Questionnaire

This brief questionnaire will be used as a teaching tool. We will critique the questions, identifying
common measurement problems that are found in many surveys (whether self administered, like this
one, or face-to-face or via phone). We will also use the results to create a codebook and data files for
analysis.

Please answer each question as best as you can and as completely as possible. Your responses will be
held in confidence and will not be identified with your name. The student identification number
requested below is required only for administrative purposes. We will learn how to add data to the files
we create later in the semester, and this can only be done by linking the new data to the old data via
this identification number.
