1. INTRODUCTION TO SAFETY
The aim of monitoring safety on site is to avoid injuries arising from
construction activities.
Generally many company has prepared the Safety Plan . This an will be
implemented in order to protect all personnel at site minimize the risk of accidents and incidents
that could result in injury.
In terms of our Safety Plan, it needs all levels of implementation and coordination in order to mitigate against risk and assure that adequate standards safety are
maintained throughout the duration of the project.
It will be a priority in all daily activities, undertakings and endeavors to
ensure the safety of workers, the public and other persons directly or indirectly associated in this
project. This safety measures will be continuously implemented throughout the duration of the
project.

RESPONSIBILITIES OF THE OFFICERS ABOUT SAFETY


Project Manager :

Overall responsibility for all health and safety matters on site.

Implement the safety Management system.

Ensure that all line management is conversant with the relevant requirements of current
legislation and of the Safety Plan and that all managers are assigned duties and
responsibilities to assist in its effective implementation.

Appoint a Safety Officer who shall be responsible to him.

Safety Officer

Is accountable to the Project Manager / Site Manager for the promotion of health and
safety systems and monitoring all site safety.

Ensure the safety plan is being correctly managed by Site Management and
Subcontractors.

To write an maintain reports on site inspections, audits and accident investigation.

To develop and maintain site safety training program.

To carry out regular liaison with Clients and Sub-contractors on safety matters..

To advise an promote safety and health on site by various means and in consultation with
the Site Manager.

Shall where necessary on behalf of the Project Manager to stop. works, which he
considers to be creating a hazard and risk.

To organize the safety proposals.

To keep records on safety.

CV of Safety Officer attached for review.

Site Managers, Adm. Officer, Engineers

Support to the Safety Officer and responsible for organizing works at site incompliance
to the required safety standard.

All Personnel

Shall take reasonable care on their own safety as well as that of others. who may be
affected by their actions or objectives at work.

Required to corporate with the management to achieve safety standard.

Encourage to proposed suggestions to improve health and safety at site to their immediate
supervisor or Engineer or the Safety Officer.

Safe Working Procedures

Safety working procedures are written to ensure that site activities are properly controlled
and executed safety and without risk to health.

The safe working procedures, which may be incorporated method statements shall be
prepared and submitted for approval prior to commencement of activity.

All safety working procedures shall be clearly identified the objectives, the sequence of
operations the foreseeable hazards and risks and the precautionary and protective
measures required and be easily understood by the personnel who are to carry out the
work. Procedures are to be included the control measures to reduce risks identified and
methods of monitoring to ensure the control measures to reduce the risks identified,
methods of monitoring to ensure the controls are applied.

Permission to Work

Where permits to work are required for specific operations such as entry into confined
spaces, work in explosive of flammable atmosphere and special lifting operations shall be
obtained approval by the Project Manager or persons authorized by him.

The Site Manager shall ensure that the issue of permits to work, and the work conducted
under them is strictly controlled. Only competent person will authorize such permits.

Security Clothing

Safety Helmets, Shoes, Glues etc.

ID passes

Controlled Site

Fully fenced

Only authorized person on site, who will be readily identifiable by

ID Passes.

Safety Signs
Safety Sign boards are demonstrate at site and some pictures are attached.
Safety Lectures

New starters will be given introduction by Safety Officer.

Follow up lectures every month.

If any incidents, workers will be called aside or lecture & discussion.

Record Keeping
Daily diary - incidents are reported to SM. Following formats are used for recording.

SITE CLEANING

Site shall be always kept clean.

Dustbins shall be provided at appropriate interval for small garbage with indication by
sign board.

Garbage dumping area shall be arranged at site with indication by sign board

All garbage shall be collected to this area, and collected garbage shall be disposed to
outside of site periodically.

A clean site is a safe site.

PROTECTIVE WEAR

Helmet
All workers/staff shall wear helmet at site.

Shoes
Bare foot, Sandals and slippers are prohibited at site.

Other Protective Wear


o Safety belt shall be worn for following works
- Structural steel erection, scaffolding erection, etc. at high place
o Goggles shall be worn for following works
- Chipping, Grinding, etc. in case small particles could be emitted .

o Welding hoods shall be worn for following works


- Welding, etc. in case harmful ray could be emitted.
o Mask shall be worn for following works
- Grinding/cutting concrete, etc. in case small particles could be emitted.

Work Clothe
Work clothes shall be in suitable size.
Short trousers and sarons are prohibited at site.

SAFETY ACCESS

Safety access to work place shall be always secured and cleaned.

Signs for safety access shall be provided.

Lighting o safety access shall be provided at night.

SCAFFOLDING/ PLATFORM

Damaged, deformed, corroded materials shall not be used for scaffoldings

Scaffolding shall be erected in either of following systems


o Prefabricated scaffolding system
Consisting of frame, brace, catwalk panel, etc.
o G.l. Pipe scaffolding
Consisting of vertical; horizontal, bracing G.i. Pilpes tightened

By cramps.
Scaffolding which is more than 5m in height shall be tied to permanent structure at
appropriate interval,

Top of scaffolding/ platform shall be equipped with handrail

The perimeter of the building will be fully scaffolding while windows


are being in installed, wall finishes applied & the roof installed, to
protect workers & allow safe access to carry out work.

PREVENTION OF ACCIDENTS DUE TO FALLS

In case of workers at heights safe scaffolding/platform shall be provided.

In case it is physically difficult to provide safe scaffolding/ plat form for works at heights,
workers must use safety belt.

Opening which may cause fall shall be closed, or barriered.

Work at heights in strong wind and heavy rain is prohibited.

In case height /depth of working platform is more than 1 .5m, safe

facilities for going up and down shall be provided.

PREVENTION OF ACCIDENTS DUE TO ELECTRICAL FAILURE

Insulated wire shall be used for wiring of temporary power supply.

Nothing shall be put on cables.

Nothing shall be put in front of distribution boards.

Circuit breaker shall be provided to each circuit.

PREVENTION OF ACCIDENTS DUE TO MISUSE OF MACHINERIES

Max. speed within the site shall be 10km/h.

No entry into the radius of operation of crane, backhoe, etc. is allowed.

Inspection before commencement of work for basic mechanical items such as brake and
clutch shall be carried out by operators.

Periodical inspection for entire machineries shall be established (detail attached to


Environmental Plan) and in effect.

Signals for hoisting shall be unified, and signal man shall be stationed at hoisting works.

PREVENTION OF FIRE

Burning surplus materials within the site is prohibited.

Smoking area shall be provided with indication by signboard and equipped with ashtrays.
Smoking is allowed only at smoking area.

Fire extinguish and bucket filled with sand shall be provided at appropriate interval with
indication by signboard.

Flammable gas cylinder shall be kept at well-ventilated place.

SAFETY SIGNS
Following signs shall be provided at appropriate places according to the site
situation/ progress.
1. Safety first
2. Wear Helmet
3. Watch your head
4. Safety access
5. No smoking
6. Smoking area
7. Fire extinguisher
8. Dustbin
9. Other signs as required

FIRST AID

A first aid base shall be provided at our Site Office. An adequate number of first aid
boxes shall be provided at various work locations.

The first aid base shall be attended with a qualified first aid staff. The first aid base shall
be for the use of Contractors all personnel employed, Engineer staff and Employer
Contractors that needs immediate first aid treatment and other personnel directly or
indirectly concerned with the project that requires first aid treatment.

The first aid base shall be have suitable sign displayed to indicate the location. The sign
shall be in English Indonesian an local dialect.
The first base shall be equipped with the following, as a minimum:

A clean water supply, suitable for drinking

A lockable cupboard to contain sufficient medical supplies

Medical supplies at least to the standard specified Safety Regulation.

A stretcher

Blankets

ACTIVITIES FOR SAFETY

SAFETY INSPECTIONS

The objective of safety inspections is to verify whether the safety management objectives
of the worksite are being met.

The Company should establish procedures to carry out internal safety inspections.

The inspections may be informal such as spot checks on activities, or formal, where
documented procedures are followed.

Personnel carrying out inspections should be competent to do so and should be fully


conversant with any relevant procedures for safe work practices, worksite rules and
regulations and statutory requirements.

The results inspections should be brought to the attention of the Safety Officer and
personnel having responsibility in the area concerned. Any corrective action should be
immediately implemented.

The Company should ensure that inspections are carried out at a specified frequency,
which will ensure high level of compliance with the provisions of the Safety
Management System.

Inspections, which a e a statutory requirement, must be carried out at a frequency and in


accordance with any procedure given in the said requirement. Other inspections should
be carried out suitable frequency and thoroughness commensurate with the dynamic and
rapid changes of the worksite. Inspection of shoring of formwork excavation, trenches,
cranes and scaffolds should be carried out after inclement weather.

Site Safety Inspection

Site Safety officer must conduct a site inspection at least once a week, using checklist.

A copy of the inspection report should be given to Site Foremen and Sub-contract
concerned.

A copy of the report will be directed to the Project Manager.

Site Safety officer shall monitor and follow-up to check if unsafe acts or conditions
rectified by if deadline. A summary of non-compliance should be submitted to Project
Manager if unsafe acts or conditions are not rectified.

Daily Safety Inspections

Full-time staff including Project Manager should conduct safety inspection


in their daily round. Observations on safety should be noted down on a prescribed form to
facilitate follow-up.

A copy of those observations should be given to the Site Safety Officer;

Non-action on the part of sub-contractors should be noted, and discussed at


Safety Meeting.

MAINTENANCE REGIME

The Objective of maintenance regimes is to ensure that plants, machinery and equipment
in the worksite do not present any safety hazard due to lack of repair aid maintenance.

The Company should implement a preventive maintenance programme and establish


procedures to ensure their safe and efficient operation. Where appropriate, all safeguards
and devices to protect workers should be fitted and maintained in an efficient working
order. The Company should identify the statutory maintenance and testing requirements
of all Lifting Equipment and Pressure Vessels. The maintenance and testing of the same
should be included in the preventive maintenance programme.

To fulfill the requirement of the foregoing, the worksite should ensure that:

Inspections of all equipment, plants and machinery are held at


predetermined and regular intervals

Defects and deficiencies identified are reported and reported to the


appropriate authority.

Corrective action is taken in an effective and timely manner.

The maintenance programme should also include plants, machinery and equipment of all
sub contractors.

The inspections and measures mentioned in paragraph 9,4 should be integrated into the
worksite preventive maintenance programme.

If outside agents are engaged to carry out maintenance and repair, it is necessary to
control and supervise their activity to ensure they conform to the agreed specifications

and maintenance contract. All equipment so maintained should be inspection and checked
before being returning for service.

Maintenance
a)

Safety Supervisor should compile list of equipment, machineries, and major tools

b)

These will be monitored for test certificates, COC, and compliance to schedule of test,
servicing and inspection.

c)

A preventive maintenance programme should be prepared:


1)

In the case of leased equipment, purchaser should ensure that leasor provide the
regular tests services, and maintenance as provided for in the contract.

2)

In the case of assets owned by main contractor, mechanic should ensure


compliance with schedule of tests, inspections and servicing.

d)

All records should be kept, and should include:

Test Certificate.

3)

Check lists used for inspections

4)

List of supplier of services

HAZARD ANALYSIS

The objective of hazards analysis is to provide a means whereby hazards or potential


hazards are identified, evaluated and managed in a way that eliminates or reduces to an
acceptable level, the risk of an incident occurring.

The Company should establish and maintain a programme for the identification and
assessment of hazards.

This programme should aimed at:


a) recording of known hazards;
b) identification of new hazards;
c) analysis of the effects or the potential effects, resulting from these hazards;
d) development and implementation of means to eliminate the hazard or to manage the
hazard in a way t at represents on acceptably low risk of incident.

The Company should ensure that persons responsible for the analysis of hazards and for
determining the means of eliminating or reducing any safety risks, are:
a) Technically competent;
b) Given the necessary support so that they can effectively perform their duties.

Analysis

a) Project Manager should ensure that sub-contractors submit method statement.


b) Sub-contractors should submit method statement and list of safety measures to the
main contractor before starting work.
C) Construction Engineer should review method statement and the proposed safety
measures.
d) Site Safety Officer/ Safety Supervisor should review sub-contractors proposals on
safety to ensure compliance with legal requirements.
e) Chairman of Safety Committee should remind sub-contractor of critical safety points
in meetings.

f) Safety Review
The Safety Review technique is a detailed inspect to identify hazardous characteristics,
job -site conditions, or maintenance activities. Using the Safety Review technique to
conduct periodic inspections of a job site helps ensure that implemented risk management
programs meet original expectations and standards.
Safety review consists of three steps:
1)

Preparing or the review

2)

Performing the review

3)

Documenting the results

Step 1: Preparing for the Review


1) The Project Manager shall define the system, method statement, construction
practice and personnel to be evaluated. The Project Manager shall form Safety
review Team and the team shall have sufficient number of reviewers to cover e
necessary construction areas and operation in the available time The members of
the review team should have diverse backgrounds an responsibilities to promote a
comprehensive balanced study.

2) The Project Manager designate shall gather a detailed description of the job site
(eg: site plans, construction drawings) and the job site safety procedures.
3) The Safety Review Team shall review the known hazards. Checklist should be
prepare wherever necessary.
4) The Safety Review Team shall also review the applicable codes, standard and
company safety management system.
5) The Safety Review Team shall then schedule interviews with specific
individual responsible for safe construction operation.
6) Where necessary, the team may request for records concerning personnel
injuries, accident/ incident reports, equipment inspection, safety/health audit.

Step 2: Performing the Review


1) The Safety Review Team shall start the review by a general tour of the job site
They shall then progress to specific inspections and interviews.
2) The inspection shall include checking the compliance of the Staff to the
companys safety manual, procedures and instructions.
3) Checklist which is prepared during the preparation stage, shall be used to help
in :he review

Step 3: Documenting the Results


1) After the review, the Safety Review Team shall compile a review report.
2) The report shall contain at least the areas of concerned and recommendation
action to be taken
3) The review team leader shall sign on the report end a copy of the report shall be
submitted to the Safety Committee.
4) The review team shall perform follow-up check to ensure that the corrective
actions taken are duly completed.

HAZARDOUS MATERIAL

The objective is to provide a means whereby the receipt, movement, storage and use of
hazardous material is managed in a way that eliminates, or reduces to an acceptable level,
the risk of an incident occurring.

Every worksite should maintain a register of hazardous material complied from Material
Safety Data Sheets (MSDS) or from other analysis.

The register should show.


a) The composition , physical and chemical characteristics of the material;
b) Safeguards to e applied in the transportation, storage, handling and use of occurring in
the handling or use of the material.

The Company should insure that there is controlled access of hazardous material to the
worksite premises sufficient to ensure:
a) that the movement of hazardous material is known and recorded;

b) that a technically competent person can receive the material and ensure its safe storage
or distribution;
c) that the material is correctly labeled;

Designated storage areas for hazardous material should be appropriate for the material
and should be secure against unauthorized access.

Hazardous material should be returned to designated storage areas when not in use.

Controlled of chemical and hazardous material


a) The control procedure shall apply to. degreaser, cleaning solvents, chemicals powder,
past , or liquid form, acids, alkalis, and other corrosive chemicals.
b) Sub-contractors should inform site Safety Supervisor before bringing in corrosive
chemicals. He should submit the following information

Relevant safety signs

c) Site Safety Supervisor should inspect the storage site, and examine the nature quantity
of chemical before allowing storage at site.
d) Site Safety Supervisor should ensure that appropriate signs, posters are in place.
e) User shall complete the chemical list form provided for all chemical used in the
worksite. Chemical include the following items:
I. Acids and Alkalis
II. Organic Chemical Solvents
III. Chemical Cleaning Agents and Degreasers
IV. Lubricants
V. Any chemical in powder, paste or other solid form
VI. Industrial Gases
VII. Any other chemical additives used in the work processes
