Instructions:
AS YOU SEE, HERE ARE CHIPS OF DIFFERENT SIZES, SHAPES, AND COLORS; LARGE
AND SMALL (point), CIRCLES AND SQUARES (point), YELLOW, WHITE, RED, BLACK, AND
GREEN (point). I SHALL TELL YOU TO DO DIFFERENT THINGS WITH THEM. IF I TELL
YOU TO POINT TO ONE, JUST POINT TO IT; IF I TELL YOU TO PICK ONE UP, YOU JUST
PICK IT UP (examiner demonstrates while speaking); AND SO ON. DO YOU UNDERSTAND?
If there are no questions proceed with #1 of Form A, on page 2.

If the patient fails to initiate a response after 5 or if the response is incorrect, return the
tokens to their original order and say, LETS TRY THAT AGAIN. You may do this only
once.
If they request more instruct them to do as much as they can remember. If they touch an
item instead of pointing for example, say, LETS TRY THAT AGAIN and give credit if
correct. You may do this only once. [Patients must follow the instructions exactly.]

Make sure they use their finger to point to the tokens.

Score 2 points for each correct response on the first trial. Score 1 point for
SCORING:
success on the second trial. There are no self-corrections.
