MAJOR LEAGUE BASEBALL PLAYERS BENEFIT PLAN. SUMMARY OF MATERIAL MODIFICATIONS March 2017 ‘The Office of the Commissioner of Major League Baseball and the Major League Baseball Players Association agreed to changes to the Major League Baseball Players Benefit Plan as part of their 2016 negotiations. This is a summary of the material modifications that will be effective April 1, 2017. This summary is intended to amend and supplement the Summary Plan Description which was previously distributed to you. You should take the time to read this summary, and keep it with your copy of the Summary Plan Description. All capitalized terms not otherwise defined in this summary shall have the same meaning as they have in the ‘Summary Plan Description. To the extent there is any inconsistency between the terms of this summary and the Plan document, the terms of the Plan document will control. |. Major League Baseball Players Pension Plan — Plan Changes The following changes will be made to the Major League Baseball Players Pension Plan ("Pension Plan”) effective April 1, 2017. ‘A. Pension Benefits for Classes VI and VII Benefit Increases for Classes VI and Vi. Effective with benefits paid on and after April 1, 2017, fixed benefits on Tables 1 and 3 for Classes Vi and VII, respectively, will be increased 25%, and the maximum benefit payable will be increased to 75% of the IRS maximum benefit, as periodically adjusted by the Secretary of the Treasury. Effective for benefits paid on and after April 1, the maximum benefit for Classes VI and VII will be $161,250, which is 75% of the current IRS limit of $215,000. Effective with benefits paid on and after April 1, 2017, widows of Members in Classes VI and VII who died prior to April 1, 2017, will receive monthly benefits equal to 110% of the monthly amount they were entitled to receive as of March 31, 2017. Pension Benefits for Class VIII Benefit Increases for Class Vill. Effective with benefits paid on and after April 1, 2017, fixed benefits on Table 5 (compensation $68,212 or less) for Class VIII will be increased 25%. Effective with benefits paid on and after April 1, 2017, widows of Members in Class Vill who were receiving fixed benefits under Table 5 and died prior to April 1, 2017, will receive monthly benefits equal to 110% of the monthly amount they were entitled to receive as of March 31, 2017. Disability Benefits Disability Benefits. Effective April 1, 2017, Disability Benefits will be increased to $6,000 per month for Active Members, Members who become disabled within five (5) years after active service ended, and Members with a disability resulting directly from active service. For all other eligible Members, the monthly maximum is increased to $6,000 per month. Effective April 1, 2017, the definition of “Total and Permanent Disability” or “Totally and Permanently Disabled” will be revised, The new definition will be a “personal disablement resulting from physical and mental illness or injury (excluding disability which: (a) was contracted, suffered, or incurred while the Member was engaged in a criminal enterprise, or (b) resulted from his current addiction to alcohol or narcotics, or (c) resulted from an intentional self-inflicted Injury) which either in the opinion of the Benefit Page 3 of 6 Plan Office, or as determined by the Social Security Administration, will continuously and wholly prevent the Member, for at least five years, from engaging in any occupation or performing any work for substantial wage or profit.” Major League Baseball Players Welfare Plan — Plan Changes ‘The following changes to the Major League Baseball Players Welfare Plan ("Welfare Pian”) will be effective April 1, 2017. These changes will be described in more detail in the Plan and Summary Plan Description. Please contact the Benefit Plan Office at (800) 669-2255 with questions about these changes. A. Health Benefits Under the Active Plan, the Buy-Up Plan, the Base Plan, and the Buy-Down Plan for All Active and Inactive Members and Their Eligible Dependents (except as otherwise noted) Physical Examinations and Preventive Care, For Members and Dependents 17 years of age and older, the Plan will cover annual routine physicals along with the following services when performed in conjunction with the physical examination for screening purposes on the same day as the office visit or within 30 days (before or after) of the physical: laboratory services for kidney, liver, and thyroid function tests; a chest x. Fay; an EKG or ECG; a fecal occult blood test for individuals age 50 and older; and other items or services that have in effect a rating of A or B in the current recommendations of the United States Preventive Services Task Force with respect to the individual involved, including well woman preventive care. Additionally, the Plan will cover prescription birth control at 100% and breast pumps at 100%. The Plan will cover tubal ligation and vasectomies under the applicable terms of the Plan for hospitalization and surgical benefits. Occupational Therapy. The Plan will cover occupational therapy for Injuries and Illness outside of a hospital stay, consistent with the provisions of Plan section 14.5(d)(vili) and section 15.6(g), as applicable Autism. The Plan will expand the coverage of Applied Behavior Analysis (ABA) programs to include those Programs designed and supervised by qualified professionals, which include either licensed clinical Psychologists with training in ABA or behavior analysts, who are board certified with supervised experience Providing ABA treatment for autism or who can clearly document equivalent training and experience. Qrthotic Devices. The Plan will increase coverage for orthotic devices from $350 per Plan Year to $550 per Plan Year. Access to Teladoc. Members living in the United States will have access to the Aetna Teladoc network of board-certified, licensed physicians who can provide consultations for non-urgent care needs such as respiratory infections, ear infections, pink eye, allergies, colds and flu by telephone as well as online or via mobile application. Claims for Teladoc consultations will be billed to Members in the same manner as any other professional service performed by a Physician in connection with the: diagnosis and treatment of Illness or Injury under section 14.5(d)(i) or 15.6(a), as applicable. Additional information on this program will be provided in a separate mailing, Page 4 of 6 Prescription Drug Benefits. Automatic Generic Drug Substitution Program. The Plan will implement an automatic generic substitution program through ESI, subject to clinically-based exceptions. All branded drugs for which there is generic substitute willbe filled automatically with the generic equivalent. However, a Member may receive a non-generic drug if the Member pays the full difference between the price of the generic drug and the non-generic drug Cholesterol Care Program. The Plan will implement a cholesterol care program that will require prior authorization of cholesterol lowering drugs known as PCSK9 inhibitors, specialist pharmacy use, and limitations on the days’ supply. Exclusion of Certain Drugs. The Plan will exclude the following drugs subject to clinically-based exceptions and exceptions noted below: © Solodyn ER © Duexis * Jublia (not excluded for Active Players) © Diclegis DR * Vimovo DR + Pennsaid * Nexium DR + Solaice © Relyyks + Kerydin © Sprix * Alcortin A Gel (not excluded for Active Players) © Econazole * Sinelee Patch * Rayos DR Ifyou take one of these drugs currently, you will be contacted directly with more information about your options. Dental Benefits. Yearly Coverage. The Plan will increase the yearly coverage amount from $5,000 for each covered Member and Dependent to $6,000. Fillings. The Plan will allow one filling per tooth every two years, Dentalimplants. The Plan will provide coverage for dental implants subject to rules set forth in the Plan. Crowns, The Plan will cover the replacement of a broken crown if that crown has been in place at least five (5) years. Page 5 of 6 B. Health Benefits Under the Active Plan for Active Players Onh Meath Benefits Under the Active Plan for Active Players Only Evaluations for Therapeutic Use Exemptions. The Plan will cover the neuropsychological testing that an Active Player needs to obtain a therapeutic use exemption for an ADD/ADHD medication with a $20 copayment. © Dependent Eligibility Under the Active Plan, the Buy-Up Plan, the Base Plan, and the Buy-Down Plan Children Under Guardianship. Effective April 1, 2017, an Active or inactive Member may cover a child for whom the Member is a legal guardian provided that the child lives with the Member and the Member pays for at least half of the child's financial support. Dependent Verification. Effective April 1, 2017, the Plan will bolster the verification Process for enrollment of spouses and dependents (e.g, requiring production of marriage licenses, birth certificates, and other documents as appropriate at time of enrollment), subject to reasonable accommodations for Active More information will be provided before the audit begins.
