Trip and Alarm Change Procedure

1 Introduction

The instrument trip & alarm settings are critical elements in the integrity of our facilities.
Following a review it was found that the trip & alarm settings shown on the Process &
Instrument Drawings (P&IDs) are inaccurate or are not shown at all hence it was decided to
record and maintain the settings in the Companys Maintenance Management System,
(MMS), with change control being the subject of a separate procedure. This procedure
replaces the Plant Modification Certificate (PMC), system for trip setting change control and
retains the principal of Design Authority approval and control of changes to key design
data.

2 Objective

The objective of this procedure is to provide a systematic, consistent and expedient


approach for change control of current trip and alarm settings for all of SUCOs production
facilities.

3 Applicability

This procedure is applicable to all SUCO production facilities and covers changes to both
Trip settings, which require the approval of the Design Authority; and Alarm settings, which
require only the approval of Operations Management.

4 Responsibilities

Area Field Manager

It is the responsibility of the Area Field Manager to ensure that the trip and alarm setting
changes are executed in accordance with this procedure; that alarm settings remain within
the operational limits, and that any deviations to this procedure are managed in accordance
with the Deviation Control procedure.

Instrument Section Head

It is the responsibility of the Instrument Section Head, to co-ordinate the field


administration, verification of as-built status, execution and recording in the MMS of
trip and alarm setting changes in accordance with this procedure.

Engineering Department

It is the responsibility of the Engineering Department to ensure that proposed trip setting
changes remain within the design limits; approve or reject proposed changes, and
maintain the Master Trip Setting database.

Operations Maintenance Department O-10

It is the responsibility of the Instrument Maintenance Support Section, (O-10), to verify and
ensure that the trip and alarm setting data is managed in accordance with this procedure; that
the MMS is maintained as the principal source of Operations Trip and Alarm Setting data
and to audit the process.

Step A1 - Production Manager

Changes to Trip & Alarm settings are usually made to improve the performance of
plant and equipment. Production Section Head will initiate the change and complete
Part A of the Trip & Alarm Change Form, (Appendix 1), indicating if it is a change to
either a trip or an alarm setting.

The initiator shall ensure that there is sufficient information in the Reason for Change and
Safety Considerations sections so that subsequent clarification by the review parties is
minimized, and that the impact of the change to Technical Integrity has been taken into
account. The initiator completes the requested box and forwards to the Area Section Head
Instrument Engineer.

Step A2 Instrument Area Section Head

The Instrument Section Head will register the form in the Area administration system, check
the request for accuracy and completeness, and forward the form to the Area Field Manager.

Step A3 Area Field Manager

If an alarm setting change is proposed, the Area Field Manager completes the Approval
section and returns the form to the Instrument Section Head for implementation. If it is a
change to a trip setting, the Area Field Manager completes the Support section and returns
the form to the Instrument Section Head for dispatch to O-10. If the proposal is rejected it
is returned to the Area Instrument Section Head who shall cancel the request and advise the
initiator.

Step 4 - Instrument Section Head

The Instrument Section Head either:


forwards the form to O-10 for review, if it is a change to a trip setting, or
implements the work (C7) if it is a change to an alarm setting.

Step B5 Register & Review O-10

O-10 will register the form, check the content and basis for change then forward to
Engineering (P-1), for review and approval.

Step B6 Review & Approval Engineering

P-1 will co-ordinate the engineering review of the change and ensures that the respective
discipline engineer(s) ensure that design integrity is maintained. If satisfied, P-1 will
approve the form and return to the Area Instrument Section Head for implementation. If
the form is not approved, it shall be returned, along with the reasons for rejection or a
proposal on alternative settings, to the Area Instrument Section Head who will inform the
initiator to either cancel the request or raise an alternative proposal.

Step C7 Implementation by Instrument Section

The Instrument Section Head shall implement the change using the maintenance work
management system (W.O.) for recording the work. On completion, Part C shall be
completed to confirm instrument technical data, and the form returned to O-10.

Step C8 Confirm Completion O-10

O-10 will confirm that implementation is complete and ensure that all necessary information
requiring update is available Trip setting changes will be forwarded to P-1 for updating the
Master Trip Setting database.
The Master Trip Setting database will be validated as part of SUCOs as-built data base.

Step C9 Update Master Trip Setting database

P-1 shall ensure that the Master Trip Setting database is updated. The Area Instrument
section head will confirm that the updating is correct and consistent with the MMS.

Step C10 Update MMS

O-10 shall ensure that the MMS is updated via the Instrument Section Head with the correct
Trip & Alarm Setting data.

6 Administration

Maintenance Management System

The Master Trip Setting database shall be held and maintained by the Engineering
department. It shall be available to other users, e.g. Area Instrument section heads and
Area production section heads, either by read only access to the system, e.g. shared folder
on the network, or by requested reports in Excel or Access formats.
The MMS shall be the principal source of Trip and Alarm settings for Operations field
users. The data shall only be changed following approved implementation and only by the
system administrators.

Routing

The Trip & Alarm Change Form can be generated by copy of the form in Appendix 1 of this
document and be routed either by normal paper mail or by fax. The Trip and Alarm Change
Form may also be downloaded and used as a template for use in E-mail communications.

MMS Audit

Periodically, O-10 shall audit the MMS to verify the Trip and Alarm settings against the
Master Trip Setting database.
