FATALITY FABRICATION OPERATIONS

What Happened?
During fabrication of mooring piles using Welding Rotator equipment, an employee was
struck by a tubular and sustained fatal injuries.

Reminder SEB Key Operating Principles

The correct equipment must be available, planned for and then used for the task
Equipment shall be used within its operating limits
Equipment shall have accurate and available User SOPs or Guidelines consistent with
manufacturer instructions, describing equipment operating limits, set-up parameters
and hazard precautions
Identify and discuss openly Line of Fire hazards when planning and briefing a job
Employ active, not passive, supervision
