Applications for Prequalification of the Consultants were invited for Planning

& Designing of the Improvement / Standardization of District Headquarter
Hospital, Nowshera through advertisement appearing in the National Press.

2)

We submitted our application for Prequalification vide our letter No.AEC/PR1/2003/2580 dated September 01, 2003.

3)

Our Company was Prequalified and asked to submit Technical & Financial
Proposal vide your letter No.20630/13-W dated 23-01-2003 including
Approach & Methodology giving special emphasis on:a. Understanding of the Objective.
b. Methodology
c. Work Plan & necessary Managing Schedule and its Presentation.
d. Innovative Idea

4)

We submitted our Technical & Financial Proposal vide our letter No.AEC/PR1/2003/2897 dated October 30, 2003.

5)

We deputed Mr. M. Rafiq, Senior Surveyor to carry out survey work of above
cited project vide our letter No.AEC/PR-1/2004/3750 dated February 28,
2004.

6)

Our Financial Proposal was accepted vide Client Award Letter No.517/20-M
dated 24-02-2004 and were advised for signing of agreement and
commencement of consultancy work.

7)

We deputed Mr. M Bashir, Lab Technician to carry out Soil Investigation

works of the above cited project vide our letter No.AEC/PR-1/2004/3808
dated 09-03-2004.

8)

In response to your letter No.21485/13-W dated 09-03-2004, we have agreed

to give rebate in our quoted consultancy fee and bring it down to your
administratively approved fee of 1.50% of the project vide our letter
No.AEC/Nowshera-1/2004/3831 dated March 15, 2004.

9)

We submitted Survey Plan, prepared Master Plan and proposed Master Plan
with Survey Plan vide our letter No.AEC/Nowshera-1/2004/3868 dated 1803-2004.

10)

We deputed our Civil Engineer Mr. Muhammad Ifikhar Ahmad for site visit

stipulated scheme vide client letter No.22152/7-M dated 01-06-2004.

We submitted seven (7) copies of PC-I for the above cited project after
incorporating all the observations of Executive District Officer (Health),
Nowshera (referred Project Director (B&R) letter No.22367/13-W dated 2606-2004 vide our letter No. AEC/Nowshera-1/2004/44469 dated June 28,
2004.

We were informed through letter from Project Director (B&R) No.22377/13W dated 29-06-2004 that present cost of the project is 935.631 Million
including the Phase-I (cost of the project for Boundary Wall / Gate and
Consultancy Services amounting to Rs. 7.715 million already approved) after
the correction and including the Equipment as per list supplied by Executive
District Officer (Health), Health Department, Nowshera.

23)

We received Project Director (B&R) letter No.22750/13-W dated 03-08-2004

through which we informed that application for Prequalification of
Contractors/Firms for subject work have called upto 24-08-2004, we were

asked for preparation of Contract Documents, Conditions of Contract, Bill of

Quantities and Evaluation of the Contractors/ Firms.

schedule / non-schedule item he also worked out as the payable premium will
not be on item rate but on single premium for schedule & non-schedule items
separately.

In response to your letter No.2996/13-W dated 13-03-2009 and No.3023 /13W of 17-03-2009 our Chief Engineer (North) submitted that the change in
specifications and scope of items / works was because our compulsion due to
change of new Code of NWFP, which cannot be compromised. We already
briefly explained this vide our letter No.AEC/Nowshera-1/2009/1371 dated
March 7, 2009. As regards the approval from Competent Authority the Deputy
Director is the Engineer Incharge of the project and has full power under
Clause 11.1 of the Contract Agreement to obtain necessary approval from
Competent Authority well before the commencement of the project.

dated Nov12, 2009. Rate analysis of steel grade 60 rolled deformed bars
ASTM 615 A for Rs. 83000/- submitted is considered to be cancelled. Letter

received from the ITTEHAD mills in response to our inquiry letter NO.AEC
DATED 11NOV.09 in respect above steel verifies for 89000/- is submitted.

To AL.Qamar traders letter no. AEC/CEN/PSH 08-09 dated dec15,2009. Visit

to project location and observation are motioned. AL.Qamar trader is directed
to remove the steel, sand and bricks form the site at once and replace it with
the new ones .improve the workmanship of steel reinforcement, shuttering,
plaster and brickwork. Improve the shuttering work the next roof i.e 1st foor .
submit up to date the steel ,bricks and concrete test report as soon as possible .

103)

To Ahamdullah construction company letter no. AEC/CEN/PSH 08-09 dated

dec15, 2009. Final notice was given and the work at t site was fully
abandoned and as per clause 17.1 of contact agreement which may cause the
termination of the contract agreement

To Wajid Iqbal Construction company letter no. AEC/CEN/PSH 08-09 dated

dec15, 2009. Observation that was made on project location dated Dec14,
2009 are stated and the instructions against the observation are stated.

investigation and height will not be more than 9-metres.

Method of safe disposal of waste: All such waste shall be incinerated

in a properly designed incinerator ,it will then dispose of with the help
of specially assigned vehicles and hand carts to landfill site .

Type of treatment plant either primary or secondary: treatment would

be of secondary type .

Safe disposal of waste and ashes produce after incineration: the

treated residues and incinerator ash will be ultimately disposed in
sanitary /secured landfill manually with the help of vehicles and hand
carts .

Public hearing date is proposed on 28/09/011as the advertisement has been
