CONTRACT OF SERVICE
KNOW ALL MEN BY THESE PRESENTS:

This contract of service entered into and executed this _____ day of _______, 2017 at the Kidapawan
City, Philippines by and between:

Department of Labor and Employment- Regional Office No. XII with office address at 102
Acepal Bldg., Mabini Ext., Koronadal City herein represented by REGIONAL DIRECTOR SISINIO B. CANO, CESO
IV hereinafter referred to as the DOLE RO XII;

-and-

TUPAD Workers, all of legal ages, Filipinos and residents of ______________________,

__________________, North Cotabato Province, herein represented as the _________________;

-WITNESSETH-

That pursuant to the provisions of DOLE Department Order No. 137-14,series of 2014, authorizing the
contracting of service in the DOLE-RO.XII to implement the Emergency Employment Program also called as
Tulong Panghanapbuhay sa Ating Displaced/Disadvantaged Workers (TUPAD), to enable the displaced
workers, the underemployed and the unemployed poor, to engage in short-term community works to
provide them with emergency/temporary wage employment, the following terms and conditions are hereby
set:

1. That the ____________ are physically fit to perform LABOR-BASED SERVICES, in accordance with the
requirements of the job;

2. That in view hereof, the Proponent are hereby contracted to perform the following works of
Streets, drainage canals, schools, Brgy. Halls, Hospital and other public areas maintenance of cleanliness
for 15 days covering the period ______, 2017 to _____, 2017, with a daily wage rate of Php295.00
(prevailing minimum wage in the RO) chargeable against the Special Program Fund. Absences,
tardiness or under-time shall be deducted accordingly; and

3. That notwithstanding the fixed duration of the employment, the contract of service,
may be terminated anytime by the First Party for just cause such as but not limited to failure to failure
meet the required outputs

City, North Cotabato.
