FINANCIAL AID TERMS AND CONDITIONS
PLEASE READ THE FOLLOWING FINANCIAL AID TERMS AND CONDITIONS. SIGN
AND DATE THIS FORM, KEEP ONE COPY FOR YOUR RECORDS, AND RETURN THE
COMPLETED FORM TO THE FINANCIAL AID OFFICE WITH A COPY OF YOUR SIGNED
AWARD LETTER.
I accept the financial aid that has been offered to me for the 2016-2017 academic year. In accepting
this offer of financial aid, I agree to the following:
1. I understand that, unless otherwise specified in my award letter, my eligibility for scholarship
and/or financial aid is based on full-time enrollment for both the fall and spring semesters. I
understand that less than full time enrollment may result in a reduction of my federal financial aid,
and cancellation of my Conservatory scholarship award.
2. I understand that scholarship/grant/loan/student employment awards for the academic year will
be disbursed to me in two halves, one-half in the fall semester and one-half for the spring semester.
3. I hereby grant permission to the Conservatory to release relevant personal, academic, and
financial information important to obtaining or continuing my financial aid. In return, I expect that
the Conservatory will respect my rights to privacy and release information only as necessary.
4. If I have been awarded Conservatory scholarship assistance, I agree that I may be required
to attend and/or perform at the annual Scholarship Recipient Concert and Reception. I also
agree to notify the Financial Aid Office of all scholarships/prizes that I receive from sources other
than the Conservatory.
5. I understand that I must maintain the minimum required grade point average (2.5 for
undergraduates and 3.0 for graduates), pass my annual jury and participate in orchestra and
any ensembles to which I am assigned in order to continue to be eligible for a Conservatory
scholarship. I understand that I must maintain SATISFACTORY PROGRESS in my course
of study, according to the standards and practices of the Conservatory, in order to continue
my eligibility for all federal aid funds. (Refer to the Conservatory catalog under
Responsibilities of Financial Aid Students regarding satisfactory progress.)

6. If I accept a Conservatory Scholarship from the San Francisco Conservatory of Music, I agree not
to consider any other financial aid offer from an institution member of the National Association of
Schools of Music for the 2016-2017 academic year, except with the written consent of the Dean of
the San Francisco Conservatory of Music.
7. If I have been awarded Federal Work-Study or Student Employment, this indicates that I
will receive priority for part-time work at/through the Conservatory. If I am interested in this
option, I understand that I need to contact the Conservatorys Human Resources Office and
express my interest in student employment during the first 2 weeks of the fall semester. After
that time, I understand that I will be treated as all other Conservatory students regarding
student employment.
8. Perkins and the Direct Stafford Subsidized Loans are excellent federal loans for paying part of
Conservatory expenses, as a full time student pays no interest or principal for these loans while
enrolled. Both loans are awarded on the basis of financial need as demonstrated on your FAFSA.
The Perkins Loan comes from a limited pool of money and must be awarded under federal
guidelines. The Federal Direct Stafford Loan program has significant resources but is limited to the
following maximum amounts:
Freshman
- $3,500
Sophomore - $4,500

Junior - $5,500
Senior - $5,500

I understand my eligibility for these loans. If I need additional resources, I understand that the
following loan options are available, depending upon my status: Additional Direct Unsubsidized
Loan Funds of $2,000 for all undergraduate students and the Direct Parent Plus Loan for Parents of
Undergraduate Students (PLUS).
Graduate students are eligible for the Direct Stafford Unsubsidized Loan of $20,500 per year and a
Graduate Plus Loan for any remaining need.
For International Students, there is the International Student Loan (which requires a co-signer who
is a US citizen or permanent resident). I understand that I need to contact the Conservatorys
Director of Financial Aid if I am interested in learning more information about additional loan
options.
If you have been awarded a loan and this is your first Federal Direct Student Loan please visit:
www.studentloans.gov in order to complete a new MPN and an entrance loan counseling
session. If you are a parent or a graduate student applying for a Plus loan, please complete the
attached loan application form and visit the www.studentloans.gov website to complete the
necessary information per the directions. Please see the attached Direct Loan informational sheet.
