CONTRACT AGREEMENT BETWEEN PREMIER DJ SERVICES

Specifics:
(a) Any overtime requested should be discussed prior to closing of said entertainment at a rate of an
additional $75.00 per hour.
(b) DJ will arrive at the event location at least one hour prior to the event
(c) In the event of a cancellation or postponement Premier Services DJs must be notified in writing
not later than 30 days prior to contracted date of entertainment. If written notification of
cancellation or postponement is not received 30 days prior to date of entertainment, deposit will be
forfeited.
(d) Once the contract is signed below by both parties and deposited has been received, the said date
will be officially confirmed and reserved on Premier DJ Services operating calendar.
(e) Please sign and mail both copies along with your deposit made payable to Roseal F. Fowlkes to the
below address. Premier DJs will then sign and send your executed original copy of the contract
back to you and your event will be confirmed and reserved onto our operating calendar.
