CONTROL OF MECHANICAL LIFTING OPERATIONS (Refer Flowchart of Lifting Operation on Page 6) The Managers of Project/Operations is responsible for the identification of non-routine mechanical lifting requirements. i) Non Routine Lifts Lifts identified as requiring formal control arrangements will be agreed with the Client Representative and will be subject to an engineering and safety assessment by the Managers of Project/ Operations who will formulate a specific lifting plan. The lifting plan will be agreed between the Managers of Project/Operations, Client Representative and the Safety Officer / Safety Supervisor before adoption. The plan will include the name of the person responsible for the lift and the implementation of the plan.(Refer Lifting Plan on Page 7 & Pre-Lifting Operation Checklist on Page 8 ) ii) Routine Lifts Routine mechanical lifting, are those lifts within the capability of existing yard plant and able to be carried out by a single crane. Trained and certified crane operators shall be employed for the same. A rigger shall assist the operator to carry out the lifting safety.

Lifting Equipment Responsibilities with regard to the use and maintenance of lifting equipment shall be as follows: i) The Material Controller shall provide slings, shackles, wire ropes etc. with manufacturer safety certificates and all equipment shall be checked by the user each time prior to use. (Refer Lifting Equipment Record ) ii) The person in-charge shall arrange for all cranes to be inspected annually and for all lifting equipment to be re-certified by qualified P.E. The crane operator and lifting supervisor shall carry out/organise for all the necessary maintenance work. The operator shall carry out daily inspection of crane as per a given check list and sign on the checklist. This record shall be available in this file. .(Refer Crane Operator Checklist Format on Page ) iii) Supervisors and foremen shall be responsible for ensuring that all lifting equipment and accessories used by them is in good working order, carries an identification mark and the current colour code, and has adequate capacity i.e., Safe Working Load Limit (SWL) for the crane. (Refer Lifting Equipment Colour Code on Page ) The Lifting Supervisor and project/operations supervisors shall ensure that unless designed to do so, cranes are not used as load carriers to move loads. Supervisors, foremen and crane operators are responsible within their respective scope for ensuring that cranes are fully able to undertake each lift and that any faults, including faults to any safety equipment/apparatus fitted to the crane are reported to the Lifting Supervisor without delay. These same people are jointly responsible for ensuring that a faulty crane is removed from service immediately and that confirmation of repair to full serviceability is obtained from the Lifting Supervisor before the plant recommences work.

All work platforms such as scissors lifts, elevating platforms, cherry pickers shall be properly maintained and shall be tested and certified by a competent and Ministry authorised third party. Maintenance records and certification records shall be available for inspection at the Maintenance Dept. 2. 2.1 ATTACHMENT: WORK PROCEDURES FOR LIFTING OPERATION Lifting operation done by cranes should comply with all relevant legislation, regulations, code of practices and any sub-legislation or amendments made there under to. Crane used for lifting shall possess a valid LM Certificate and such certificate must be displayed on the crane. The Load Capacity Chart (manufacturer or certified by Approved Person) shall be displayed on the crane. Crane shall be marked with its safe working load (maximum and minimum radius), and the LM number or other means of identification. Crane shall be used in accordance with manufacturers operation instruction and / or the Approved Persons instructions. Crane shall be provided with: - An adequate braking mechanism; - A swing lock or swing brake capable of prevention of rotation; - A brake or other device adequate to bring the crane to a stop from any travel for which it is designed, together with a means of locking the crane so as to hold its stationary; - A load and / or radius indicator with warning alarm; - A load / radius table to be available in the crane cab of every variable radius jib-crane; - An automatic safe load indicator; - Crane hook with a safety latch to prevent displacement of the sling or load from the hook. Crane shall be examined by an Approved Person after any major repair or any repair done on the hoisting or operating mechanism, boom / jib and after over loading incident. No crane shall be operated unless all measures are taken to prevent toppling or overturning or contact with overhead obstruction (such as structure, power line) When crane are reversing, an addition person shall be required to ensure that the path of travel is clear.

Crane cabin shall lock when the operator is not around. Unauthorised person shall not be permitted to enter in cab or operate the crane. Good housekeeping of the crane shall be exercised and maintained. Crane shall be maintained to ensure that all components of the lifting and operating mechanism are in good, safe and sound functioning condition. A maintenance program shall be required and implemented effectively for the crane, and the maintenance record shall be kept in the cabin. The operator shall carry out daily inspection and testing of crane No lifting operation shall be performed when the load (stationary or otherwise) exceeds the safe working load. The lifting supervisor must verify that the strength of lifting gears and equipment are adequate and their certified SWL (Safe Working Load) are in line with the load they bear. The lifting operation area and other areas with potential hazard / danger must be cordon off to prevent access for all unauthorized personnel. Only authorized personnel involved in the lifting shall be allowed in the lifting operation area. Safety measures shall be taken to address any potential hazards and / or danger that may arise out of the lifting operation. Weather forecasts shall be monitored before and during lifting work. No lifting shall be performed during abnormal conditions like wind speed is too high, poor lighting, obscured view due to raining, etc. The operator shall be able to observe the load continually during lifting. The loads shall be raised vertically to avoid swinging during lifting. Whenever required the load shall be controlled by means of tag lines to prevent the load from
turning or swinging.

Hoisting or lowering loose materials such as small brackets, cut pipes, etc, shall be done in covered containers or receptacles, and to be secured firmly. Only competent, trained and authorised person shall perform lifting and crane operation, rigging and signaling. Lifting Supervisors, riggers and signalers shall wear distinctively coloured safety helmets or other clearly identifiable clothing. Signals shall be specified. Two-way communications shall be arranged whenever required. Only authorised signalers shall be allowed to give signals and signs to the crane operator. All lifting gear must be identifiable and checked before and after use. When a load is suspended at two or more points with slings, the eyes of the lifting legs or the slings shall be shackled together and placed on the hook or shackled directly to the hoisting hook.

Hoisting of personnel to height by crane is strictly prohibited, unless otherwise approval is given in writing by The Commissioner for Workplace Safety and Health (Ministry of Manpower). No crane shall operate in such a position and / or location that any part of the crane, its load in any position, the jib boom, or its swing comes within 3 metres of a live power line unless otherwise the power is de-energised or the crane shall be effectively earthed. Underground cables and / or pipelines which are crossing / located under the crane access shall be adequately protected with mats or steel plates to prevent damages. Using plate clamps 2.34.1 2.34.2 Areas of work must be kept clear during operation. Loads must not be lifted a mans height. However tag lines can be used to control lifting. All lifting clamps are subjected to annual inspections by Ministry of Manpowers (MOM) Approved Examiner.

OBJECTIVE To establish Safe Work Practices (SWPs) for known hazardous activities, so that such works are carried out safely. DESCRIPTION Safe work practices can minimise, if not eliminate the risk of an accident. This element contains well-defined procedures and instructions to carry out specific works, tasks and equipment operations safely. SCOPE OF APPLICATION Safe work Practices shall be established for other hazardous work or work processes as and when required. The SWPs serve as a guide. Persons In-charge of respective tasks are to ensure that all working personnel follow all prescribed safe work practices as required under the local code of practices and mandatory requirements.

4.2.

For new methods of work and those works without SWPs 4.2.1. For new work or equipment to be used, Person In-charge (PIC) is to submit a Work Method Statement or procedures for review by the Safety Department and Safety Committee, prior to commencement of the work. The procedure for submission of new method of statement is:4.2.1.1. 4.2.1.2. 4.2.1.3. 4.2.1.4. 4.2.1.5. 4.2.1.6. 4.2.1.7. 4.2.1.8. PIC to submit Risk Assessment (RA) & Work Method Statement (WMS) at least 1 week prior to commencement of work; Safety Department to review the RA & WMS; Related parties to discuss or meet to go through the safe work practice for better understanding and input; PIC to disseminate the SWP and safety measures to be taken to concerned staff/ contractor; Concerned parties to commence work only after complying with the safety requirements in the SWP; Safety Department to incorporate the SWP into this SMS; Safety Supervisor shall supervise for compliance to safe work practices (SWPs); Safety Department shall inspect periodically for adherence to SWPs.

4.3.

Hazard Analysis 4.3.1. As a precursor to development of SWPs, Risk Assessment and Job Safety Analysis may be carried out to identity the hazards related to the job

activity, so that the development of new SWP is systematic. For details, refer to the element on hazard Analysis. 5. 5.1. RESPONSIBILITY Department Managers 5.1.1. 5.1.2. 5.1.3. 5.2. Ensure that safe work practices are established for existing and new hazardous works in the premises; Review safe work practices prior to submission to Safety Department; Monitor the compliance of staff and contractor in adhering to the prescribed safe work procedure.

Superintendents & Supervisors 5.2.1. 5.2.2. 5.2.3. Obtain a copy of SWPs prior to commencement of hazardous work; Communicate the contents to the parties involved in the work; Ensure compliance to safe work practices.

Review Risk Assessment, Work Method Statements submitted by Department & Contractors; Incorporate safety provision in the Work Method Statement, as necessary; Request for additional information or details if Work Method Statement submitted contains insufficient information; Meet, discuss and present the safe work practices to relevant staff/ contractors; Conduct periodic inspection of staff and contractors on compliance to safe work practices. Incorporate newly established SWP into this SMS. Review established SWPs, amend and update as and when necessary.

SAFE WORK PROCEDURES Safe Work Procedures and Master list are as attached in Appendix. Amendments required for safety improvement of the SWPs, are to be brought up to the Safety Department for discussion and then to adopt as recommended by the Safety Committee.
