Patients file documentation for fourth year students

* Please note that you must write notes with correct spelling. Avoid the story style, write in SHORT NOTES style * Write your FULL name with CLEAR letters at the END of your notes * Correct abbreviations are used only when applicable. For example, (MAF) instead of Master Apical File, but NOT (Lido) for Lidocaine * Your notes should include the following sections: -C/C (Chief complaint). Note that this could be related to the tooth you are going to work on - Hx of C/C (History of chief complaint) - Mhx (Medical history) -Rx done (Treatment done): 1. The number and location of the tooth, e.g. 2. LA: specify the technique (e.g. ID block, lower right quadrant), the type of anesthetic used (e.g. Lidocaine 2%, epinephrine 1:100,000), and the amount (e.g. the cartridge) 3. Preparation work*; e.g. Class I cavity preparation with buccal extension 4. Liner/base (if applicable) 5. Restorative material; e.g. Class I amalgam restoration 6. P.O.Is (Postoperative instructions) 7. Any further notes; e.g. the restoration need to be re-done (specify the reason), the patient was told about the consequences *For root canal treatment (RCT), you need to write down each step in SHORT NOTES within the (Preparation work) section as follows: Access cavity and pulp extirpation EWL (Estimated Working Length), CWL (Corrected Working Length). Reference point for each canal Straight line access with Gates Glidden (e.g., size 2 and 3) Cleaning and shaping, IAF (Initial Apical File), MAF (Master Apical file), FAF (Final Apical File) Master cone, Obturation of 2 canals (if the tooth has 2 canals and so forth) Course coordinator/ Dr. Aveen Aljaff
