countries of origin; Prepare RFQ specifications, data sheets and documents for equipment and materials in accordance with the CONTRACT. Ensure that the requirement of pre-commissioning, commissioning, start-up, Two Years Operational Spare Parts and Insurance Spares, SPIR forms and final documentation to be reviewed by QP as per Exhibit 1; The RFQ shall specify the quality program with which SUBCONTRACTORS shall comply and that the requirements are to the latest ISO 9000 series, and QPs QA/QC procedure QSP-QA-09, which is included in Appendix F;

APPENDIX A Review bids received from SUBCONTRACTORS and prepare technical and non-priced bid tabulations nominating the selected SUBCONTRACTOR and identifying compliance, non-compliance and improvements in offers. Review all equipment/material selections and verify their suitability and operability for the service intended. SUBCONTRACTORS shall be selected based on compliance with purchase specification, data sheets, scope of WORK, maintainability criteria, maintenance philosophy and sparing philosophy as per the guidelines stipulated in EXHIBIT 5. Before placement of purchase orders, CONTRACTOR shall assess and ensure that the financial status, availability of SUBCONTRACTORS to provide recommended pre-commissioning, commissioning, startup, Two Years Operational and Insurance Spare Parts (including quantity and unit prices thereof), present workload and suitability of the SUBCONTRACTORS are satisfactory.
Minimum SUB-CONTRACTOR Documentation Requirements and QP rule shall be as stipulated in EXHIBIT 1. CONTRACTOR shall provide, for QP endorsement, their materials management controls system procedures. a. i) Materials and Equipment Handling, Storage and Preservation The CONTRACTOR shall undertake all materials and equipment handling, storage, warehousing and preservation as required under the CONTRACT to ensure the SCHEDULED COMPLETION DATE is achieved.

ii) Fragile equipment sensitive to weather or WORKSITE conditions (such as instruments) shall be stored by the CONTRACTOR prior to their installation in the WORK in totally heated/air-conditioned rooms. When fragile equipment has been installed in the WORK they shall be protected from local prevailing environmental conditions and potential damage in accordance with SUBCONTRACTORS instructions. iii) The CONTRACTOR shall be responsible for and ensure; 1. Provision of warehouses and outdoor storage facilities with fencing and adequate fire and security measures at all times. 2. All equipment and materials shall be carefully handled and stored in a designated area at the WORKSITE that clearly indicates the material/equipment is for the CONTRACT and is the property of QP. 3. Handling, preservation and storage of equipment and materials shall be carried out in conformance with SUBCONTRACTORS instructions. 4. The CONTRACTOR shall provide experienced personnel for warehouse management and goods inward/outward control utilising PC based software allowing the status of materials to be given at all times. b. i) Expediting (Desk and Field) In order to ensure the requirements of the EXECUTION PROGRAM are met, the CONTRACTOR shall perform proactive field and desk expediting of equipment, materials, SUBCONTRACTORS data, technical assistance, personnel and documentation from the placement of order to delivery at the WORKSITE. The CONTRACTOR shall establish and maintain an electronic management system to monitor progress on all the purchase orders and prepare and issue a detailed schedule and status reports to QP on a monthly basis. The electronic management system shall be able to identify those

APPENDIX A items of the equipment and materials, which are critical or are potentially critical from the EXECUTION PROGRAM point of view and require special attention to ensure timely delivery, together with an action plan to achieve it. The CONTRACTOR shall provide electronic copies of all field-expediting reports to QP within 24 hours of completing the visit backed up by a formal paper copy within two (2) working days. If there are problems, QP shall be notified immediately of the problems together with actions taken by CONTRACTOR to rectify the problems.
ii) All expediting efforts shall be directed towards achieving or improving the equipment and material delivery dates while maintaining the purchase order design specification, conditions and SCHEDULED COMPLETION DATE. Any actual or potential deviation or slippage from these requirements shall be immediately transmitted to QP in electronic format. The CONTRACTORS expediting activities shall include, but not be limited to, the following: 1. Take all appropriate actions to obtain all technical data and other documentation, including completed SPIR forms and final documentation, from the SUBCONTRACTORS in a timely manner. 2. Expedite all replacement materials, associated documents and SUBCONTRACTORS assistance during the guarantee period. 3. Ensure all documentation associated with all SUBCONTRACT or procurement activities are received within one month of the delivery . c. Inspection and Testing i) The CONTRACTOR shall execute Field inspection activities for all equipment and materials in accordance with EXHIBIT 7 of this Appendix A Scope of WORK and the CONTRACTORS own procedures to ensure that materials and equipment are provided to the correct standards and specifications. The CONTRACTOR shall maintain an electronic management database for all inspection activity and shall provide QP with an electronic copy of the inspection report within 24 hours of completion of each inspection visit, backed up by a formal paper copy within two (2) working days. ii) Shop inspection shall be carried out by CONTRACTOR at both the point of manufacture and, where necessary, at the source of SUBCONTRACTORS materials.

APPENDIX A iii) The CONTRACTOR shall be responsible for all quality assurance activities including SUBCONTRACTORS inspection. CONTRACTOR shall comply with the requirements of the FEED specifications included in Appendix-F.
iv) Provision shall be made at all times for QP to have the option of participating in pre-inspection meetings, inspection visits and test witnessing. The CONTRACTOR shall prepare anticipated weekly testing programs providing QP sufficient (at least fourteen days) advance notice for their participation. The CONTRACTOR shall note that QP will witness the Factory Acceptance Test (FAT). The CONTRACTOR shall provide twenty-one days notice to QP for FATs. Inspection reports shall be issued electronically to QP within 24 hours of the inspection visit. The CONTRACTOR shall prepare and provide to QP an inspection co-ordination procedure to be developed in accordance with the CONTRACT. QP Personnel shall be mobilized to the SUBCONTRACTOR premises, at the discretion of QP, for critical inspections (stage inspection during manufacturing or FAT). However, if the scheduled inspection is delayed or cancelled, due to which there is additional mobilization/ demobilization or extended stay of QP personnel, then the cost of such additional mobilization, demobilization or extended stay shall be back charged to the CONTRACTOR. v) The CONTRACTOR shall submit to QP electronic format written reports on all inspections carried out, which are in sufficient detail for QP to monitor the effectiveness of inspection. vi) The CONTRACTOR shall compile all fabrication and manufacturing data folders, containing mill test certificates and all other relevant inspection data, and submit the same upon delivery of equipment and/or material to the WORKSITE. CONTRACTOR shall comply with the requirements of the FEED specifications for compilation of Quality Data Dossier in Appendix F. vii)The CONTRACTORS inspection procedure shall be endorsed by QP and shall meet the electronic management requirements. Status reporting shall be undertaken and provided to QP on a monthly basis.

All third party inspection reporting shall be simultaneous and in equal detail to both QP and CONTRACTOR. All major equipment and systems shall be subject to Factory Acceptance Tests (FATs). QP Nominated Representatives may elect to witness all Factory Acceptance Tests (FATs) and Site Acceptance Tests (SATs). CONTRACTOR shall give QP sufficient notice of any such testing to enable a Contractor appointed inspector to witness the testing.
d. Shipment and Freight Forwarding i) The CONTRACTOR shall be responsible for sanctioning the release of completed equipment and materials for shipment after satisfactory completion of final inspections and certification and shall execute forwarding activities for all equipment/material. ii) The CONTRACTOR shall be responsible for organizing and coordinating packing, marking and transport of equipment from the source of supply determined in the purchase order to the CONTRACTORS Fabrication facilities, Qatar or Offshore location such work shall include: 1. Preparation of seaworthy/airworthy packing, marking, shipping and documentation specifications; 2. Inspection and certification as required for packing, marking and loading; 3. Taking responsibility for authentication of shipping documents (including Certificate of Origin) by Qatar Embassy/Consulate; 4. Develop plans and procedures for freight forwarding handling and customs clearance of equipment and materials 5. Organize and ensure timely shipment of materials and equipment to the WORKSITE.

APPENDIX A Prepare, maintain and issue a monthly shipping progress report and monthly shipping forecast in a format approved by QP; 7. For Long Lead Items, the CONTRACTOR shall take responsibility for the goods ex-works and shall ensure timely delivery to the CONTRACTORS onshore fabrication site. 8. Arrange for the importation of all materials and equipment purchased, for receiving and off-loading at the WORKSITE; 9. Arrange for the insurance of all freight at all stages of shipment to ensure adequate cover against loss or damage to equipment so as not to jeopardize QPs interest. 10. CONTRACTOR shall comply with all the requirements of the specification for Goods protection during Export Shipment instructions included in Appendix-F.

4.5.4 Third Party Certification (TPC)


1. The CONTRACTOR shall be responsible for arranging the services of QP approved TPC Agency, where required, for equipment and materials in the country of origin for statutory, insurance, or any other reasons. The requirement for TPC services shall be as stipulated in equipment/material specifications and scope of services as per Exhibit 4 to this Appendix A Scope of WORK. The TPC shall function independently and report simultaneously to QP and the CONTRACTOR. The CONTRACTOR shall prepare a Certification Matrix in accordance with the requirements of EXHIBIT 4 for QPs review/approval. 2. The TPC scope of WORK shall include, but not limited to, the following: a. Checking project equipment and bulk material items; b. Attending Manufacturing kick-off meetings and pre-inspection meetings to specify and ensure the entire quality requirements have been met; c. Reviewing the high criticality rating of equipment and packages before the placement of purchases orders; d. Ensuring that the type of certification required by QP is compliant with CONTRACT Specifications, Data Sheets and International Standards; e. Reviewing SUBCONTRACTORS documentation requirements lists and identifying those documents required for certification purpose;

4.5.5 Customs Clearance/Duties


1. The CONTRACTOR shall be responsible for the payment of all applicable Custom Duties for all material and equipment imported for the WORK to the State of Qatar in accordance with APPENDIX D. 2. The CONTRACTOR is fully responsible for arranging in land transportation, sea/air freight, voyage insurance, customs clearance (including payment of legalization fees) and transportation of all equipment/material to the WORKSITE. 3. CONTRACTOR shall ensure all Custom Clearance, legalization and other appropriate fees are paid with respect to Equipment, Materials, Spares, provision of fills or inventories for equipment including packages (such as oil, grease, chemicals and desiccants).

4.5.6 MESC (Material, Equipment, Standards and Codes) Coding Services N/A 4.5.7 Spare Parts
1. General a. The CONTRACTOR when issuing RFQs or tenders for SUBCONTRACTS, shall instruct the SUBCONTRACTORS to

APPENDIX A indicate the requirement of spare parts for pre-commissioning, commissioning, start-up, insurance and two years operational spare parts. The CONTRACTOR shall ensure that, while submitting their quotation, the SUBCONTRACTORS shall provide description of the spare part items, number of units recommended, ex-work unit price, including sea worthy packing. QP will not approve any purchase order to be placed unless accompanied with an itemized list with prices of recommended insurance spares and Two Years Operational Spares.
b. c. Selection of SUBCONTRACTORs by the CONTRACTOR shall take account of spare part support on short notice. All materials including spare parts shall be subjected to an appropriate traceability procedure so as to ensure conformance with specifications and easy retrieval of the documentation pertaining to each item in all circumstances. CONTRACTOR shall comply with all the requirements of the FEED specifications for general spares parts included in Appendix-F.

Pre-Commissioning Spare Parts The CONTRACTOR shall procure and supply pre-commissioning spare parts, chemicals and lubricants, special tools and handling gear for each item of equipment as necessary for pre-commissioning of each item of equipment for the FACILITY. 3. Commissioning and Start Up Spare Parts The CONTRACTOR shall procure and supply commissioning and start-up spare parts, chemicals and lubricants, special tools and handling gear for each item of equipment as necessary up to COMPLETION. 4. Two Years Operational Spare Parts (Including Insurance Spares) QP may procure two years operational spare parts, through CONTRACTOR for which CONTRACTOR will be reimbursed in accordance with the provisions of Appendix B of the CONTRACT. A provisional amount of QR 500,000/-has been included in the CONTRACT PRICE in Appendix B towards procurement of two year operations spares. The CONTRACTOR shall be reimbursed on the actual quantity of spare parts ordered. The prices quoted by SUBCONTRACTORS are to be kept firm throughout the duration of CONTRACT and no escalation in this regard will be entertained by QP.

APPENDIX A CONTRACTOR shall provide to QP for review the filled in SPIR forms for 2 years operational spares immediately after the placement upon of Purchase Order to the SUBCONTRACTOR, Based on the instructions from QP, CONTRACTOR shall procure the quantity approved by QP. CONTRACTOR shall deliver the procured spares to QPs Maintenance / Material Department.

5. SPIR Forms a) The SPIR procedure and a sample SPIR form are enclosed in each specification in Appendix F. The CONTRACTOR shall obtain through the successful SUBCONTRACTORS a completed SPIR form, duly filled in within three (3) months of placement of purchase orders. Following review of the SPIR, QP may instruct CONTRACTOR to procure the Two Years Operational Spare Parts. It shall be noted that 2 years operational spare parts are required to maintain the FACILITY immediately after the CONTRACT COMPLETION DATE. Consequently, completion of SPIR form with all related drawings, etc shall be undertaken expeditiously by the CONTRACTOR. CONTRACTOR shall ensure that the SPIR Forms are duly completed with all information required. CONTRACTOR shall then forward the same to QP for final approval. CONTRACTOR shall resolve all shortfalls highlighted by QP.

4.5.8 Preferred SUBCONTRACTOR List 1. EXHIBIT 12 contains lists of SUBCONTRACTOR who are generally recognized by QP as bona fide suppliers of materials and equipment. This list is not intended to be exhaustive; inclusion does not signify any more than provisional recognition of a Vendor. Prior to placement of any purchase order, CONTRACTOR shall furnish full details of all proposed vendors and their products for QPs review and project approval. 2. Should CONTRACTOR wish to propose a SUBCONTRACTOR not listed in EXHIBIT 12, full qualification documents for the proposed vendor, including details of cost savings to QP, shall be submitted along with written justification for deviation from the Preferred Vendor List for QP review. CONTRACTOR shall not proceed, in any respect, regarding the proposed equipment/material until QP has approved the deviation in writing.

APPENDIX A 4.6 SUB-CONTRACTING & OTHER GENERAL SERVICES 4.6.1 Sub-Contracting


1. CONTRACTOR shall establish with SUBCONTRACTORS approved by QP, all necessary SUBCONTRACTS to meet the requirements of this CONTRACT and in accordance with procedures agreed with QP. 2. Deleted. 3. An unpriced form of Preliminary Asset Capitalization Schedule (PACS), enclosed as EXHIBIT 18 to this Appendix A shows the minimum level of asset classification details required by QP.
2. CONTRACTOR acknowledges and accepts: a) He has fully understood the level of classification of assets required by QP, the information and supporting documents that it shall be required to provide throughout the term of the CONTRACT for finalizing the ACS. b) He has examined and understood the PACS, endorsed and enclosed the unpriced PACS in Appendix E and the priced PACS in Appendix B. CONTRACTOR has ensured that the total of the costs allocated/appropriated to all the assets reconciles with the CONTRACT PRICE. 3 During the term of the CONTRACT, using the PACS as a basis, CONTRACTOR shall develop the ACS by updating the same on regular basis on the actual QP assets generated during the performance of the CONTRACT, and on the actual costs to be allocated/appropriated to the assets, including collation of adequate and appropriate documents, and additional related information as required by QP. The ACS shall be deemed as final when QP REPRESENTATIVE is satisfied that the cost allocated to the assets truly reflects the actual costs of the assets, and the total of the costs allocated to the assets reconciles with the CONTRACT PRICE.

HEALTH, SAFETY AND ENVIRONMENTAL 4.7.1 General 1. CONTRACTOR shall construct the WORK in accordance with QPPHL-S-001, related industrial city safety regulations and State of Qatar HSE requirements. 2. Maximum safety shall be the target during installation, commissioning, start-up and operation of the FACILITY. CONTRACTOR shall comply with the requirements of QP Safety Regulations for Contractors. CONTRACTOR shall provide complete details of their HSE organization chart at corporate level and project level clearly identifying the personnel. This shall be submitted immediately after the award of CONTRACT. 4. CONTRACTOR and key SUBCONTRACTOR shall have clearly defined HSE policy at corporate level. 5. CONTRACTOR as minimum shall have project HSE manager and safety officer and QP reserve the right to ask CONTRACTOR to employ more safety personnel based on the project magnitude. 6. CONTRACTORs key safety personnel shall have minimum 5 years experience in construction safety and should have worked in at least one similar project. Key safety personnel should preferably have undergone safety courses such as NEBOSH. 4.7.2 HSE Manual Prior to commencement of any field activity, CONTRACTOR shall prepare a project specific HSE manual and submit to QP for review and approval. While developing the manual, CONTRACTOR shall liaise with Fire and Safety Department of the area of construction and obtain necessary site-specific safety requirements. It shall as a minimum contain the following: 1. Policy and statement of commitment, 2. Corporate organization as well as project specific organization chart 3. Definition of roles and responsibilities of HSE personnel as well as connected management personnel 4. Interfacing with QP HSE personnel and line management 5. Procedures for reporting, training/induction, PTW, 6. Competency level and CVs of all project HSE personnel 7. Procedure for SUB-CONTRACTORS and their personnel 8. APPENDIX A 4.7.3 Training 1. CONTRACTOR shall ensure that all labour, directly or indirectly employed by them have undergone necessary safety training as required by QP procedures as well as statutory requirements and shall provide records to substantiate the same. 2. BA/H2S course conducted by QP is mandatory for all personnel who are required in the field. CONTRACTOR shall give minimum two-week advance notice to QP with all relevant details of their personnel who are required to undergo the training. QP based on the overall training schedule will advise CONTRACTOR on timing. 4.7.4 Safety Meetings 1. CONTRACTOR shall arrange weekly toolbox safety meeting wherein job specific hazardous activities related to WORK either scheduled or in progress shall be discussed and necessary measures for implementation shall be identified. 2. Independent of weekly toolbox meeting, before commencement of any new WORK at WORKSITE, tool box meeting shall be conducted 3. CONTRACTOR shall record all toolbox meetings and advise QP, in writing, of the safety topics discussed and numbers of attendees. 4. Safety Engineers/Officers, supervisors, new personnel who are being inducted for the specific work and where necessary site managers shall participate in the toolbox meetings. 5. CONTRACTOR shall inform the safety statistics to their employees periodically and shall notify QP in their weekly progress report. 6. Displaying the safety statistics on the site in English and Arabic shall be considered and QP may direct the CONTRACTOR do the same if considered necessary. 7. CONTRACTOR shall conduct senior level monthly Safety Management meetings and minutes of the meetings shall be submitted to QP for information.

CONTRACTORS premises including their messing facilities, food etc shall be inspected, audited for safety and hygiene by a competent doctor and safety inspectors on a monthly basis and report submitted to QP. When necessary QP will also participate in such audits. CONTRACTOR shall implement any recommendations arising out of such inspections. 4.7.7 Permit to Work System (PTW) CONTRACTOR shall comply with QP Work Permit Procedure as detailed in Safety Regulations for CONTRACTOR. CONTRACTOR shall adopt and apply the PTW system of QP in all QP restricted and unrestricted areas. An allowance for a period of up to seventy two (72) hours from the date of receipt by QP of the Permit to Work application shall be considered for the issue of work permits.

APPENDIX A CONTRACTOR shall ensure that a WORK execution plan, method statement and approvals of third party like Ministry of Environment (MoE), municipality, other ministry approvals etc shall be attached along with the permit. QP Construction and Safety team shall approve the PTW before submitting it to operational authority. Prior to crossing any third party installation such as cables, pipes, high-tension overhead power lines, etc., permission shall be obtained from the necessary authorities and notification given to them at least two weeks in advance. Wherever road deviations are required, all end users to be informed two weeks in advance. Police permission and local authority permissions shall be taken for approval of detour roads and attached with the PTW.

4.7.8 Accident Reporting 1. CONTRACTOR shall immediately report to QP any accident or near miss. The actual reporting procedure shall accord with section No. 17 of the CONTRACTORS Safety Regulations Rev.01 1992. CONTRACTOR shall conduct a detailed investigation and identify any and all hazards that arise of any incident and submit to QP the intended corrective actions proposed. Based on frequency of incidents, CONTRACTOR performance will be subject to review and where necessary WORK shall stopped until corrective measures are in place.

4.7.9 Safety in Commissioning Pre-Start up safety review shall be conducted at the time of mechanical completion of the FACILITY. 4.7.11 Safety Personnel CONTRACTOR shall identify and nominate adequate safety officers for each area where WORK would be carried out. QP reserves the right to instruct CONTRACTOR to employ more safety personnel if deemed necessary. 4.7.12 Environmental Compliance 1. General

CONTRACTOR shall ensure compliance to state environmental legislations, QP procedures and area specific environmental standards during the CONTRACT. CONTRACTOR shall liase through QP environmental departments with Ministry of Environment (MoE) for necessary clearances and approvals. CONTRACTOR shall also assist QP where required to get the Consent to Operate permit from Ministry of Environment (MoE). 2. Radioactive Material

All parties engaged in activities involving the use of radioactive sources/isotopes shall be licensed in accordance with Qatar Law No. 31 of 2002 concerning protection from radiation. CONTRACTOR shall familiarize himself with the provisions of the subject law and is responsible for obtaining the necessary licenses prescribed by law before commencement of the WORK. CONTRACTOR shall ensure that such licenses are maintained throughout the duration of the CONTRACT. CONTRACTOR shall at all times comply with the requirements of the law. APPENDIX A elsewhere in the CONTRACT, CONTRACTOR shall release, protect, indemnify, defend and hold harmless QP and its AFFILIATES, and its and their respective officers, employees and agents from all claims, liabilities and legal action of every kind and nature resulting from CONTRACTORS failure to comply with the obligations set forth in this clause. 3. Environmental Monitoring a. CONTRACTOR shall install appropriate on line monitors where necessary to demonstrate compliance with State and QP environmental standards. b. CONTRACTOR shall devise a monitoring program designed to monitor environmental effects predicted in the environmental impact assessment. CONTRACTOR shall supply all appropriate instrumentation to carry out this program. CONTRACTOR shall indicate manpower requirements to carry out the program. CONTRACTOR shall indicate any other support facilities (such as laboratories), which may be required to carry out this program. c. CONTRACTOR shall supply a suitable database system for archiving environmental monitoring data. Archiving shall be such that compliance with State regulations may be demonstrated when requested by the State authorities, and also such that management may have computer network access to the environmental history of the WORK. 4. CONTRACTOR shall initiate an EIA and generate a report incorporating emission data obtained from all vendor

APPENDIX A equipment. CONTRACTOR shall be responsible for compliance with the local Environmental Control Regulation and CONTRACT specifications. In general, the FACILITY shall be designed to meet the ambient air quality and water quality guidelines of State of Qatar, MMAA environmental regulations and CONTRACT Specifications. b. A list of air emission sources including stationary emission sources for SOx, NOx, CO, Ozone Depleting Sources and Total Suspended Particles shall be produced. c. CONTRACTOR shall ensure that all materials used in any hot and cold insulation system meet the requirements of the specifications. Test reports and third party certification specified in the specifications shall be made available to QP. d. During preparation of the FACILITY operating procedures, CONTRACTOR shall ensure compliance with the environmental requirements of the State of Qatar. e. The release of hydrocarbon products and their by products into the atmosphere or environment generally, shall be restricted by the procedures. CONTRACTOR shall ensure that the operating procedures emphasize the requirement to carefully handle all chemicals, such that spillage and operator contamination is avoided. In the event of any spillage, the procedures shall state the action to be taken to restore the site to its original uncontaminated condition. SHOC cards or posters shall be exhibited at locations of usage and shall be addressed in the safety plan. 6. Noise Control Measures CONTRACTOR shall comply with the following criteria with respect to noise control, and shall ensure that the new FACILITY and equipment conform to the requirements: a. Noise control shall be considered as an essential feature in the design of the FACILITY and equipment. The sound pressure level shall not exceed 90 dBA (85 dBA maximum design basis over an 8 hour working day) as a general rule in WORK areas and 75 dBA at the inner edge of perimeter infrastructure roads.

CONTRACTOR shall be responsible for identifying any necessary reduction of equipment sound level limits specified in QP specification, to ensure compliance with all sound level provisions.

4.8

ONSHORE FABRICATION AND CONSTRUCTION 4.8.1 General

1. The CONTRACTOR shall undertake fabrication and construction of the FACILITY that maximizes onshore WORK and minimizes offshore WORK, as well as all ancillary and temporary items that are necessary for the expeditious COMPLETION of the WORK. The FACILITY for PS-2Q and PS-3Q; all structural framings and support structures shall be fabricated onshore in accordance with APIRP2A requirements and shall be transferred for offshore installation. All structures are designed for single lift installation.

Construction shall be understood as including all pre-commissioning activities, i.e., all conformity verification and static test necessary to ensure the FACILITY is in conformance with the detailed design documents, drawings, mechanically complete and ready for commissioning. All details of the WORK shall comply with Approved For Construction drawings and specifications and all applicable codes and standards acceptable to QP.
3. The CONTRACTOR shall ensure that the FACILITY is mechanically completed as far as possible onshore and all punch list activities shall be cleared wherever possible onshore. 4. Wherever possible the CONTRACTOR shall ensure all precommissioning WORK is carried out onshore, but the final FACILITY pre-commissioning and commissioning shall be carried out offshore upon completion of the hook-up of the new FACILITY to the existing PS-2Q & PS-3Q facilities. 5. Fabrication of all pipework shall conform to the requirements of QP Corporate Standards indicated in various specifications in Appendix F.

4.8.2

General Construction Requirements

1. CONTRACTORS scope of construction WORK shall include, but not be limited to, the following: a. Obtain all licenses, approvals, visas, passes and permits including QPs Permit to Work System, that are required and necessary to execute the WORK at the WORKSITE. Undertake pre and post-modification surveys of the PS-2Q & PS-3Q living quarters complex and existing wellhead platforms locations. Establishing the WORKSITE and WORKSITE safety rules, and providing a safety manager at the start of the WORK to implement the same. Providing construction and supervisory personnel. Preparing construction procedures for the WORKSITE, which shall be submitted to QP for approval, and implementing the same. Managing, coordinating and supervising the WORK, including of the SUBCONTRACTORS, to ensure the WORK is performed to the required schedule, quality and Health, Safety and Environmental requirements. Preparing detailed schedules for the WORK in accordance with QP requirements, regular monitoring of this schedule and

Providing manpower and construction facilities loading charts. Providing schedule and progress reporting in accordance with QP requirements. Providing a Quality Assurance Plan and developing quality control procedures and implementing the same. Providing all construction equipment required for the WORK. Providing all temporary items and temporary facilities, including transportation, sanitary facilities, office supplies, office equipment and other such items as may be required for the construction, transportation, installation, hook-up and onshore/offshore commissioning of the FACILITY. Maintaining construction records in formats approved by QP. Arranging, if necessary, training of labour to perform the WORK in accordance with applicable codes, supervision of trade qualification tests, and maintenance of test records. Ensuring timely mobilisation of SUBCONTRACTORS representatives and/or any specialists, and their retention at the WORKSITE for the required duration. Ensuring that the FACILITY is set out and constructed in accordance with Approved For Construction drawings and specifications. Preparing, submitting for approval and carrying out pre-commissioning checks, in accordance with the approved procedures. In particular, the format of all WORKSITE reports shall be submitted for approval. Maintaining and operating an adequate system of control of availability of drawings and specifications at their latest revision at the WORKSITE. Managing field changes and revision of drawings. Maintaining Weight Control Reports. Preparing rigging studies as required. Preparing transportation studies as required. Preparing load and internal sea fastening studies as required.

2. The CONTRACTOR shall be responsible for evaluating all personnel proposed for the project to ensure they are capable of safely performing the WORK assigned. All costs associated with this shall be the responsibility of the CONTRACTOR. 3. The CONTRACTOR shall provide, operate and maintain all the WORKSITE facilities throughout the duration of the WORK.

APPENDIX A 4. Welder and NDT qualification and performance shall be in accordance with the requirements as detailed in EXHIBIT 7 of this Appendix A. This WORK unit includes all activities associated with welding and NDT qualification for onshore and offshore WORK for structural and piping WORK according to specifications in Appendix F.
5. The CONTRACTOR shall execute all activities for the fabrication, prefabrication and erection of the FACILITY and its systems and equipment, in accordance with specifications and other documentation included in Appendix F and all relevant engineering specifications and drawings, updated/prepared by the CONTRACTOR during detailed engineering phase as Approved for Construction Status.

4.8.3 Process

1. The CONTRACTOR shall supply all the materials necessary for the complete piping. These materials shall be procured and/or fabricated to the specifications reviewed and amended by the CONTRACTOR and agreed by QP. These materials shall be procured only from QPs approved SUBCONTRACTOR List. 2. CONTRACTOR shall ensure that the suppliers of the materials provide a Manufacturing Procedure Specification (MPS), Quality Plan and Inspection and Test Plan (ITP), which ensure that all material supplied and fabricated for this CONTRACT meets the requirements of the QP approved specifications.

MECHANICAL COMPLETION
1. CONTRACTOR shall prepare a detail Mechanical Completion Procedure with all necessary forms and acceptance Certificates for QP approval. 2. The CONTRACTOR shall bring the FACILITY to mechanical completion. Mechanical completion is a multi-disciplinary over-all check (refer to EXHIBIT 2 to this Appendix A. It shall be done with the objective of confirming that: a. Equipment/package units supplied are erected and completed as per AFC drawings and documentation. b. The installation of the FACILITY is complete in accordance with AFC drawings and SUBCONTRACTORS recommendations; c. Adequate access is available to all items of the equipment for operation and maintenance 3. Most of the mechanical completion WORK will be done at the CONTRACTOR onshore facility, where a thorough check shall be carried out. Detection of errors/omissions and access problems at the onshore facility will provide time for the CONTRACTOR to initiate a corrective action before offshore installation and hook-up. 4. The state of mechanical completion for a system/sub-system presumes that all the WORK, including the necessary checking/inspection as per the

APPENDIX A AFC drawings/specifications and the applicable codes/practices, has been performed during the construction period.
5. CONTRACTOR shall ensure that all activities leading to Mechanical Completion are properly documented, verified and inspected. The Mechanical Completion Dossier shall be prepared by the CONTRACTOR to document this stage of Completion and shall include all test records (Inspection Test Records) developed by the CONTRACTOR throughout the CONTRACT and have been verified and agreed with QP for all disciplines and activities. 6. Please refer to EXHIBIT 2 for more details with respect to the Mechanical Completion of the FACILITY. 7. Generate punch lists and liquidate action points and present them to QP 8. CONTRACTOR, in consultation and approval with QP, shall classify the punch lists as Category-A and Category-B. Category A shall be defined as those punch lists points without completion of which the next activity of pre-commissioning and commissioning cannot be undertaken safely. This category shall also include such points that cannot be attended to later when the system is charged with operating fluid. All other points shall be classified as Category-B. QPs decision will be final with regards to such classification of punch list points. 9. CONTRACTOR shall be obliged to attend to all the punch list points to the satisfaction of QP. Category-A points shall be completed as a priority. Only upon completion of Category-A points shall pre-commissioning activities commence.

4.10

ONSHORE PRE-COMMISSIONING
a) The CONTRACTOR shall carry out all onshore pre-commissioning activities in accordance with specifications included in Appendix F and EXHIBIT 3 and detailed procedures and plans to be produced by the CONTRACTOR and approved by QP. b) The CONTRACTOR shall pre-commission all equipment, as far as practically possible, at the onshore WORKSITE in accordance with all necessary procedures and plans approved by QP. c) The CONTRACTOR shall ensure that construction activities are completed, mechanical completion is achieved and all punch list items are rectified before pre-commissioning is started. APPENDIX A testing of equipment shall include all the associated process, ancillary, safety, equipment protection and instrumentation control facilities as a complete system, to the maximum practical extent possible.
d) The CONTRACTOR shall issue the necessary documentation to certify that each part of the FACILITY has been satisfactorily constructed, is mechanically complete and punch list items has been rectified and has been pre-commissioned e) Pre-commissioning shall include, but is not limited to: 1) Systematic conformity checks, carried out on each part or item of equipment or component, such as pressure gauges, motors, cables, to visually verify the condition of the equipment, the quality of installation, the compliance with drawings and specifications, manufacturers instructions, safety rules, codes, standards and good practice. 2) Static, de-energized test of specific equipment, to ensure the quality of a number of critical components. This cold testing WORK will concern all disciplines, and cover activities such as, calibration of instruments, machinery alignments, setting of safety valves, pressure testing of piping and testing of cable continuity 3) Piping and vessels flushing, cleaning and chemical cleaning if required. 4) Pre-commissioning shall be categorized by system and sub-systems. 5) Pre-commissioning preparation shall start at the engineering stage, together with commissioning and start-up preparation. It shall include the preparation of the documentation, including breakdown of the FACILITY in systems and sub-systems in line with the start-up sequence for the FACILITY. 6) The CONTRACTOR shall identify at the engineering stage all systems, and parts thereof that are to be fully or partially pre-commissioned during the onshore construction phase of the WORK. f) The CONTRACTOR responsibilities for pre-commissioning shall include but is not limited to the following: 1) Obtaining the necessary SUBCONTRACTORS documentation; 2) Preparing pre-commissioning plans and procedures as required to achieve COMPLETION of the WORK; 3) Providing a team of suitable experienced and qualified personnel including SUB-CONTRACTOR Engineers/ Supervisors for precommissioning; 4) Providing all necessary pre-commissioning equipment, tools, spare parts, all consumable items, instruments and fluids such as, inhibited water, chemicals, temporary power, compressed air, nitrogen, lube oil; portable water, and diesel.

APPENDIX A 5) Undertake all necessary scheduling, planning and progress reporting required by QP in accordance with the CONTRACT. 6) Coordinating activities of SUBCONTRACTORS participating in the WORK; 7) Conducting WORK planning meetings as required; 8) Developing all required software items; 9) Producing and Maintaining all necessary documentation, certification and records as per CONTRACT.
g) Review of all in-line equipment, valves, instrumentation and auxiliary equipment to ensure that they are capable of withstanding the hydrostatic test pressure. In cases where items are found unsuitable, the CONTRACTOR shall either completely remove such equipment, instruments and valves prior to tests and replace them with spools, or isolate the valves with blind flanges. CONTRACTOR should clearly indicate in the pre-commissioning procedures the selected method to be adopted for each piece of equipment. h) The CONTRACTOR shall ensure that in developing all pre-commissioning procedures and when undertaking WORK that the following documents are provided: 1. Pre-Commissioning Check Lists per system/ sub-system/ Equipment. i) Pre-commissioning Check Lists shall define the scope of WORK that must be systematically performed on each item of equipment/system and shall also be used as a log to record the checks made. ii) The conformity checks defined above shall be carried out in accordance with specifications and reported on Precommissioning Check Lists. iii) One Pre-commissioning Check list shall be filled in for each sub-system and type of equipment or package. Pre-Commissioning Test Sheets i) Pre-commissioning Test Sheets shall be produced and shall identify, individually, each and every test to be undertaken in accordance with the CONTRACT specifications and shall detail the test results. Pre-Commissioning Status Reports i) Pre-commissioning Status Reports shall be provided by the CONTRACTOR in accordance with the requirements of the CONTRACT and shall be an exhaustive list arranged by system, discipline and type of equipment,

APPENDIX A Pre-commissioning Status Reports shall provide a comprehensive and detailed status of all pre-commissioning activities by system, discipline and equipment type, indicating checks and tests to be made.

Pre-commissioning Punch Lists i) Pre-commissioning punch lists shall be established by CONTRACTOR and QP for each sub-system being precommissioned, for the purpose of recording all discrepancies, damaged or missing equipment, malfunctions, missing items and any deviation from the design drawings and specifications. ii) The CONTRACTOR shall clear the punch lists prior to applying for the Ready for Commissioning Certificate for each subsystem. Pre-commissioning Dossiers CONTRACTOR shall compile documentation pertaining to the precommissioning activities in a dossier arranged by sub-system and by discipline, which shall contain all information required to demonstrate a sub-system has reached the Ready for Commissioning status. The pre-commissioning dossiers shall include the following: i) Ready for Commissioning Certificate signed by both CONTRACTORS construction and Commissioning Teams and QP authorized Personnel; ii) Signed off Punch lists status; iii) Marked up drawings; iv) Specific procedures used for pre-commissioning; v) Pre-commissioning Status Index; vi) Pre-commissioning Check Lists; vii) Pre-commissioning Test Sheets; viii) As constructed drawings; ix) SUBCONTRACTORS shop test reports. x) SUBCONTRACTORS documentation and site reports;

PRESERVATION AND PROTECTION OF INSTALLED EQUIPMENT AND SYSTEMS


a. Following the installation and pre-commissioning of the equipment and systems at the onshore WORKSITE, the CONTRACTOR shall undertake all measures necessary to prevent deterioration and/or damage to equipment and systems at WORKSITE, transportation to the offshore WORKSITE and installation at the offshore WORKSITE with particular attention to lining and non-metallic pipework.

b. CONTRACTOR shall ensure all sensitive equipment, installed is preserved during the Transportation and Installation phases of the WORK. c. CONTRACTORS preservation and protection measures shall include but not be limited to: 1) Covering all instruments and equipment (preferably weather proof boxed covers); 2) Covering/anti-rust coating of all bolts and threads; 3) Openings, ports, vents plugged and/or covered; 4) Compliance with equipment SUBCONTRACTORS recommendations with regards to long-term preservations/storage; 5) Suitably protect all sensitive electrical, hydraulic, instrumentation equipment. 6) Fumigation of manned areas and rooms 7) Tie-in of stairs and ladders to prevent movement. d. The CONTRACTOR shall, where necessary, prepare detailed preservation procedures and submit them for QP approval. The procedures shall cover the reinstatement and re-testing of systems and equipment at the offshore WORKSITE.

4.12

WEIGHT AND CENTRE OF GRAVITY MONITORING, CONTROL AND WEIGHING OF THE FACILITY
a) The CONTRACTOR shall develop and maintain a Weight and Centre of Gravity (COG) Control Report (WCR) covering the FACILITY throughout the duration of the WORK. The WCR shall contain the actual weight and COG of each component and structure, and shall conform to the weight control procedure and specification. b) The CONTRACTOR shall utilize internationally recognized software developed for the weight and COG control and monitoring. The CONTRACTOR shall obtain QP approval with respect to the software prior to its implementation and use in the project. c) In order to determine the actual weight of the FACILITY, the CONTRACTOR shall weigh the FACILITY in accordance with the requirements of the specifications and/or weighing procedures, to be developed by CONTRACTOR for the approval of QP. d) CONTRACTOR within his price shall account for three weighings at appropriate time during the construction phase to be agreed with QP

e) The FACILITY actual final weight and centre of gravity shall meet the criteria of the specifications. f) The CONTRACTOR shall give advance notice of the date, time and place of the weighing activities in order to allow the attendance of QP representatives, the Marine Warranty Surveyor and the Project Third Party. g) At least six (6) weeks prior to weighing, the CONTRACTOR shall submit detailed calculations demonstrating the load transfer structure and the foundation have adequate load carrying capacity for the weighing operation. h) All weighing apparatus/instrumentation shall be calibrated and certified for the required purpose. i) j) Weighing by the transportation/load-out barge is not acceptable to QP. An Inspection Certificate shall be obtained from the Project Third Party for all structures or part of structures, as required.

LOADOUT, TRANSPORTATION, CONSTRUCTION 4.13.1

Introduction
Loadout, transportation and offshore installation shall include all necessary WORK, services and construction equipment required for transporting and installing the FACILITY, as well as all construction WORK, ancillary WORK, services and equipment that are necessary for the safe, reliable and expeditious COMPLETION of the WORK. Loadout, transportation and offshore installation shall comply with approved specifications, drawings, procedures, all applicable International Marine regulations, standards and all applicable Qatari codes and regulations. The CONTRACTOR shall submit for approval his procedures, which are proposed for the WORK. CONTRACTOR shall ensure all necessary pre and post installation surveys are carried out for the offshore installation WORK.

APPENDIX A The transportation and offshore installation scope of WORK to be undertaken by the CONTRACTOR shall include, but not be limited to the following:

Preparing safety procedures, submitting such procedures for approval by QP and providing a Safety Manager at start of the WORK to implement them; Preparation of a loadout, transportation and installation plans for each platform. Provide any installation aids, temporary struts, bracing or riggings during any phase of the installation; Provide an adequate number of seaworthy cargo and crane barges, which shall be stable and of sufficient size and structural strength to ensure that the stresses in the barge, cargo and sea fastening are within acceptable limits; Providing an adequate number of sea going tugs, with ample power and size to operate safely for each particular route; Providing experienced transportation and installation personnel; Preparation of all necessary Marine Installation Documentation (charts, Anchor Patterns etc) which shall be subject to approval by QP; Preparing transportation and installation procedures, which shall be submitted for approval by QP, and implementing them; Ensuring timely mobilisation of SUBCONTRACTORS and/or any specialist, and their retention for the required duration; Managing, co-ordinating and supervising the WORK of SUBCONTRACTORS to ensure WORK is performed to the required schedule, reliability, quality and safety requirements; Providing all construction equipment and materials required for the transportation and installation including, but not limited to, the following: i) Transportation marine spread and support, including transportation cargo barge with ballasting and/or jacking equipment, power packages, tow tugs and towing gear and any other support vessels and equipment as required. Ensuring that the FACILITY are transported and installed in accordance with international and/or Qatari marine navigation regulations, specifications and approved drawings, procedures and technical analysis; vi) Maintaining and operating an adequate system of control of availability of drawings and specifications in their latest revision at the WORKSITE and on the marine spreads; vii).Maintaining and operating adequate control procedures on the installation marine spreads; viii)Preparing remedial and contingency plans where transportation and/or installation problems arise and implementing them as necessary; ix) Performing barge inspection and survey prior to delivery to the load out site and confirming that the barges are suitable for load out, transportation and installation of the FACILITY; x). Undertaking dimensional surveys of the deck undersides and transition pieces, and confirming that the topsides can be installed within specified tolerances; xi) Ensuring that, whenever possible, all temporary items required to support the installation of the FACILITY are in place prior to load out of the FACILITY. xii) Performing the full scale FACILITY installation test at the WORKSITE after load out and prior to sail away, including ballasting, jacking, control including power generation system tests. xiii).Following such inspection and testing, the CONTRACTOR shall repair all defects or deficiencies observed to QPs full satisfaction and at his own cost and shall not proceed with the transportation to the WORKSITE prior to receipt of approval. 4.13.3 Specific Requirements for Transportation and Installation

Transportation and Installation Constraints

4.13.4

CONTRACTOR shall take into account when planning and executing the transportation and installation WORK that constraints will apply, these shall including, but not limited to, the following: 1. The CONTRACTOR shall prepare installation schemes and procedures that accommodate the operational constraints of working near the PS2 and PS3 complexes; Marine spreads, or parts thereof, may enter the WORKSITE only if the approach, positioning and mooring procedures are approved by QP. The CONTRACTOR shall take into account information regarding no go areas around the PS2 and PS3 complex and the wellhead platforms, which will be provided by QP. The CONTRACTORS planning, management and installation shall allow for complying with all regulations in force on WORKSITE including, but not limited to: periods of radio silence and, IP-OPS-012 Offshore Permit to Work IP-OPS-033 Offshore Mechanical Isolation Strategy IP-OPS-034 Scaffolding For Offshore

The CONTRACTOR shall develop his offshore schedule for his WORK in co-ordination with QPs Project Team and QPs Operation, Drilling, Marine and Safety Departments to ensure all total/partial platform or Wellhead Platform shutdowns are established to allow installation to proceed. 5. The CONTRACTOR shall develop emergency procedures covering all periods of installation at the PS2 & PS-3 complex, ; 6. The CONTRACTOR shall take into account the installation WORK will take place between two existing flares, which may be operational at the time of installation. The CONTRACTOR shall provide any necessary shielding for personnel and sensitive components of the FACILITY; 7. The CONTRACTOR shall bear the responsibility of obtaining directly, or through agents, the required visas, clearances, passes, documentation and/or fees for personnel and construction equipment assigned to WORKSITE and/or any location where WORK is performed 8. The transportation and installation marine spreads, cranes, tools and shackle and wires shall have valid certification. 9. Marine Spread, Vessels and onboard equipment (telecom and safety) are subject to QP approval prior to sail away. 10. CONTRACTOR shall submit his marine spread movement schedule within the field, anchor patterns and installation procedures for QP approval. 11. CONTRACTOR to license and obtain clearance and permits from QP Operations when interfacing with existing facilities. 12. CONTRACTOR to realize that certain WORK may necessitate partial or total PS-3 facility shutdown. CONTRACTOR to liase promptly with QP Operations, via QP Project Team, for obtaining such request for shutdowns.

4.13.5

Management, Supervision and Support Services

The CONTRACTOR shall provide all management, supervision and support services throughout the duration of the offshore WORK.

APPENDIX A CONTRACTOR shall undertake the onshore and offshore installation WORK by preparing, providing, operating and maintaining all facilities and services required to support the WORK throughout its duration. 4.13.7 Internal Sea Fastening

The CONTRACTOR shall submit his internal sea fastening design and procedures for approval by QP. 1. The internal sea fastenings shall be designed so as to prevent movement, damage, stress or strain to the FACILITY structures and installed systems and equipment during load out, sea transportation from the load out jetty to the offshore WORKSITE, and during the offshore installation of the FACILITY. The sea fastenings shall not affect the integrity of the permanent FACILITY. Internal sea fastening procedures shall include dismantling of equipment for transportation, installation and preparation for sail away. CONTRACTOR shall liaise with SUBCONTRACTORS for such equipment needing special precautions during transportation. 2. The CONTRACTOR shall fabricate the internal sea fastenings and install them prior to load out of the FACILITY onto the transportation barge. 3. Internal sea fastening shall be designed so that cutting away offshore is easy and fast. 4. Internal sea fastening shall include steel reinforcements and supports, timber supports, slings, lashings, packing, crating and tying down of portable or loose equipment and materials. 5 .Internal sea fastening shall be applied topsides, structures, pipes, cable trays, vessels, equipment, materials, etc. Internal sea fastening shall also be applied to the internal parts of equipment and vessels, as applicable. 6. Testing of sea fastening shall be limited to NDT of welds as per approved procedures. 7. Items and equipment, or parts thereof, that are particularly vulnerable to movement, damage, stress or strain due to their type or location shall be dismantled and packed/crated for transportation. Voyage protection shall be given to all surfaces susceptible to corrosion due to exposure.

APPENDIX A 8. Following the offshore installation of the FACILITY, the CONTRACTOR shall dismantle the sea fastenings and equipment, except as otherwise directed by QP, and remove them from the WORKSITE. 4.13.8 External Sea Fastening

1. The CONTRACTOR shall survey all points of interface between the FACILITY and the transportation barge and shall design, fabricate and install all necessary external sea fastenings. 2. The CONTRACTOR shall ensure that all sea fastenings between the FACILITY and the transportation barge shall be compatible with the particular structural arrangements required for offshore installation of the topsides onto the jacket. 3. CONTRACTOR shall ensure sea worthiness of the entire FACILITY during transportation.

4.13.9

Load-out

The CONTRACTOR shall undertake the Load out which shall be carried out in accordance with specifications and shall include, but not be limited to, the following activities: 1. Provision of all necessary load out documents including procedures, which shall be submitted to QP for approval; 2. Placement of any part of the FACILITY required to be loaded out to the appropriate load out area within the CONTRACTORS facilities including undertaking all necessary reinforcements and securing WORK (including the internal sea fastening of equipment within the FACILITY and voyage protection); 3. Provision and installation of all materials and works for ballasting operations, control systems, winches, guides, bumpers, positioning systems, etc.; 4. Load out onto transportation barges including provision, installation and removal of all temporary works, materials and equipment necessary for load out operations, including barge/quay bridge, skidding or trailer assemblies on the FACILITY, load out quay and on the transportation system; 5. CONTRACTOR shall be responsible for the compatibility between the various skidding or trailer assemblies of construction yard, quay / barge bridge and barge;

APPENDIX A 6. CONTRACTORS load out procedure shall include an emergency retrieval provision, to return the FACILITY to the load out quay; 7. CONTRACTOR shall survey all points of interface between the FACILITY and the transportation barge; 8. Deleted; 9. CONTRACTOR shall issue advance notification of the load out operation to allow attendance by QPs representatives and the Marine Warranty Surveyor. 10. CONTRACTOR shall carry out all requirements listed in Exhibit 15. 4.13.10 Transportation

1. Condition of Transportation Barge The CONTRACTOR shall undertake a full inspection of the transportation barge before installing barge grillage, external and internal barge reinforcement, ballasting and/or jacking equipment for installation and any other equipment, thereby ensuring the barge is ready to receive the structures . 2. Pre Load out Survey Prior to load out of the FACILITY onto the transportation barge, the CONTRACTOR shall have conducted a survey of the FACILITY, all installation aids, temporary items and any associated structures to ensure the suitability of all such items for load out, transportation and installation. 3. Mobilization of Transportation Marine Spread CONTRACTOR undertakes all necessary mobilisation work, which shall include but not be limited to the following activities: a) The provision, preparation and supply of the transportation marine spread complete with all associated equipment and manpower to the onshore WORKSITE quayside ready for load out; The provision and submission to QP of all necessary documentation required under the CONTRACT and marine regulations for the transportation marine spread and its operations, including all vessels, barge and craft details, specifications, drawings, current certifications, insurance and any necessary independent surveys;

4. Transportation to Offshore Worksite CONTRACTOR shall transport the FACILITIES and all associated loose pieces, prefabricated items, equipment, tools, etc, to the Offshore WORKSITE. In doing so, the work shall include, but not be limited to, the following activities: a) Inspection of external sea fastenings and FACILITIES on the transportation barges to ensure the FACILITIES have been secures correctly for offshore transportation; Preparation and issue of manifests and insurance documents; Obtaining approval for commencement of transportation; Movement of tugs, transportation barges, supply boats that make up the transportation marine spread for transportation of FACILITIES between onshore and offshore WORKSITES, including all necessary mooring and stand-by up to the complete off loading of the FACILITIES and disposal by CONTRACTOR of temporary items, including load out aids, external sea fastenings, installation aids, etc;

4.14

HOOKUP 4.14.1 Introduction

1. The CONTRACTOR shall undertake necessary hook-up WORK at PS2Q and PS-3Q tie-ins. This includes ancillary and temporary WORK that are necessary for the expeditious completion of the WORK. 2. All details of the WORK shall be in accordance with drawings specifications, codes and regulations given in Appendix F and developed by the CONTRACTOR to detailed design Level (AFC) stage under this CONTRACT and shall be subject to approval by QP. 3. CONTRACTOR shall provide all hook-up management, supervision, technical support, labour, transportation, equipment, Material, offshore marine spread and support, marine spread, consumables, temporary facilities and hook-up services at the WORKSITE in accordance with the requirements of the CONTRACT.
14.14.2

General Hookup Requirements

Hook-up preparation shall start at the engineering stage and shall include Field Site Surveys and preparation for approval of plans, schedules, detailed procedures and AFC level drawings. CONTRACTOR shall identify and execute: Number and duration of the mandatory Field Site Surveys required in order to design the Tie-ins on the PS2Q and PS3Q. Direct hook-up WORK, i.e. hook-up not involving interface parties; Interface hook-up WORK. The CONTRACTORS hook-up scope of WORK shall, as a minimum, include: 1. Design, site survey and AFC level drawings.

Managing, coordinating and supervising the WORK of all SUBCONTRACTORS to ensure WORK is performed to the required schedule, quality and safety requirements; 7. Maintaining hook-up records in formats to be submitted for approval; 14. Ensuring that the FACILITY is hooked up in accordance with AFC drawings and specifications, and in accordance with the approved procedures. In particular, the format of all WORKSITE reports shall be submitted for approval; 15. Maintaining and operating an adequate system of control of drawings and specifications at their latest revision at the WORKSITE; 16. Maintaining and operating an adequate material control procedure at the WORKSITE; 17. Preparing remedial plans where late material/equipment delivery and/or other hook-up problems arise; 18. Managing field changes and revision of drawings; 19. Preparing a complete set of as-built drawings; 20. Preparing rigging studies as required; 21. Preparing marine surveying studies as required; 22. Preparing transportation and riggings studies as required; 23. Carry out as-installed surveys at the WORKSITE as required; 24. Deleted. 25. NOTE : The hook-up disciplines and hook-up scope of WORK above are not intended to be exhaustive and are for guidance only. The CONTRACTOR shall be responsible for identifying all hook up disciplines, and identifying the total hook up scope of WORK.

4.14.3

Hookup Constraints

1. The CONTRACTOR must be aware that drilling activities may be ongoing on the adjacent wellhead platforms during the time of his hookup activities. 2. The CONTRACTORS planning and management of hookup activities shall include for complying with all regulations in force on the WORKSITE including, but not limited to: periods of radio silence and allow for working under controlled work permit system by QP Operations Department and IP-OPS-012 Offshore Permit to Work IP-OPS-033 Offshore Mechanical Isolation Strategy IP-OPS-034 Scaffolding For Offshore IP-OPS-035 Confined Space Entry for Offshore IP-OPS-036 Offshore Excavation Procedure IP-OPS-037 Isolation, Lock Off and Tag IP-OPS-038 Procedure for Handling Permitted Electrical WORK at Offshore Sites. 3. Working in the vicinity of existing flares. 4. Rough weather and high seas may be reasons for down time.

CONTRACTOR shall provide all WORKSITE management/supervision throughout the duration of the WORK, including: Hook-up management; External interface management; Hook-up supervision; Safety management; Field engineering and technical control services; Materials control; SUBCONTRACTS management as well as management of SUBCONTRACTORS representatives; Welding Inspection and NDT; Certification Services; Procurement; Removal/disposal of surplus materials; Dismantling of construction/hook-up aids and temporary items, except as otherwise directed by QP.

4.14.5

Removal of Equipment from Preservation

1. CONTRACTOR shall re-instate all systems and equipment put into a state of preservation at the WORKSITE. 2. The CONTRACTOR shall re-instate, clear, re-test and re-commission all systems and equipment as applicable in line with the procedures prepared by the CONTRACTOR and approved by QP.

4.14.6

Pre-Commissioning Offshore

1. CONTRACTOR shall carry out offshore pre-commissioning activities on accordance with specifications included in Appendix F, Exhibit 3 and detailed procedures and plans to be produced by CONTRACTOR and approved by QP. 2. CONTRACTOR shall pre-commission the systems, sub systems, and parts thereof that were not pre-commissioned, or were dismantled following pre-commissioning. 3. The CONTRACTOR shall carry out all offshore Pre-Commissioning WORK in accordance with specifications included in appendix-F,

4. Pre-commissioning activities, not involving interface parties, shall be carried out directly by CONTRACTOR. Pre-commissioning activities involving interface parties shall be carried out by the CONTRACTOR in accordance with interface documents to be developed by the CONTRACTOR and approved by QP. The WORK shall be carried out by the CONTRACTOR under the supervision of QP and interface parties involved. 5. COMMISSIONING 4.15.1

Introduction
CONTRACTOR shall prepare a detailed commissioning procedure including all necessary forms and acceptance certificates for QP approval. The CONTRACTOR shall maximize commissioning completion at the onshore WORKSITE and shall identify at the engineering stage all systems and equipment that are to be commissioned at the various WORKSITE. For those systems and equipment to be commissioned at the offshore WORKSITE, commissioning activities not involving interface parties shall be performed directly by CONTRACTOR. Commissioning activities involving interface parties shall be carried out by the CONTRACTOR in accordance with interface division of WORK to be developed by the CONTRACTOR and approved by QP. The WORK interface shall be carried out by the CONTRACTOR under the supervision of QP and interface parties involved. The CONTRACTOR shall demonstrate that each part of the FACILITY has been satisfactorily commissioned. The CONTRACTOR shall demonstrate that each SUBCONTRACTORS package has been satisfactorily tested in operation and that the SUBCONTRACTORS performance guarantees have been met.

APPENDIX A The CONTRACTOR shall commission the utilities systems, and have them operationally tested and completed in sufficient time to allow for the commissioning of the process and other systems.
The CONTRACTOR shall, when planning and performing commissioning activities, take into account the fact that, as soon as any utilities, process systems or drilling activities are under commissioning or in operation, the CONTRACTOR shall be required to enforce an approved safe system of WORK, which accommodates and interfaces with other parties safety requirements The CONTRACTOR shall comply with the requirements stipulated in the enclosed EXHIBIT-03.

Commissioning, by the CONTRACTOR will include: a) Dynamic verifications of each elementary electrical and instrument function; b) Actual run in and on line tests of utilities and major equipment; c) Preparation for gas in. In undertaking the commissioning activities the CONTRACTORS responsibilities shall include the following a) Supply enough quantities of Diesel to operate one or two of the generators to provide power as necessary for the Hookup, pre-commissioning, and commissioning activities: b) Supply of enough quantities of nitrogen to purge the sub-sea pipelines and topside facilities. c) Providing a team of suitable experienced and qualified personnel for commissioning. Pre-Commissioning and Commissioning Philosophy/ Strategy


The CONTRACTOR shall arrange for preparation of commissioning to be commenced at the engineering stage, in close liaison with QP. Commissioning documentation shall identify the systems and equipment that are to be completed onshore and the systems and equipment that are to be completed offshore. All commissioning preparation documentation shall be submitted for approval by QP. All such pre-commissioning and commissioning operations shall be organized by sub-system, following an agreed start up sequence. The preparation activities shall include the following: a) b) Define the FACILITY start up sequence; Organize the FACILITY into systems and sub-systems, which will form the basis for the organization of commissioning and start up activities and for the issue of the relevant completion certificates; Prepare and issue of marked up coloured PEFDs, single line diagrams and loop diagrams showing the limits of the subsystems; Compile equipment lists for each sub-system Prepare a pre-commissioning, commissioning and start up network plan, schedule;

APPENDIX A Prepare the detailed commissioning procedures, based upon major equipment manufacturers own commissioning procedures, including task registers and commissioning program Quantify the WORK and prepare manpower histograms; Collect all required engineering and manufacturers documentation; Carry out the engineering required for temporary commissioning facilities.

Commissioning Activities
Preliminary Checks a) Prior to commencement of commissioning activities, the CONTRACTORS commissioning team shall carry out preliminary checks to verify that each sub-system is actually ready for commissioning. This shall include a thorough review of the pre-commissioning dossier and a physical check of the FACILITY at the WORKSITE. The preliminary checks shall be carried out by discipline and shall be formalized on a specific report form. The major outcome of such preliminary checks is identification of a number of new punch list items to be added to the precommissioning punch lists.

Functional Tests a) Functional tests are the live tests to be carried out on each instrumentation, electrical, Mechanical Rotating and telecommunication system related to a given basic function. b) Functional tests are to be carried out on every loop. Random checks shall not be allowed. c) One functional test sheet form shall be prepared for every type of functional test.

Operational Tests a) Each sub-system shall be subject to an operational test. b) Each operational test shall be conducted in accordance with a specific operational test procedure to be established and approved at the commissioning preparation stage.

APPENDIX A c) Operational tests shall consist of bringing a given system into operation under conditions as close as possible to operating condition. d) Mechanical preparation of each piece equipment prior to start up shall be included in the operational test e) Wherever applicable, SUBCONTRACTORS performance guarantees shall be verified during such operational tests

Pre Start-up Activities Commissioning shall include specific pre start-up activities required to prepare for gas in, such as a) Leak tests at the operating pressure b) Drying out; c) Inserting, using nitrogen. d) Loading of chemicals e) Chemical cleaning f) Such activities shall be covered by reports and specific procedures, which shall be subject to approval 5. Technical Database a) During the commissioning preparation phase, CONTRACTOR shall list all basic functions, grouped by sub-system, discipline and type of function in a Technical Data Base, which shall constitute an exhaustive description of the physical content of each sub-system. b) The technical database shall indicate the type of commissioning operation that each basic function is subjected to, by reference to the task report form for such operation. c) The Technical Data Base shall be used as the main management tool for commissioning preparation and follow up. 6. Commissioning Punch Lists The punch lists established during pre-commissioning shall be continually updated during commissioning operations to reflect the addition of newly discovered items, as well as the deletion of cleared items. The CONTRACTOR shall clear all the punch lists items.

COMPLETION ACTIVITIES 4.16.1

Start-Up of the FACILITY


Upon completion of all commissioning activities of the FACILITY, the CONTRACTOR shall carry out the start-up to the entire FACILITY in accordance with procedures to be developed by CONTRACTOR and approved by QP. The start-up activities shall be planned in accordance with execution program and shall confirm the design intent of the FACILITY in terms of all design parameters and specifications. CONTRACTOR shall provide all personnel, equipment, tools, rigging, temporary facilities, consumables and any other utilities as required to undertake the FACILITY start-up. CONTRACTOR shall liaise with and mobilise any SUBCONTRACTORS in close co-ordination with QP to assure a timely successful start-up of the FACILITY.

Initial Operation and Maintenance


The CONTRACTOR shall mobilise all operating and maintenance personnel and SUBCONTRACTORS representatives as required for the operation and maintenance of the FACILITY on a full time basis up to the COMPLETION DATE. The operation and

APPENDIX A maintenance of the FACILITY shall be undertaken by CONTRACTOR in accordance with the operation manuals and SUBCONTRACTORS instructions.
2. The CONTRACTOR shall set-up a procedure and an management system for the operation and maintenance of all project equipment with a view to record and log all intervention and operating parameters related to each item of equipment. These procedures and systems shall be developed and coordinated with QP operations via the QP Project Team.

4.16.3

Performance Tests

The CONTRACTOR shall co-ordinate with QP and develop a Detailed Test Procedure to be Approved by QP for each of the following tests, to demonstrate that the FACILITY complies with all requirements of Specifications, Design Intent and meet all the Guarantee Points. 1. Design flexibility test (Demonstration Test) CONTRACTOR shall also demonstrate that the FACILITY is operating at a steady and stable condition complying with all requirements of the design parameters and the intent of design flexibility, any other tests specified elsewhere in Appendix-A or in the documents included in Appendix-F. CONTRACTOR to Produce list of these tests and agree the requirements with QP. 2. Performance Test a) Upon completion of the start-up of the FACILITY, the CONTRACTOR shall assure that the FACILITY is operating at a steady and stable condition at full design conditions before undertaking the test. b) When the CONTRACTOR considers the FACILITY is running smoothly in a steady & stable condition and ready for the performance test, the CONTRACTOR shall notify QP giving at least 48 hours notice that the test is to commence. Subject to confirmation by QP that the CONTRACTOR can proceed with the test with QP Personnel in attendance, the CONTRACTOR shall undertake the performance test in accordance with approved procedures, for the test period without any interruption. In case of any interruption during the test, the CONTRACTOR shall re-start and repeat the test, from the beginning.

APPENDIX A c) When the CONTRACTOR considers the test has been successfully completed the CONTRACTOR shall present all the results to QP. d) As soon as practical thereafter, QP shall confirm the results of the test by issuing the CONTRACTOR a notice certifying the successful completion of the test. 4.16.4

Project Completion / Handover


The CONTRACTOR shall complete all remaining WORK under the CONTRACT such as, punch list items clearance, Touch up paint, Site cleaning, site clearance, material reconciliation, final documentation, training and any other WORK as specified in the CONTRACT. The CONTRACTOR shall ensure that WORK is completed on or before the SCHEDULED COMPLETION DATE. Handover of the FACILITY to QP shall be in accordance with detailed Handover Procedure, which shall be developed by the CONTRACTOR and Approved by QP, incorporating all requirements of this CONTRACT. QP will provide a sample of formats to be used as Punch list Items, Mechanical Completions, Taking over Certificates, Acceptance Certificates etc, The CONTRACTOR shall proceed with the hand-over of the FACILITY to QP in a progressive and coordinated manner so as to allow a smooth HAND-OVER after all training for QP personnel has been conducted by CONTRACTOR. Handover of the FACILITY to QP shall take place after the successful completion of all the tests specified, and QP approval of the Acceptance Certificates. The CONTRACT will be completed when the CONTRACTOR has handed over all deliverables required by QP under the CONTRACT by the COMPLETION DATE.

Project Dossier CONTRACTOR shall prepare the Project Dossier, in accordance with requirements of EXHIBIT 10. Close Out Report CONTRACTOR shall produce close out Report in accordance with the requirements of EXHIBIT-11.
