Upon Request of the above mentioned Owner, the undersigned inspection engineer subjected to general terms and
interventions of Lloyds Inspection Agency, has witnessed the following operations as mentioned below.

b) Witness operational & functional tests of the equipment.
c) Review of calibration/test reports & other documents from the manufacturer.
d) Issue of certificate of thorough examination of the equipment.

a) Visual examination of the Tower Light equipment found satisfactory.
b) Operational & functional tests of the Tower Light found satisfactory.
c) Review of calibration / test results & documents found satisfactory.
VI. COMMENTS / REMARKS:
The above mentioned Tower Light equipment was found satisfactory as far as could be seen.
The above mentioned Tower Light equipment to be used as per manufacturers recommendations.
VII. VALIDITY: This document is valid for one year from the date of inspection.
VIII. CLAUSES 1:1 COMMENTS & TERMS OF USE OF THIS REPORT:
1) This report doesnot cover any pressure testing procedures, facilities, designs or any other parameters or any operating instructions and only restricted to visual inspection & witness tests only.
2) This report does not give approval or any authenticity of the total equipment capacity mentioned by equipments steel tags or claimed by the equipment owner's/or by contractors/suppliers.
3) This report issued without prejudice for the ends and purpose it is intended for use by the equipment and issued without any liabilities by the Lloyds Inspection Agency Pvt. Ltd Company.
4) The responsibilities and liabilities of this above mentioned equipments and operator in use of this equipments for any accidents / incidents always will be by the equipment owner and the operators himself.
5) This report cannot be taken as record of evidence of passing of this equipment nor the qualification of the operator in use against us, in case of any accidents/ incidents in work site or in any location in Kuwait.
