Intervention Quality Check

When completing the digital form, we are expecting all teachers/interventionists to score the intervention
session and the progress of the student for each session. This score will be on a scale of 0-5. Each number
assignment will correlate to both a mastery level of a student, and the instructional approach that student
needs. Please see the scale below for clarification.
0 Student has no mastery. Prior skills are required to be taught before the TEK itself can be addressed.
1 Student has below 20% mastery. TEK should be taught as if it is being introduced to the student for the
first time.
2 Student has lower than 40% mastery. TEK skills should be retaught and guided practice opportunity
provided.
3 Student has lower than 60% mastery. TEK skills MAY need to be retaught, guided and independent
practice should be provided.
4 Student has lower than 80% mastery. Independent practice with teacher review should be provided.
5 Student has greater than 80% mastery. Independent practice and rigor may be increased. Student may
be exited from intervention for this TEK
