comparative data and reporting tools to help faculty practice organizations identify
opportunities to increase the efficiency and productivity of clinical practice. The
FPSC's web-based tools provide actionable comparative data in the areas of
clinical performance (measure and manage clinical activity, understand visit mix,
optimize practice yield, monitor coding compliance and improve charge capture)
and revenue cycle management (reduce and prevent denials, ensure maximal
collections, improve timeliness of collections, reduce days in accounts receivable,
and identify undercharges).
In collaboration
with UHC and the AAMC, FPSC members have access to the largest informal

ID Database Name Database Description

An all-payer database of inpatient care provided to children in the United States

designed for researchers to study a range of topics including the economic burden
of pediatric conditions, access to services, quality of care and patient safety, and
the impact of health policy changes. It was developed as part of HCUP to analyze
national estimates of hospital utilization, charges, quality, and outcomes for
children (ages 20 and younger). The KID contains aggregated hospital discharge
HCUP Kids' Inpatient data from all states participating in HCUP (varies by year) but sampling and
1012 Database (KID) weighting enables national estimates.

The largest all-payer emergency department (ED) database in the United States,
including data on both ED visits that result in an admission, and those that do not.
It is developed as part of HCUP for analysis of geographic characteristics, hospital
characteristics, patient characteristics, and the nature of visits (e.g., common
reasons for ED visits, including injuries). The NEDS includes ED charge
HCUP Nationwide Emergency information for over 75% of patients, regardless of payer, including patients
1013 Department Sample (NEDS) covered by Medicaid, private insurance, and the uninsured.

The largest all-payer inpatient care database in the United States. It can be used
to identify, track, and analyze national trends in health care utilization, access,
charges, quality, and outcomes. Inpatient stay records include clinical and
resource use information typically available from discharge abstracts. The NIS
includes charge information for all patients, regardless of payer, including persons
covered by Medicare including Medicare Advantage, Medicaid, private insurance,
and the uninsured. The NIS' large sample size enables analyses of rare conditions,
HCUP Nationwide Inpatient such as specific types of cancer; uncommon treatments, such as organ

State Ambulatory Surgery Databases (SASD) are outpatient databases from Data
Organizations in participating HCUP States; these databases capture surgeries
performed on the same day in which patients are admitted and released. The
SASD contain the ambulatory surgery encounter abstracts in participating States,
translated into a uniform format to facilitate multi-state comparisons and analyses.
All of the databases include abstracts from hospital-affiliated ambulatory surgery
sites. Some contain the universe of ambulatory surgery encounter abstracts for
that state, including records from both hospital-affiliated and freestanding surgery
centers. Composition and completeness of data files may vary from state to state.
Researchers and policymakers use SASD to compare inpatient surgery data with
ambulatory surgery data; identify State-specific trends in ambulatory surgery

The State Emergency Department Databases (SEDD) are a powerful set of

databases, from data organizations in participating States, that capture discharge
information on all emergency department visits that do not result in an admission.
Information on patients initially seen in the emergency room and then admitted to
the hospital is included in the State Inpatient Databases (SID). SEDD data files
beginning in data year 1999 are available through the HCUP Central Distributor.
The SEDD contain the emergency department encounter abstracts in participating
States, translated into a uniform format to facilitate multi-state comparisons and
analyses. All of the databases include abstracts from hospital-affiliated emergency
department sites. Composition and completeness of data files may vary from State
to State. The SEDD contain a core set of clinical and non-clinical information on all
patients, regardless of payer, including persons covered by Medicare, Medicaid,
HCUP State Emergency private insurance, and the uninsured. In addition to the core set of uniform data

The State Inpatient Databases (SID) are a powerful set of hospital databases from
data organizations in participating States. The SID contain the universe of the
inpatient discharge abstracts in participating States, translated into a uniform
format to facilitate multi-State comparisons and analyses. Together, the SID
encompass about 97 percent of all annual discharges in the U.S. Some States
include discharges from specialty facilities, such as acute psychiatric hospitals.
The SID contain a core set of clinical and nonclinical information on all patients,
regardless of payer, including persons covered by Medicare, Medicaid, private
insurance, and the uninsured. In addition to the core set of uniform data elements
common to all SID, some include other elements, such as the patient's race. The
SID contain clinical and resource use information included in a typical discharge
HCUP State Inpatient abstract, with safeguards to protect the privacy of individual patients, physicians,
1017 Databases (SID) and hospitals (as required by data sources).

Medicare HOS data files are available as public use files (PUFs), limited data sets
(LDSs), and research identifiable files (RIFs). HOS PUFs contain the majority of
the survey items collected on the HOS instrument. HOS LDSs and RIFs are
comprised of the entire national sample for a given cohort (including both
respondents and non-respondents), and contain all of the HOS survey items. The
HOS LDSs include plan identifiers as well as several additional variables
describing plan characteristics. They also contain protected beneficiary-level
Health Outcomes Survey health information such as date of birth; however, specific direct person identifiers
1018 (HOS) (i.e. name and health insurance claim number) are not included in LDSs.

The comparative database contains staff survey responses to the AHRQ

Hospital Survey on Patient sponsored Hospital Survey on Patient Safety Culture, which asks questions about
Safety Culture Comparative perceived patient safety culture in hospitals. Hospitals voluntarily administer
1019 Database Report surveys to their staff and AHRQ publishes bi-annual reports of survey results.
The Medicare Provider Analysis and Review (MEDPAR) file contains records for
100% of Medicare beneficiaries who received hospital inpatient services at a Long-
Term Care Hospital as defined in the Social Security Act. The file contains same
Long-Term Care Hospital data elements found in the National Hospital MEDPAR Limited Data Set (LDS),
(LTCH) PPS Expanded including the billed Diagnosis-Related Group (DRG), the current DRG and the
1020 Modified MEDPAR proposed/final DRG.

MarketScan HPM Database combines information on workplace absence, short-

term disability and worker's compensation with medical/surgical claims and
outpatient drug data. This includes employees with HPM data (users) as well as
employees who did not incur absence records, disability cases, or workers
compensation cases (non-users). The HPM Eligibility table can be used to
MarketScan Health and examine lack of HPM experience (absence, STD, LTD, WC) as an outcome for a
Productivity Management study. Database allows researchers to assess both the direct and indirect costs

The MPSMS is a nationwide surveillance system designed to identify rates of

specific adverse events within the hospitalized fee-for-service Medicare population.
An adverse event is defined as an unintended patient harm, injury or loss more
likely associated with the patient's interaction with the health care delivery system
than from diseases the patient may have. The surveillance system identifies
retrospectively adverse events from randomly selected inpatient Medicare
discharges and administrative data. MPSMS is a primary source of data for the
Medicare Patient Safety AHRQs annual National Healthcare Quality Report (NHQR) and the National

National Cardiovascular Data The PINNACLE (Practical Innovation and Clinical Excellence) Registry is the
1041 Registry (NCDR) - PINNACLE nations largest practice-based cardiovascular quality improvement registry.

Provides member hospitals with quarterly performance reports with comparison

data. Data are presented at the nursing care unit level, as well as for the hospital
as a whole. Categories of comparison data include, but are not limited to: hospital
staffed-bed size, Magnet(R) status, teaching status, ownership, geographic region,
National Database of Nursing state, and metropolitan status. Data extracts may be obtained by researchers
1042 Quality Indicators (NDNQI) approved by the American Nurses Association.

The Clinical Outcomes Research Initiative (CORI) was developed to study

outcomes of gastrointestinal (GI) endoscopic procedures in "real life" settings.
Physicians participating in the CORI consortium produce GI endoscopy reports
using a specialty electronic health record. Data from the reports are sent
National Endoscopic Database electronically to a central data repository where they are pooled with data from
1043 (NED) other consortium participants in the National Endoscopic Database (NED).

The National Healthcare Safety Network (NHSN) is a voluntary, secure, internet-

based surveillance system that integrates patient and healthcare personnel safety
systems. Personnel collect data on central line-associated primary bloodstream
infections (BSIs), ventilator-associated pneumonias, or urinary catheter associated
urinary tract infections (UTIs) that occur in patients staying in a patient care
locations. In NHSN, these locations are further characterized according to patient
National Healthcare Safety population: adults, children, or infants (in tables, pediatric and nursery locations
1044 Network (NHSN) are so noted).

The Outcome and Assessment Information Set (OASIS) is a group of data

elements that form the basis for measuring patient outcomes for purposes of
outcome-based quality improvement. Since 1999, CMS has required Medicare-
certified home health agencies to collect and transmit OASIS data for all adult
patients whose care is reimbursed by Medicare and Medicaid with the exception of
Outcome and Assessment patients receiving pre- or postnatal services only. Since 2008, OASIS includes
1051 Information Set (OASIS) process data items as well as outcome data items.

ID Database Name Database Description

Press Ganey Urgent Care Resource containing proprietary Urgent Care patient experience measures and
1075 Patient Experience Survey subscales.

Links two large population-based sources of data that provide detailed information
about Medicare beneficiaries with cancer. The data come from the Surveillance,
Epidemiology and End Results (SEER) program of cancer registries that collect
clinical, demographic and cause of death information for persons with cancer and
the Medicare claims for covered health care services from the time of a person's
Medicare eligibility until death. The SEER program collects information about
cancer site, stage, and histology for persons newly diagnosed with cancer who
reside in one of the SEER geographic areas. Medicares master enrollment file is
used to identify which persons in the SEER data are Medicare beneficiaries. For
people who are Medicare eligible, the SEER-Medicare data include claims for
SEER-Medicare Linked covered health care services, including hospital, physician, outpatient, home
1076 Database health, and hospice bills.

Skilled Nursing Facility (SNF)

MEDPAR Limited Data Set The file contains records for 100% of Medicare beneficiaries who use SNF
1077 (LDS) services including charge amounts, prescriptions and diagnosis codes.

Provides custom reporting capabilities to give users complete control of their data
analysis. Participants of the VQI may now select variables of interest and filter their
Society for Vascular Surgery data set to a desirable patient population for specific analyses. Additionally, users
1078 Vascular Quality Initiative are able to benchmark results with their peers for further comparison of outcomes.

The Leapfrog Hospital Survey looks at hospital quality and safety, focusing on
hospital structures, processes of care, and outcomes. Hospitals voluntarily self-
1080 The Leapfrog Hospital Survey report their status on patient safety and safety of selected procedures.
The Clinical Data Base (CDB) is a web-enabled resource that consists of two main
portals: the Report EXPRESS and the Report BUILDER. Submitted on a monthly
basis, data are passed through UHCs robust editing and proprietary risk
adjustment methodologies. Upon acceptance, data is used to populate up to three
UHC database products, depending on the level of participation, including Clinical
Data Base, Core Measures and Resource Manager. Databases are generally
populated within 24 48 hours of receipt. Member hospitals can begin submitting
data as early as two weeks after the close of a month, but typically participants
submit 30 days after the close of each month. The Clinical Data Base provides an
expanded set of comparative data and analytic tools to support the clinical
operations of member institutions. Opportunities for improvement are identified by
combining patient encounter level and line-item transactional detail to yield
information on patient outcomes. Users also have access to a portal called The
Physician Insight Tool which allows users to create by-physician reports. This
functionality permits a user to profile physicians and benchmark internally or
externally on over the same 100 variables found in the CDB. Users can run reports
for individual physicians and groups of physicians with the ability to benchmark
1081 UHC Clinical Data Base against two internal or two external comparison groups.

In addition to the CDB, which can be used for outcome and comparative reporting,
the Resource Manager (CDB-RM) database provides additional capabilities
necessary to track data needed for evaluating resource utilization and associated
outcomes at a greater depth. The CDB-RM application builds upon the CDB
foundation to provide line-item information in pharmacy, blood products, imaging
and diagnostics, accommodations (for all unit types, including intensive care,
cardiac care, and step-down units), laboratory, medical/surgical supplies.
Hospitals use this information to standardize resource use, identify trends in
practice patterns, and advance cost management initiatives. More broadly, users
can view detailed resource utilization for patient encounters, including estimated
1083 UHC Resource Manager drug costs and product and accommodation use.
