Aither the #revious #assport ,umber or the file number may be mentioned here along ith date of issue and place of issue in the relevant boxes. $n case previously applied for passport but the same as not received*issued, then the details such as file number, date applied and place here applied should be furnished here. 1uppression of facts may attract penal provisions as per the section FG of the #assports Act, FEH+. COL2MN 33.a0: $f ever travelled on Amergency Bertificate (AB) or ever deported or repatriated to $ndia at :overnment cost, then the details of AB number, date and place of issue along ith original seizure memorandum, place and country from here deported or repatriated should be furnished in this column. Aven if the AB details are not available readily, at least the place and country from here deported or repatriated should invariably be given here. All such applicants should furnish details of circumstances of their repatriation*loss of passport in a form of notarized affidavit. COL2MN 36: @etails li(e educational !ualifications, visible distinguishing mar( and the height in centimeters are to be provided against the respective item. COL2MN 38: Regarding citizenship, hether it is by birth, descent, registration or naturalization is to be recorded in the box space provided by either )*@*R*, as the case may be. $f held any other citizenship before $ndian citizenship, then please furnish the previous citizenship in the blan( space provided. For employment purpose, such passport holders re!uire emigration clearance from the office of #rotector of Amigrants (#"As). "ther ise, they ill be stopped from travelling at the port of exit. Applicants are, therefore, advised to apply for AB,R, if they are eligible, to ensure hassle6free travel abroad. COL2MN 3:: $n this column, hile applying for the first time for the minor children ho are less than FD years of age, the details of valid passports held by BOTH OR EITHER parents should be furnished in the relevant column. $n such cases, passport to their minor child ill be issued ithout any police verification. Further, in the cases here the parents do not hold valid passports, applications for such minors can be made on the basis of three documents of parents details of hich are given in para B()) of S%c$#on IV of the #assport $nformation )oo(let along ith 1tandard affidavit at Ann%@ur% I. $n all such cases, passport to their minor child ill be issued on post6police verification basis. A declaration on plain paper as given at ann%@ur% H is needed to be filled up in each case. $n case the minor child ho is bet een F& and FD years of age ishes to 6obtain a full validity passport for FL years, the same can be issued only on submission of 1tandard Affidavit as in Ann%@ur% FIG and any three of the FK document as mentioned in para B ()) of S%c$#on IV of #assport $nformation )oo(let by the parents and on payment of fee e!uivalent to the normal passport fee i.e. Rs. FLLL*6 for a %H pages passport, as applicable for an adult. "ther ise the validity of the passport is restricted to five years or attaining the age of FD, hichever is earlier. $n case of single parent and children born out of edloc(

or in case of parents ho are ;udicially separated, a s orn affidavit before a 2agistrate, stating the facts of the case along ith documentary proof to be submitted as per Ann%@ur% FCG. $n the NORMAL CO2RSE the signature*consent of both parents is re!uired for issue of a passport to the minor .Ann%@ur% FHG0. 4o ever, if in case the applicant parent is not in a position to get the consent of the other parent, FOR 5HATEVER REASON, the parent applying for the passport of the minor may sign the form and submit a s orn affidavit as per Ann%@ur% FGG stating the facts and circumstances of the case along ith the application. The affidavit should also state that in case of a court case he*she ould be responsible and not the passport office. $n such cases, here only one parent is applying, the physical appearance of the child may be re!uested to ensure the applicantMs parent has the actual custody of the child. COL2MN 3;: The applicant should give correct information. 1uppression of any fact may lead to fine up to Rs.&LLL per offence and other penal provisions as applicable under the provisions of the #assports Act, FEH+ as amended from time to time. COL2MN 3<: $n this column, the name and address along ith 2obile or Telephone number*e6mail of person to be intimated in the event of death or accident is to be furnished. COL2MN 3=: This column is a self declaration made by the applicant about o ing allegiance to the sovereignty, unity and integrity of $ndia, not voluntarily ac!uiring the citizenship or travel document from any other country etc. Also this column contains declaration in furnishing true information in the application form and an applicant is a are that it is an offence under the #assport Act FEH+ for giving any rong information or suppression of any material information in getting the passport. N%! &a((&or$ means applying for a passport for the first time or applying for a passport if you have never held any passport earlier. $n case an application as ever made at any #assport "ffice prior to the present application and hether the application as processed or closed, or even if passport as issued and not received by the applicant, details thereof should be given in the relevant column in the present application. Failure to disclose these details, if any, ould invite applicable penalty. R%#((u% means an application for another passport in lieu of an existing one hich has either expired or is about to expire. An applicant may apply for a ne passport in lieu of an expired* about to expire passport upto % years after and F year before the expiry of the #assport. Re6issue of passport should not be mista(en for renewal of passport. Rene al of passport means that a passport hich as originally issued for a short validity of one to five years 6 under certain emergency conditions on the part of the applicant 6 is no re!uired to be extended to its full validity of ten years from the date of issue of the passport. (Renewal is a zero fee service and the application should be made in Form No. 2). A N%! passport is issued in lieu of a lost*damaged passport.
