The largest all-payer emergency department (ED) database in the United States,
including data on both ED visits that result in an admission, and those that do not.
It is developed as part of HCUP for analysis of geographic characteristics, hospital
characteristics, patient characteristics, and the nature of visits (e.g., common
reasons for ED visits, including injuries). The NEDS includes ED charge
HCUP Nationwide Emergency information for over 75% of patients, regardless of payer, including patients

The largest all-payer inpatient care database in the United States. It can be used
to identify, track, and analyze national trends in health care utilization, access,
charges, quality, and outcomes. Inpatient stay records include clinical and

includes charge information for all patients, regardless of payer, including persons
covered by Medicare including Medicare Advantage, Medicaid, private insurance,
and the uninsured. The NIS' large sample size enables analyses of rare conditions,

State Ambulatory Surgery Databases (SASD) are outpatient databases from Data
Organizations in participating HCUP States; these databases capture surgeries
performed on the same day in which patients are admitted and released. The
SASD contain the ambulatory surgery encounter abstracts in participating States,
translated into a uniform format to facilitate multi-state comparisons and analyses.
All of the databases include abstracts from hospital-affiliated ambulatory surgery
sites. Some contain the universe of ambulatory surgery encounter abstracts for
that state, including records from both hospital-affiliated and freestanding surgery
centers. Composition and completeness of data files may vary from state to state.
Researchers and policymakers use SASD to compare inpatient surgery data with
ambulatory surgery data; identify State-specific trends in ambulatory surgery
HCUP State Ambulatory utilization, access, charges, and outcomes; and conduct market area research and

The State Inpatient Databases (SID) are a powerful set of hospital databases from
data organizations in participating States. The SID contain the universe of the
inpatient discharge abstracts in participating States, translated into a uniform
format to facilitate multi-State comparisons and analyses. Together, the SID
encompass about 97 percent of all annual discharges in the U.S. Some States
include discharges from specialty facilities, such as acute psychiatric hospitals.
The SID contain a core set of clinical and nonclinical information on all patients,
regardless of payer, including persons covered by Medicare, Medicaid, private
insurance, and the uninsured. In addition to the core set of uniform data elements
common to all SID, some include other elements, such as the patient's race. The
SID contain clinical and resource use information included in a typical discharge
HCUP State Inpatient abstract, with safeguards to protect the privacy of individual patients, physicians,

Medicare HOS data files are available as public use files (PUFs), limited data sets
(LDSs), and research identifiable files (RIFs). HOS PUFs contain the majority of
the survey items collected on the HOS instrument. HOS LDSs and RIFs are
comprised of the entire national sample for a given cohort (including both
respondents and non-respondents), and contain all of the HOS survey items. The
HOS LDSs include plan identifiers as well as several additional variables
describing plan characteristics. They also contain protected beneficiary-level
Health Outcomes Survey health information such as date of birth; however, specific direct person identifiers

The Leapfrog Hospital Survey looks at hospital quality and safety, focusing on
hospital structures, processes of care, and outcomes. Hospitals voluntarily self-

The Clinical Data Base (CDB) is a web-enabled resource that consists of two main
portals: the Report EXPRESS and the Report BUILDER. Submitted on a monthly
basis, data are passed through UHCs robust editing and proprietary risk
adjustment methodologies. Upon acceptance, data is used to populate up to three
UHC database products, depending on the level of participation, including Clinical
Data Base, Core Measures and Resource Manager. Databases are generally
populated within 24 48 hours of receipt. Member hospitals can begin submitting
data as early as two weeks after the close of a month, but typically participants
submit 30 days after the close of each month. The Clinical Data Base provides an
expanded set of comparative data and analytic tools to support the clinical
operations of member institutions. Opportunities for improvement are identified by
combining patient encounter level and line-item transactional detail to yield
information on patient outcomes. Users also have access to a portal called The
Physician Insight Tool which allows users to create by-physician reports. This
functionality permits a user to profile physicians and benchmark internally or
externally on over the same 100 variables found in the CDB. Users can run reports
for individual physicians and groups of physicians with the ability to benchmark

completing Core Measures data. Core Measures Reporting to The Joint
Commission (TJC) and Centers for Medicare and Medicaid Services (CMS) is

costs. The UHC Clinical Data Base Fast Track product has met the criteria for
inclusion in the accreditation process and is included on The Joint Commissions

the Resource Manager (CDB-RM) database provides additional capabilities
necessary to track data needed for evaluating resource utilization and associated
outcomes at a greater depth. The CDB-RM application builds upon the CDB
foundation to provide line-item information in pharmacy, blood products, imaging
and diagnostics, accommodations (for all unit types, including intensive care,
cardiac care, and step-down units), laboratory, medical/surgical supplies.
Hospitals use this information to standardize resource use, identify trends in
practice patterns, and advance cost management initiatives. More broadly, users
can view detailed resource utilization for patient encounters, including estimated
