Code No. 504.10 EMERGENCY DRILLS Students will be informed of the action to take in an emergency. Emergency drills for fire, weather, and other disasters shall be conducted each school year. Fire and tornado drills shall be each conducted regularly during the academic school year with a minimum of two before December 31 and two after January 1. Employees shall participate in emergency drills. Licensed employees shall be responsible for instructing the proper techniques to be followed in the drill. It shall be the responsibility of the superintendent to develop for board approval an emergency plan.

FIRE
EMERGENCY PLANS FOR THE SOUTH HAMILTON SCHOOLS
REQUIRED BY IOWA CODE 100.31 AUTHORIZED BY BOARD POLICY 504.10 (ADOPTED 4/84, REVISED 12/95) The plan has been given to the local fire department and the local police. The school is also connected to a 911 system which will be used. The district has electronic warning systems and is also connected to the security system which operate 24 hours a day, seven days a week. The local fire department is on the "call" list of our security system and is notified upon activation of the alarm system. LOCATION OF EXIT ROUTE POSTERS Each office, classroom, and work area in the building will have a poster that clearly explains the exit route and the exit door to use in case of a fire emergency. LOCATION OF FIRE ALARMS AND THEIR USE Fire alarms are located in each building. Activating an alarm switch, most of which are pull down type, will automatically activate the fire horn which is a continuous horn. LOCATION OF FIRE EXTINGUISHERS Main hallways in both buildings are equipped with fire extinguishers. In addition, fire extinguishers are available in areas that would most likely be prone to fires being started. EMPLOYEES SHOULD BE TRAINED IN THE PROPER USE OF FIRE EXTINGUISHERS AND REMINDED OF LOCATIONS. THIS SHOULD BE DONE ON EMPLOYMENT AND AT LEAST EVERY THREE YEARS THEREAFTER. Please note that trained inspectors inspect fire extinguishers and the sprinkler system annually.

FIRE CONTAINMENT PROCEDURES Teachers should make certain that windows are closed (if time permits) and close doors when leaving the room. This will cut off drafting. The teacher should not neglect supervision of exiting the students in order to close windows. The building custodian or maintenance supervisor will proceed, if safety allows, to the monitoring system located in each building. (This information should be relayed to the firemen arriving at the scene.) The principal and the custodian can then determine if fire extinguishing should be used.

NOTIFICATION OF EMERGENCY PERSONNEL The building warning system will notify the fire department upon the setting off of smoke, heat, or alarm switch activation. The building principal should also instruct a secretary to call 911 after determination has been made that the alarm was "the real thing" and is of a serious nature.

PERSONNEL RESPONSIBILITIES A. The attendance secretary should check with each teacher to see that role has been taken. This will ensure that no personnel or students are left in the building. Secretaries in both buildings should make certain that fire proof record files are closed. B. The custodian will proceed to the master fire monitoring system to determine the location of the fire zone. (This information will later be relayed to the principal/fireman.) C. Teacher aides will check restrooms in the MS/HS(delete) to alert anyone in the restroom of the emergency. (Aides should go to the nearest restroom at the time of the emergency.) D. Teachers will evacuate students as quickly as possible using the designated routes or safest routes as determined at that time. Teachers should be the last to leave the room and close the door. (Teachers should check restrooms if they are not responsible for a classroom evacuation.) E. Special education students needing assistance because of vision, hearing, or orthopedics, will be the last students to leave the room with the teacher or aide responsible for them at the time. F. The school nurse and building principal will be responsible for evaluating drills and recording same. The nurse and the building principal shall be responsible for assuring that persons are properly and routinely trained to do what is required of them. The nurse should make recommendations to the building principal for improvements in the evacuation process. STAGING AREAS Once students and staff have exited the building, they should proceed to at least fifty feet away from the building and should stay together so that roll taking could take place. Care should be taken to not get in the way of fire department equipment and/or personnel.

FIRE DRILL RULES A. Teachers should discuss the fire drill exit routes and rules with classes at the beginning of each semester. B. Students should walk quietly and orderly to the designated exit. Teachers will accompany their students out of the building. C. Teachers should take roll (report to attendance secretary at each building) once their class has reached their designated staging area. D. Books, coats, or other belongings should not be taken when evacuating a room. E. All clear to reenter the building will be verbally given by the building principal or his/her designee. EVACUATION ROUTES AND PROCEDURES

The fire alarm is a continuous horn. Persons leaving the building should walk in an orderly fashion and keep in the same group so that roll can be taken upon reaching the staging area (Jewell Community Pool). If a teacher or supervisor is uncertain of a route, get the students out of the building by taking the nearest route to an exit.

Signs which show or explain fire and tornado drill routes should be prominently displayed in each classroom. When the fire buzzer sounds, the children should leave the room quickly without stopping to get coats, or to take anything with them. During a fire drill a TEACHER should do the following:


1. Have a couple of children selected to close the windows. 2. Turn off lights and close the door. 3. Check to make sure all children are out.

TORNADO
LOCATION OF TORNADO EVACUATION POSTERS Each office, classroom, and work area in the building will have a poster that clearly explains the shelter evacuation area for that room. TORNADO WARNING SYSTEM The warning will be given by office personnel by using a portable air horn. The person/persons will circulate throughout the building sounding the horn. The secretaries will also give warning over the intercom system. The air horn has a totally different sound than the fire alarm. The warning system will be initiated when, (1) a tornado warning has been given for the area, (2) when a tornado has been spotted by anyone in the building or when clouds look very dangerous, (3) whenever in doubt, it is better to be safe than sorry. COMMAND CENTER AND INFORMATION CENTER As soon as possible afterwards a command center will be established near the building where the threat exits. The local police and Hamilton County Sheriffs Office will be in charge of this center. The Superintendent [if not present, the Grades 7-12 Principal] shall establish an information center. At that center, the Superintendent will have radio contact with other administrative personnel, the Transportation Director, the sheriffs office, Jewell Fire and Rescue, and the local police. All information to the media relating to students, school cancellation and other school related matters should go through this center only. More than likely, the center will be established at one of the local churches or at the Superintendents residence. RELEASE OF STUDENTS
As a general rule, students will be released to their parents only. Since the elementary students will be in an area inaccessible to the public, parents will have to wait until that point in time that the Superintendent releases them. This will be done only after consultation with the law enforcement officers. Parents can contact the Information Center with inquiries about their child [ren] by calling 1-515-836-HAWK (4295). The Superintendent [or other person in charge] is the only individual authorized to cancel school. This will be placed over the radio stations that the district normally uses for cancellations. The building principals should stay with their students at all times. Only in emergency situations should they leave the students and staff. All staff members [unless otherwise assigned] should stay with the kids and exhibit as much calm as possible under the conditions. Remember if you act in an unusual manner, the students will also.

INFORMATION FOR PARENTS A phone bank will be established with the help of the Ellsworth Cooperative Telephone Company. Parents can contact 1-515-836-HAWK (4295) with questions regarding their children, school cancellation information or other matters. The following individuals shall be assigned to that phone bank, which is located at the site of the Ellsworth phone office:

Lannie Schafroth will also be stationed at this site to coordinate communications with other staff in regard to answering parental concerns about their children. Lannie will also inform parents via Twitter, School website and Infinite Campus about the phone number to call for information.

PERSONNEL RESPONSIBILITIES DURING A DRILL A. The secretary will sound the warning over the intercom system. If a storm does hit the building, the secretary will assist in taking roll to assure that all students are accounted for. Fireproof files containing student records should be closed and secured. B. Teachers will evacuate students as quickly as possible to the designated shelter staging area. Teachers will take roll after all students are in the proper staging area. C. The principal and all teachers will be responsible to see that everyone receives the warning (playground, athletic fields, etc.). D. The custodian will be prepared to shut off gas lines and other utilities that could become potentially dangerous. The custodian will also try to maintain contact with emergency crews such as fire departments, police, etc.. E. Special education students needing assistance will be assisted by the teacher in charge or teacher aide responsible for the student at the time of the emergency. The special education teacher and/or associate will assist in carrying any student with limited mobility down the stairs and if he/she is not available other available individuals should be summoned to assist. If all else fails, students of other teachers should assist. The elevator should not be used as the electricity could go off leaving the student stranded in the elevator. TORNADO DRILL RULES A. Teachers will cover tornado drill rules and staging sites at the beginning of each semester. B. Teachers will be responsible for seeing that posters are properly posted in their classrooms. C. When the alarm is sounded, students will walk quietly and orderly to the designated shelter staging area. D. When students arrive at the shelter staging area students will kneel on the floor and cover their heads with their hands, book, or anything available. If the area is crowded, students may have to stand. Ventilation should be considered if too crowded. Another place may be more desirous. E. Teachers will take roll and be prepared to report attendance to the secretary or authorities. Every student in each teachers charge should be accounted for. F. Students should not leave the school grounds without permission. G. Buses should not be sent on the routes if a warning is evident. H. Stay away from glass. Flying glass is a very dangerous potential in a tornado.

REMEMBER---TEACHERS AND ADMINISTRATORS SHOULD BE ABLE TO ACCOUNT FOR ALL STUDENTS FOLLOWING THE EMERGENCY. STUDENTS SHOULD NOT LEAVE THE SCENE UNLESS ACCOUNTED FOR AND RECORDED WITH THE SECRETARY.

TORNADO PROCEDURE FOR OUTDOOR EVENTS A. In the event a tornado during an outdoor event, the administrator in charge of the event will open the nearest school building to serve as a shelter for those present. B. Any staff present will help direct those present to shelter areas. Locker rooms, rooms without windows, and hallways will be used as shelter areas. C. Coaches are to account for all student/athletes present for the event. TORNADO PROCEDURES ELEM. The tornado shelter areas are the following:

BOMB THREAT
1. The person taking the call should keep the caller on line as
long as possible.

2. Using a bomb threat caller form, get as much information as


possible from the caller.

3. The administration will determine if the threat calls for


evacuation of the buildings.

4. Call 911 to alert authorities. 5. Use fire evacuation procedures to remove everyone from
both buildings.

6. Do not use walkie-talkie radios or cell phones. 7. Off-site evacuation location will be given only when an
evacuation occurs.

8. Move students and staff to designated area 9. Take attendance and account for all individuals 10. Remain in designated area until all-clear signal is given.

BOMB THREAT
Emergency Plans
LOCATION OF EXIT ROUTE POSTERS Each office, classroom, and workroom will have a poster that clearly explains the exit routes for fire emergencies. These routes will be used in case of a bomb threat. NOTIFICATION OF EMERGENCY PERSONNEL AND EVACUATION OF BUILDING The principal or office secretary will copy any information that the caller gives. Try to get as much information as possible and try to keep the caller on the line as long as possible using the form provided to record such an incident. When the call is ended an administrator should be notified, then secretary or principal should immediately dial *57 to begin the trace on the call. After that *57 call is made, or at the same time by another employee, 911 should be dialed in order to alert the authorities. As soon as possible the fire alarm will be activated by school personnel if it is determined that the building should be evacuated. When the call is ended whoever took the call should 1. Notify building administer 2. Complete report 3. Talk to no one other than the administrator 4. Dial *57 to begin the trace on the call. After that *57 call is made, [or at the same time by another employee] 911 should be dialed in order to alert the authorities. 5. As soon as possible the fire alarm will be activated or an announcement made by school personnel if it is determined that the building should be evacuated. 6. Follow evacuation procedures to the level determined by administration. Our plan calls for the evacuation of both attendance centers even when only one of the centers has been threatened.

Elementary - Students at the elementary will follow these routes for evacuation of the buildingRoutesFourth and Third grade use north doors Second grade use middle doors First grade/ Prep-K/ DLC use East Main entrance Media Center use East Main entrance Kindergarten use classroom exit doors Preschool room use classroom exit doors Gymnasium use south exit doors 5th - 6th Wing use west doors Art, Music, Resource rooms use Art room door Computer lab, Title/RR, Guidance use doors north of lunchroom Band, Gym use southeast gym doors If time permits, and we know we will going to off site building evacuation, the buildings can be evacuated by having the students load buses, (both schools) in the normal bus loading zones. Students are to go directly from building to buses in an orderly fashion. Wait inside for the buses to come.

At the Grades 7-12 building, the students will be evacuated to the area near the city ball fields directly
east of that site. The students will be picked up in buses to transport them to an off-campus location. When snow makes it infeasible to walk directly past the swimming pool area to the designated location, the students will walk along Park Street. During foul weather conditions, after students are safely evacuated, the following staff are designated to go to the bus barn to drive buses to those Grades 7-12 students in order to provide shelter from the elements:

Following the arrival of the buses at the 7-12 evacuation area, students will be transported to an off-campus location. When possible students will be loaded on the buses by grade level. TEACHER AND SECRETARY INFORMATION You should take a copy of the student attendance MASTER list with you. School secretaries will take CRISIS RESPONSE BOX to evacuation site. At the evacuation site, roll should be taken. Students should break into grade level groups in order to facilitate this. In the case of inclement weather roll should either be taken at the evacuation site or on the bus prior to departure to the off-campus location(s). Radios on the school frequency will be assigned to (delete)Cell phones will be used with Superintendent, Grades 7-12 Principal, Grades K-6 Principal and the phone bank site. B. Additionally, the Superintendent will have cell phone contact with law enforcement and local fire and rescue. C. Only when the law enforcement officials have cleared the buildings will staff or students be allowed to reenter the buildings. Vehicles should not be removed from either parking lot until cleared by the school. D. The School Nurse should accompany the students in whatever building she happens to be in. Its important that the nurse stay close to a cell phone in order to learn about injury cases.

EMERGENCY PROCEDURE FOR ATTEMPTED SUICIDE OR ACCIDENT


This procedure is recommended for an attempted suicide or accident at school. TEACHER/PEER DUTIES 1. Remain calm and use COMMON SENSE. 2. Never doubt a student's threat or said attempt. 3. Bring student to the office personally, if conscious or call office. Do not leave student alone at any time. PRINCIPAL/COUNSELOR/NURSE 1. Compile as much information about the attempt in as short a time as possible (how many, where, when, etc.) 2. An appropriate staff member will accompany the student in the ambulance and remain at the hospital until a parent arrives. 3. Document happenings as soon as possible. 4. Staff will be notified of pertinent facts at earliest possible time. SECRETARY/NURSE/PRINCIPAL 1. Call 911. 2. Call parent. 3. Call hospital if source is recovered, giving the name and estimated number of pills taken. 4. Notify superintendent of the situation. SUPERINTENDENT 1. Notify board members.

NOTICE If an attempt is by a method other than pills or an accident occurs on school property: 1. DO NOT move student. 2. Call 911. 3. Keep spectators away and do not disturb site pending investigation of incident by authorities. 4. Render first aid if qualified.

ADDITIONAL SUGGESTIONS 1. The school secretaries should be given a prepared statement to be used in response to incoming calls concerning the crisis. They should not deviate from the prepared statement. 2. The superintendent should be the only person who interacts with the media. 3. An announcement that has been written by the administration will be distributed to the staff. This announcement will describe the facts of the crisis. The staff members will be instructed to read this announcement at the beginning of their first period classes. 4. Kandy Martin at the AEA should be contacted. Kandy Martin or Denise Gajewsky will coordinate a crisis intervention team for the specific suicide or accident. At the present time, they manage the crisis intervention teams locally through the social workers in the AEA office. They will also call in any other experts or staff needed to deal with the crisis. Other local AEA personnel may assist in making these contacts. SUGGESTED GUIDELINES AFTER A SUDDEN DEATH


DAY OF DEATH Faculty 1. School administrators will verify the information about the death. 2. Kandy Martin at the AEA should be contacted. Kandy Martin or Denise Gajewsky will coordinate a crisis intervention team to deal with the death(s). At the present time, they manage the crisis intervention teams locally through the social workers in the AEA office. They will also call in any other experts or staff needed to deal with the crisis. 3. School principals may activate the phone tree to inform all faculty and K-12 support staff of the death and to request their arrival at school 30 minutes earlier than usual to attend a special faculty meeting if it is necessary. The phone tree should be in place before a crisis occurs. 4. Telephone conferences with selected staff members are held to plan tentative activities for the next day (day after a death). 5. School administrators should call substitute teachers ahead of time. The substitutes will be at school in case any staff members are unable to perform their duties. They may also be used to float throughout the school to provide teachers a few minutes relief from classroom duties as needed. 6. It will be the right of the School District to hold all students in class until students are released directly to parent or guardian. No student will be allowed to leave the building without the accompaniment of the primary guardian. Note: In this particular case the school has the responsibility to protect a student during this critical time. It has been documented; students who are close in nature to the victim share a grieving moment, possibly causing harm in a self-inflicting way. These students need to be monitored/comforted at all times during these critical situations. FIRST DAY AFTER A DEATH 1. School principals meet with selected staff 30 minutes before meeting with faculty to plan the aftermath of a death. 2. Principals review the facts of the case with all the faculty and support staff to dispel rumors, to discuss the plan for the day, and to allow for faculty members to express their feelings. Faculty/staff are encouraged to lend support to one another. Inform the staff that the superintendent is the only person who will be interacting with the media.

3. A member of the school staff, an outside support person, or counselor, may give the faculty all the facts of the death. They may describe some of the feelings the students may be experiencing following the death of an individual....disbelief, anger, denial, sadness, loss. Suggestions are reviewed on ways to encourage the expression of grief in their classes. 4. An announcement that has been written by the school counselors will be distributed to the staff. This announcement will describe the facts of the death. The staff members will be instructed to read this announcement at the beginning of their first period classes. 5. A C.A.R.E. center is established in one of the rooms in the school. Additional pupil personnel staff from other buildings and from neighboring school districts and the AEA Crisis Team may be called in to assist with the crisis. Staff from your local mental health center may be helpful. A member of the staff makes phone calls to parents of students who are particularly upset or may be at risk. The staff member can explain the students' reactions to the death and the team member can make appropriate suggestions to parents on ways to manage their children. It may be necessary for parents to come to school and take the youth home for the day or to receive immediate professional help. Resources for professional help are made available. School social worker and school psychologist will be called on for help. 6. A call may be made by the principal or designee to a local mental health center and/or school social worker/school psychologist to notify them of the death and arrangements are made to elicit help from them. When outside consultants are called upon, they can be useful because they are generally not emotionally involved with the case and can provide objective support and direction. Some of the services they provide are: Individual counseling for students Speaking at a general faculty meeting on the issue of suicide and/or death; identification, prevention, response, and help with some of the concerns generated in staff or in students. Conducting evening informational meetings for all concerned community members. 7. "Peer Facilitators" are assembled to work through their feelings, and staff members can offer them some guidelines for helping their troubled friends. 8. A letter should be sent home with all students. This letter will inform families about what has occurred and will also inform them about what the school district is doing to assist the students. 9. School staff are assembled at the end of the school day. The principals conduct the meeting and do the following: a. Allow for the expression of feeling and mutual support. b. Review the events of the day. c. Review the characteristics of high-risk students (those who seem especially upset or depressed or show other signs of not coping well) and compile a confidential list of staff observations of distressed students' reactions during the day. d. Announce the wake and/or funeral arrangements and make arrangements for staff to attend in order to provide support to students and their families. e. Review the plan for the next school day. 10. Teacher, Counselor, and/or Principal visit family to offer support and sympathy (minimum of 2 people).

SECOND DAY AFTER A DEATH 1. Announce an open meeting for concerned parents. This can be an evening meeting. 2. Meet at the end of the day to discuss staff concerns regarding events of the day. 3. Administration, counselors, staff, and clinical staff continue crisis intervention, answer phone calls of anxious parents, and meet with concerned staff. 4. Principals may call or write letters to all parents of "high-risk" students reminding them to seek professional evaluation. 5. Outside consultants and school staff conduct an evening meeting of concerned parents. The consultants, making use of their "expert" status, urge the families of those students who are not coping well to pursue evaluations at one of the public or private mental health agencies listed in their area. Administrators, staff members and clinical staff are available to privately answer parents' specific concerns about why some children were identified as "at risk" or any other concerns parents may be having. Clinical staff meet for half an hour following parent meetings to discuss meeting.

THIRD DAY AFTER A DEATH (OPTIONAL) 1. School/Community Steering Committee may be formed and generate plans for a meeting involving interested community leaders to discuss a community-wide response to the needs of youth of the towns or area. Take into consideration leisure time activities, alternatives to school suspension, incarcerated and minority youths' concerns, and runaways. 2. "Front-line" staff who have been dealing directly with the crisis meet with a consultant for the expression of feelings and mutual support. (This is a very necessary ingredient, as "burnout" can be a problem with crisis situations.) Also, former losses of "front-line" staff may come to current awareness. A mental health consultant is very useful to prevent staff burnout. ADDITIONAL SUGGESTIONS These suggestions should be addressed soon after the youth's death has been announced. 1. The superintendent should be the only person who interacts with the media. 2. The school secretaries should be given a prepared statement to be used in response to incoming calls concerning the crisis. They should not deviate from the prepared statement. For example, We can confirm that there has been a death. All other information needs to come from the superintendent." 3. The principal and/or counselor along with members from the staff, should visit the victim's family at home. Expressions of sympathy and support, should be expressed. Also, the student's personal belongings from lockers, etc. can be returned to the family at this visit or plans can be made to return the belongings to the family. 4. If the funeral occurs during the school day, it is a voluntary service for students, family and faculty who wish to attend. The service will not glamorize the death and references can be made to the act as an unwise decision or an error in judgment if it was a suicide. The service may serve as the institutional "signal" that the official period of mourning is over. The work is not over at this point, but can be an ongoing process of prevention if death was by suicide. You will need to make a decision at your school when "business must go on as usual."

RESPONSE OF STAFF AFTER A YOUTH DEATH 1. Staff are encouraged to allow for the expression of grief in their classes in whatever extent they are comfortable. The guiding principle is to return to the normal routine within each class and within the school as soon as possible. 2. The teachers are asked to dispel rumors whenever and wherever possible and to discourage any "glorification" of the death. 3. Review the known facts and dispel rumors. 4. Help students separate reality from fantasy and de-mythologize the suicide act. 5. Inform them of the C.A.R.E. center at the school and availability of support from it. Inform them of community resources for mental health. It is helpful to distribute a written list of names, phone numbers, and addresses of resources. 6. Encourage students to express their reactions in whatever way is appropriate for them. All responses are acceptable, from being severely upset to no reaction whatsoever. 7. Discuss possible guilt or feelings of responsibility that students may be having. 8. Ask students to be supportive of one another and to escort any friend who is upset to a member of the C.A.R.E. team or to the C.A.R.E. center. 9. Reassure students that any adult in the building is available to help in an emergency or is willing to listen. 10. Encourage students to discuss their feelings with their parents.

SOUTH HAMILTON COMMUNITY SCHOOL DISTRICT SCHOOL BUS ACCIDENT PROCEDURE


PURPOSE The purpose of this procedure is to: 1. Provide a standardized approach to the management of school bus accidents involving South Hamilton Community School District busses and students. 2. Provide emergency care as appropriate for the incident, while make the most efficient use of available resources.

PROCEDURE FOR BUS DRIVER 1. Call base by radio (preferred) or telephone (515-827-5479). Give exact location of accident, along with information about severity, injuries and hazards. (Recipient of information should immediately call 911 to report accident then contact the Superintendent and Transportation Director with accident information.) 2. Set parking brake; shut engine off. Turn ignition switch back on to maintain radio contact unless conditions such as fire or other safety concerns prevent you from doing so. 3. Remain calm and reassure passengers. Keep the situation as orderly and low-key as possible. 4. Assess conditions. Be alert to the potential for fire, fuel leaks, bus rolling/tipping, severe weather/sited tornado, rising water or collisions with other vehicles. 5. Use warning devices, such as reflectors, where applicable. 6. Keep passengers in vehicle unless fire or other hazardous conditions exist that warrant evacuation. If evacuation is necessary, analyze conditions to determine the safest exit from the bus and safest gathering location. Move evacuated students to the nearest safe location at least 100 feet from the bus. It is extremely important that injured persons are not moved unless a hazard exists that presents an imminent danger of further injury. NOTE: The safety of the bus and equipment is secondary to the safety of the students. No attempt should be made to save equipment or personal items until all students are removed from the bus safely, are out of danger and are supervised. 7. Protect the scene: a. Protect the passengers and vehicle from further accident and injuries; b. Protect the scene from traffic and people so that evidence is not destroyed. c. Under normal circumstances, the vehicle(s) involved should not be moved unless law enforcement personnel advise the driver to do so. 8. Be prepared to give information to emergency medical personnel regarding individual students medical or physical requirements.

PROCEDURES FOR GENERAL OPERATIONS 1. Emergency services agencies (police, fire, EMS) will take charge of the accident scene upon their arrival. The Transportation Director and Superintendent, or appointed representative, will be dispatched to the accident scene and will interface and work cooperatively with the designated incident commander at the Command Post. 2. All injured and potentially injured persons (as determined by EMS personnel) will be transported to area hospitals by ambulance. Anyone not requiring medical treatment will be transported as per item #4 (below). NOTE: The responsibility for the determination of injuries and potential injuries for any person involved in the accident rests with the highest appropriately trained on-scene EMS personnel. In the event that the number injured persons exceeds ambulance capacities, a school bus or other vehicles as needed will be dispatched, at the discretion of EMS personnel, to transport injured students to the hospital. EMS personnel will be placed on the bus or vehicle for further care during transportation to hospital. 3. If it has been determined by emergency response authorities at the scene that the accident is minor in nature (little or no damage to school bus, estimated forces involved suggest no mechanism for injury, no complaints or signs of injury), every effort will be made to avoid unnecessary transport of the children to area hospitals. In that event, the school district representative(s) at the scene (with the exception of the school bus driver) will have the option, as legal guardians of the children under18 years of age, to sign a release form declining hospital transport. The form utilized (Appendix A) will list the names of all children that have been evaluated by EMS personnel, have been found to lack mechanism of injury, signs of injury AND have no complaints of injury. Any child not listed on the release form (by virtue of the fact that injuries cannot be ruled out) will be transported to a hospital for further evaluation. 4. Anyone not requiring hospital medical attention will be transported to the school in the following manner: a. School vehicles, such as automobiles and vans, not involved in the accident will be dispatched to the scene at the request of the designated accident commander and school district representative. b. School counselors and other personnel, as needed, will accompany these vehicles and ride with students to school buildings.

Stop food service until health officials approve restarting it. j. Remain in CRBNE Shelter-in-place mode until notified by designated official.
