PRELIMINARY OBSERVATION REPORT ON RESUMPTION


DATE OF RESUMPTION: 26TH OCT 2015
SITE: NDDC/ NDU HOSTEL AMASSOSMA
SCOPE OF WORK: CONCRETE WORK ON 2 BAYS
PRELIMINARY OBSERVATION
1. Two bays have been casted and entire soffit formwork removed but wood were not properly sorted out and
well arranged
2. Site had been cleared of all grasses and shrubs
3. Materials on site as displayed by the materials chart
4. Workers yet to start work, complained of money but made them understand that their monies had been paid
after making enquiries with the head of audit (Mr George)
5. Site plant (concrete mixer)not in good working condition, needs to be fixed
6. Reinforcement rods were in a ditch of water and needed to be taken to a dry area close to where they will be
used.
7. The office caravan needs to be roofed
8. The recovered plywood from the previous work were largely not in good condition
9. Cement store is gradually depressing into the ground
Actions taken

1. Encourage the workers to begin work the next day as their monies have been paid and assured them they
will not be owed
2. Demanded to see and schedule meeting with all labour leads
3. Employed workers to transfer the reinforcement steel to a dry place and close to where they will be needed
to avoid rust

Recommendations
1. Repair of concrete mixers as they will be used for the concrete work to avoid hiring
2. Additional set of carpenters and iron bender be employed/ engaged to speed up the work and commence
casting on time b/cos of the condition of the cement
3. The slab platform/ formwork be coated with used engine oil to ease subsequent removal and forestalling
damages
4. Site be cleared of all debris inclusive of all damaged wood with nails for safety reasons
