Resumen La obesidad es una enfermedad muy prevalente en un gran nmero de pases desarrollados y en vas de desarrollo, que adems muestra una tendencia a la progresin. La franja
de pacientes obesos susceptibles de ser tratados mediante tcnicas endoscpicas puede ser muy
amplia. El baln intragstrico de Allergan es el tratamiento con mayor aplicacin en Espa
na y
en Europa. Presenta un perl de ecacia a corto plazo bueno, a largo plazo subptimo y su perl
de seguridad es elevado, aunque pueden aparecer complicaciones graves, por lo que no debe
trivializarse su indicacin ni su implantacin. Adems, existen diferentes dispositivos endoscpicos que consiguen que los pacientes obesos pierdan peso mediante diferentes mecanismos de
accin, pero estn en fase de desarrollo tecnolgico y de validacin cientca. Es pronto para
recomendarlos, y solamente deberan ser evaluados en el contexto de ensayos clnicos.

Sistemas de tratamiento endoscpico de la
CA, EE. UU.), aunque fue la empresa Bioenterics el fabricante original y quien introdujo el baln por vez primera en
el mercado. Bioenterics fue adquirida posteriormente por
Allergan. Otros sistemas menos conocidos son el baln rellenable con aire (comercializado con el nombre Heliosphere),
el baln intragstrico ajustable (de Spatz), la prtesis intragstrica ajustable (de EndogAst) y el baln antral esttico.
Un segundo grupo de sistemas endoscpicos para tratar la obesidad acta produciendo una malabsorcin de
los nutrientes a nivel yeyunal. El sistema que as acta
se denomina funda duodenoyeyunal (comercializado como
Endobarrier).
Un tercer grupo de sistemas endoscpicos produce una
reduccin de la capacidad volumtrica gstrica mediante
la colocacin de puntos de sutura que intentan reproducir endoscpicamente intervenciones quirrgicas baritricas
que son efectivas en el tratamiento de la obesidad.
Entre ellos estn la gastroplastia vertical endoluminal (de
EndoCinch), la gastroplastia transoral (TOGA), el sistema
restrictivo endoscpico transoral y la plataforma operativa
endoscpica sin incisiones (denominada en Espa
na con el
acrnimo POSE, como traduccin del ingls primary obesity surgery endolumenal).Un cuarto sistema endoscpico
acta modicando la neuromodulacin gstrica mediante
la inyeccin gstrica de toxina botulnica o la implantacin
quirrgica o endoscpica de dispositivos que producen modicaciones en la estimulacin elctrica gstrica, que se han
popularizado como marcapasos gstricos.
Finalmente, existe un conjunto de sistemas que intentan rescatar pacientes que han vuelto a engordar despus
de haber sido sometidos a una intervencin quirrgica
baritrica. Entre ellos guran el StomaphyX , el Over-thescope-clip (de la empresa Ovesco) y el sistema POSE antes
mencionado (tabla 1).

baln62 .

en un polmero plstico impermeable, de unos 60 cm de
longitud, que se ja mediante anclajes metlicos en el
ploro y desciende en forma de funda cilndrica a lo largo
del yeyuno. Impide el contacto de los nutrientes con la
mucosa intestinal, y en consecuencia provoca malabsorcin.
Las caractersticas exibles y suaves de la funda no son
impedimento para que la secrecin biliopancretica uya
fcilmente por el espacio que se establece entre la mucosa
intestinal y la propia funda hasta alcanzar el contacto con
los alimentos ingeridos una vez alcanzado el extremo distal de la funda. Su extraccin se realiza mediante frceps
de cuerpos extra
nos; la extraccin puede ser traumtica,
pues deben deshacerse los anclajes y retirarse a lo largo del
estmago y del esfago67 .
Este dispositivo ha experimentado algunas modicaciones en su dise
no y estructura desde su primera implantacin,
que han permitido mejorar especialmente su seguridad68-70 .
Cuando se ha evaluado su ecacia y seguridad en un
estudio multicntrico y aleatorizado en una serie de 26
pacientes que iban a acceder a ciruga baritrica, demostr que en 4 pacientes fracas la colocacin de la funda
por motivos tcnicos, la duracin del procedimiento fue
de 35 min, el porcentaje de exceso de peso perdido al
cabo de 3 meses fue del 19,0% en el grupo tratado con
la funda duodenoyeyunal versus el 6,9% para el grupo
control, siendo la diferencia estadsticamente signicativa
(p < 0,002). Llam mucho la atencin la mejora del metabolismo glucdico observada en 7 de los 8 pacientes con
diabetes mellitus tipo 271 . Este ltimo dato ha sido recibido con un cauteloso entusiasmo por la comunidad mdica
endocrinolgica.
Con la gastroplastia vertical endoluminal iniciamos la exposicin de una serie de sistemas teraputicos endoscpicos
orientados a reducir la capacidad del estmago para
albergar alimentos, provocando una sensacin de plenitud
precoz. Todos los sistemas que vamos a describir permiten
colocar puntos de sutura a travs de un endoscopio convencional o de un endoscopio de doble canal. Unos lo hacen
mediante el mismo endoscopio, otros se ayudan de sobretubos ms o menos complejos. El punto de sutura a travs
de un endoscopio constituye uno de los retos actuales de la
endoscopia teraputica avanzada. El punto de sutura debera ser transmural, rme, seguro, estable y permanente.
En buena parte, el desarrollo futuro de la ciruga endoscpica a travs de oricios naturales (NOTES) depende de
que seamos capaces de dise
nar sistemas y aplicar puntos
de sutura de calidad. Los sistemas que vamos a describir
han sido aplicados en series cortas de pacientes. Todos ellos
han sido publicados pero no todos han conseguido perpetuarse, pues algunos han dejado de aplicarse debido a su
ecacia moderada, a su complejidad o a su bajo perl de
seguridad.
El sistema de gastroplastia vertical endoluminal (EndoCinch) encuentra su antecesor en un desarrollo tecnolgico
realizado por Swain et al.72 , que posteriormente fue
modicada por Bard Endoscopic Technologies (Billerica,
Massachussets, EE. UU.). Este sistema fue dise
nado para
tratar endoscpicamente la enfermedad por reujo
Gastroplastia transoral
La gastroplastia transoral (TOGA) es un sistema igualmente
restrictivo que tambin realiza puntos de sutura mediante
un sobretubo transportador del endoscopio y de un voluminoso sistema de sutura automtica tipo grapadora que
coloca puntos de sutura formando una manga tubular
estrecha a lo largo de la curvatura menor gstrica. La
experiencia comunicada indica que el procedimiento es
efectivo, con un porcentaje de exceso de peso perdido al
cabo de 12 meses del 52,2%. La tcnica precisa unas 2 h
para su realizacin en las que el paciente est sometido a
intubacin orotraqueal. Los puntos de sutura son transmurales, aparece un sndrome postintervencin caracterizado
por nuseas y vmitos de 7 das de duracin hasta que el
paciente presenta una buena tolerancia a los alimentos
ingeridos. Las complicaciones descritas fueron leves79,80 . Sin
embargo, durante el seguimiento endoscpico se ha podido
observar que algunas de las suturas colocadas se desprendan. De hecho, la prdida de peso que se manifestaba
progresiva durante los primeros 4 meses, se estabilizaba sin
descender ms de forma signicativa81 . La prdida de los
puntos de sutura constituye una seria contrariedad tcnica,
que podr condicionar seriamente la continuidad de este
desarrollo tecnolgico. En trminos quirrgicos, un sistema
de sutura que pierde los puntos de sutura y desmonta una
construccin realizada es un sistema no aceptable.

acetilcolina en las terminaciones neuromusculares colinrgicas. El efecto de la toxina botulnica es temporal;
se pierde de forma paulatina pasados los primeros 3 a
6 meses, sin producir ninguna lesin permanente sobre las
estructuras en las que acta85 .
Las dosis administrada en los pacientes afectos de obesidad pueden oscilar entre 100 y 500 UI de toxina botulnica. El
lugar de inyeccin ms utilizado es el antro gstrico, aunque
tambin se ha asociado la inyeccin en el antro con la inyeccin en el fundus gstrico86,87 . El nmero de inyecciones
realizadas oscila entre 8 y 24. Los resultados de efectividad,
todos ellos en series cortas de pacientes, indican resultados
discrepantes: en 4 estudios no se observ ecacia88-91 , y en 3
estudios la prdida de peso fue de entre 4,0 y 4,9 puntos de
IMC86,87,92 . Los datos de seguridad de todos ellos indican que
no existen efectos secundarios, con independencia de la tcnica utilizada, dosis o lugar de inyeccin en el estmago93,94 .
