Transcript of Records (TOR) that indicates and has the following: degree/course exact date of graduation Special Order (S.O.)/exemption/accreditation/deregulation status scanned picture and; the remark: FOR BOARD EXAMINATION PURPOSES ONLY Note: -Graduates of government schools and institutions/programs accredited by recogni zed accrediting agencies under the FAAP are exempted from S.O. -Graduates of New Schools/Degree/Programs must submit School Recognition and/or Permit to Operate. 5.
