Phaze Concrete, Inc. was incorporated in 2003 in Utah as a concrete subcontracting firm servicing the commercial market. As
a subcontractor, the company performed turn-key concrete contracts for big box stores, schools, and parking structures, and
airport work. As the company has grown it has expanded operations to include earthwork and utility operations. In 2013 Phaze
Concrete and Jack Daniel 's Construction, Inc. merged. Jack Daniel' s Construction is a Utah contracting firm with experience
in General Contracting which includes municipal and county projects, road construction, and excavation, utility, and concrete
operations. Today, Phaze Concrete is a nationwide contracting firm having performed contracts in every state west of the
Mississippi River, and with an ongoing presence in several major metropolitan areas, including Kansas City, Oklahoma City,
Denver, Phoenix, Portland, and Seattle.

I was born and raised in southern Utah I Northern Arizona. I enjoy outdoor activities such as fishing,
hiking, and hunting. I love to spend time with my children. I also enjoy flying my Cessna 182.
Started working earthwork while still in high school. After graduating from high school in 1971 I spent 4
years building dams and dikes for the Soil Conservation Service. I operated Dozer, Scraper, Compactor, Road
grader and helped with Survey.
I was married in 1977 and worked for various contractors in Residential and
Commercial Concrete up until 1981 in Utah, Arizona, and Nevada.
Started working for Meadow Valley Contractors in 1981. Worked as job Foreman on jobs such as Bridge
at I-15 & California Wash, Laughlin Waste Water Treatment Plant, Crown Point Subdivision development,
Bridge across Virgin River at Mesquite Nevada.
1985 worked as project Superintendent on projects such as Bridges on highway between Virgin and
Rockville Utah, Bridge at Sloan Channel on East Charleston, Las Vegas, Bridge widening on I-15 and Craig Road
in Las Vegas, Parking Garage at Palace Station Las Vegas, Floor slabs at Whisky Petes, and Gold Strike Inn.
1986 worked in Phoenix Arizona as project Superintendent on Arizona Department of Transportation
projects such as Ray Road & 1-10 Traffic Interchange, ($6 million) East Tunnel Outfall, ($3 million) and West
Tunnel Drainage System ($7 million). These Tunnel Projects involved massive underground concrete structures
connecting the tunnels to the Salt River. Also included a pumping station with 3 - 30" Engine driven pumps as
well as smaller pump stations. We constructed several inlet connections to the 25' diameter tunnel 50'
underground, working on down town city streets.
1989 worked as Project Manager for Meadow Valley Contractors on Payson Show low Highway,
Mogollon Rim job ($15 million). This Arizona Department of Transportation job reconstructed approximately 3
miles of road in sensitive forest environment. It had considerable rock excavation and required major blasting. It
also included 2 concrete bridges one of which was 910 feet long, Post Tensioned cast in place concrete structure,
built on a Soffit fill over 140 feet in height.
1991 Worked as Project Manager on Spring Mountain Road and I-15 Traffic interchange ($15 million
Nevada DOT) in Las Vegas, which involved removing 2 concrete bridges and constructing 4 new concrete
bridges and approximately 2 miles of new concrete pavement. This project had very high traffic environment.
1993 worked as Project Manager on Salt Lake City new runway project doing the grading and drainage
($25 million), which included dewatering, soil stabilization, importing 6 million tons of granular fill, miles of
drainage piping and electric conduits, and utility vaults.

1995 worked as Project Manager on an Arizona DOT project called Mariposa Road in Nogales Arizona
($10 million) This project included removing 2 freeway bridges, and constructing 2 new bridges on 1-19,
widening 2 existing bridges, 3 miles of reconstruction of 2 lane highway into a 4 lane highway. It also included
soil cement bank protection and gabions. The project is a major route into and from Mexico and involved
working with the Mexican and US Government Authorities as the highway connected to the border.
1996 worked as Project Manager on a Utah DOT project in Salt Lake City, 1-15 and
Woods Cross ($18 million). This project involved A+ B bidding with a $17,000 per day incentive/penalty of
which we received 24 days of incentive. This project reconstructed approximately 2 miles of I-15 and remove
and replace 4 concrete bridges. Also included reconstructing portions of 3 surface streets. This project was very
traffic intensive.
1999 worked as Project Manager constructing a Rest Area for Utah DOT on I-80 west of Salt Lake City
($3million). This project included constructing the Rest Room structures, Sewer system, Concrete parking lots,
Caretakers Residence, Rejuvenating an existing well, installing well pump storage tank and constructing a
complete potable water system.
January 2000 to September 1, 2000 worked as Project Manager on Nevada DOT project on US 95 in Las
Vegas ($5 million) widening 2 bridges, involved high traffic volumes. This project had a time incentive which
we were able to achieve the maximum bonus.
In the 19 years I worked for Meadow Valley Contractors I was very much involved in bidding on most of
these projects noted as well as many many others.
As Project Superintendent I was responsible for the operations in the field, duties involved hiring crews,
designing forms, ordering material and equipment, and comparing costs to budgets.
As Project Manager I was responsible for the entire project, duties involved corresponding with the
owners, scheduling, subcontract administration, material procurement, and hiring superintendents, establishing
budgets and tracking costs.
September of 2000 in an effort to work closer to home I began working for Telecor Inc. of Hildale, Utah.
I worked as Project Superintendent on Washington City Water Improvements, concrete water tank, and Enterprise
City Water Improvements, which included 2 pump houses and a chlorination building.
In October of 2000, along with some investors I organized JNJ Engineering Construction, Inc.
incorporated in the State of Arizona, and now licensed in Utah and Nevada.
March through June of 2001 I worked as Project Superintendent for JNJ on Marana High Plains Effluent
Recharge Facility. This was a Pima County project consisting of constructing 5 earthen ponds and a Pump
Station and associated piping and landscaping. The project was valued over $600,000.00.
July through September of 2001 I worked as Project Superintendent for JNJ on a project which replaced over
1500 water services for the city of Peoria, Arizona.
September of 2001 through May of2002 I worked as Project Superintendent for Steeds, Inc. On a
$3,000,000.00 Sewer project for Hildale City in Southern Utah. This project consisted of approximately 15 miles
of Sewer main outfall line collector lines, and force main and re-use line. We constructed an elevated sewer

crossing a major drainage. We also constructed a pump station and 90 acres of clay lined sewer lagoons with over
500,000 cubic yards of excavation. It also included Boring & Jacking across Utah State Highway 59.
May through October of 2002 I worked as project Superintendent for JNJ on the Northern Hills Mainline
Replacement, which was an $836,000 job for Metropolitan water District in Tucson. This project consisted of
Water mains, vaults, valves, water services, and landscaping.
From September o2002 through April of 2003I worked as project Superintendent for JNJ on an Arizona
Department of Transportation project in Douglas Arizona. This project consisted of Constructing a new multi-use
pathway along the west side of US 191B in the Town of Douglas from. 3rd street to 14th street. Along the new
pathway, there were constructed two ramadas, a box culvert extension, landscaping, Irrigation and lighting and
other incidental work. This project was valued at over $1, 522,000.00.
From March of2003 through the end of the year I worked as Project Superintendent on the Willard Street
I Mingus Drainage Improvement project for the City of Cottonwood Arizona which included 11 concrete box
culverts, relocation of existing sewer and water lines and installing several storm drain pipes, and the associated
asphalt replacement. This project was valued at approximately $1,100,000
From January 2004 through April I was involved as project superintendent in construction of Sidewalk,
Curb & Gutter and Storm Drains on Jordan Road for the City of Sedona, Arizona. This project was valued at
approximately $400,000.
In March of 2004 I worked as superintendent of the Colorado City Airport taxiway construction, about
$500,000 project consisting of grading, storm drains paving and installing taxiway lights and signs and pavement
markings.
In August of 2004 through March of 2005, I worked as project superintendent on the Cottonwood Old
Town Street Improvements, which consisted of installing curb, gutter, sidewalk, and streetlights through the
historic district of Cottonwood, Arizona. This project was valued at approximately $900,000.00.
Starting in August of 2005 I worked as Project Manager on the Las Vegas Springs
Preserve, Trails project for the Las Vegas Valley Water District, valued at
$2,600,000.00, which consisted of constructing trails, and 8 waysides, 1waysides with shade structures, and a
Ramada with composting restrooms powered by solar power, in an environmentally and historically sensitive
area of Las Vegas, Nevada.
From October 2000 I was actively involved in the management of JNJ Engineering Construction, Inc. as
well as being responsible for the referenced projects, and many others not noted here that had other
superintendents and project managers.
Since February 2013 I have been involved with Phaze Concrete in Project Management working in the
Civil Contracting division of the business.

Daniel Jessop
VP Concrete Division
Daniel has worked for several years in the contracting industry. He has managed crews, subcontractors,
dealing with schedule and material procurement. His experience includes the construction of roadways, box
culverts, drainage structures, dams and reservoirs, as well as all scopes of excavation and concrete surrounding
commercial and industrial building. Some of the projects were: Foxglen Park water line-Flagstaff, AZ City
Drainage Improvements-Lake Havasu, AZ Home Depot Excavation and Concrete-Kingman, AZ Short Creek

Daniel has managed production of many of the projects in the above list. With numerous qualities as an

Ernest Jessop
Project Manager
Ernest has worked for several years in the General and Civil Contracting industry. He was a foreman on a
2 mile long sewer line project in Heber City, UT. Then in 1997, he was a foreman on a sewer line project on
Brewer Rd and Palisade Dr in Sedona, AZ. He superintended the installation of all utility services in a 40 unit
subdivision for the BIA in Red Mesa, AZ. He also replaced an existing water mainline with a new ductal iron
water mains and new fire hydrants in a subdivision in Flagstaff, AZ. In Prescott, AZ, he supervised and laid out,
formed and poured culvert bridges, cast in place concrete flatwork, curb and gutter, and other structures within the
storm drainage scope. He has superintended dozens of road resurface or widening projects throughout the states of
Arizona and Texas. Ernest superintended all the concrete, utilities and earthwork scopes on Sams Club #4843 on
South Freeway in Pearland, TX, and, on WalMart #5799 on Speedway Blvd in Tucson AZ. He, also, was the
project manager for the CR 61 Road replacement project in Kersey, CO.
He has been involved with the Management of Safety, Compliance, and Inspections. Coordination with
the Corp of Engineers, Subcontractors, materials, and manpower. Management of the Contract, Submittals,
Material Resourcing, Production Schedule, Certified Payroll, and AIA Draw submission.
With his extensive experience as a tradesman, operator, supervisor and manager, we are confident in his

As a Superintendent, he has led our crews to complete several retail box stores, Dollar General, Aarons
Rental projects, etc. Our scope included site utilities, earthwork, storm drainage, building and site concrete
placement, and steel erection. Project sites are as follows: Kingsville, TX Lancaster, OH Urbana, OH San
Antonio, TX Pearland, TX Lawton, OK Brian, TX College Station, TX. He also superintended a
Walmart on Montana St in El Paso, TX with the site utilities, earthwork, storm drainage, and building and site
concrete placement scopes. He managed a couple concrete tilt-up projects, a Toys-R-Us in Houston, TX and a
warehouse in Hildale, UT, which included all concrete scopes.
His performance of the responsibilities as a Superintendent have shown us his outstanding ability to bring
his projects from contract to completion on time and under budget every time.

Thomas has worked for several years in the Civil Engineering and General Construction industry. We feel
that he is a professional and conscientious leader, well qualified to manage or superintend production on any
project we contract.
He superintended the excavation, drainage and utility placement for Providence Center overpass in Cedar
City, UT. He also worked on the excavation on the construction on the Virgin River Bridge in LaVerkin, UT. In a
section of downtown Salt Lake City, he supervised a crew in replacing all water, sewer, and storm drainage, along
with replacing the road, curbs and sidewalks.
In Lake Havasu City, AZ, he set thousands of feet mainline sewer pipe with over a thousand residential
connections and associated road and site work, and he superintended construction on a city water tank and the
recharge wells associated therewith. Constructed numerous ADOT Spec box culverts and associated road work on
an airport access road in Lake Havasu City and on public roads in Sedona, AZ.
He has superintended a rock and mass excavation, utiltities, and concrete project for the Bureau of Indian
Affairs in Dennehotso, AZ. He was a site manager on a highway reconstruction project in Weld County,
Colorado.
